    Diabetic Gastroparesis: A Risk Factor for Perioperative Aspiration

    Diabetic gastroparesis is one of several long-term complications of diabetes mellitus. It is characterized by delayed gastric emptying secondary to autonomic neuropathy. As the incidence of diabetes mellitus continues to rise, it becomes even more important for anesthetists to address the risk of aspiration posed by diabetic gastroparesis. Currently, aspiration of gastric contents accounts for nearly half of all major airway complications in the operating room, often due to incomplete assessment of risk factors. Understanding the pathophysiology is the first step in recognizing diabetic gastroparesis as a significant risk factor for aspiration. Every effort should be made to mitigate perioperative aspiration, starting with the ability to identify risk factors where they exist

    Person-Environment Fit, Substance Use Attitudes, and Early Adolescent Substance Use

    BACKGROUND: Adolescent substance use is a national health concern. While the literature is clear on the prevalence of substance use during the adolescent developmental period, a dearth of literature is available on the developmental contexts, particularly the influence of school factors, in which substance use occurs. OBJECTIVES: This study examined the intermediary role of substance use attitudes on the relationship between school racial composition and alcohol and marijuana use in a sample of sixth to eighth graders. METHODS: The sample consisted of 4228 middle school students (89% White; 53% female) in the Midwest. A moderated mediation analysis was conducted on the relationship between school racial composition, substance use attitudes, and substance use, with race as the moderator. RESULTS: Results indicated a significant relationship between the percentage of White or Black students in a school and alcohol or marijuana use and that this relationship was mediated by substance use attitudes. These relationships did not differ significantly by student race. Conclusions/Importance: Preliminary findings indicate the importance of considering school racial composition as a systems level risk or promotive factor for attitudes toward substance use as well as substance use outcomes

    Detection of Galba truncatula, Fasciola hepatica and Calicophoron daubneyi environmental DNA within water sources on pasture land, a future tool for fluke control?

    Abstract Background: Increasing trematode prevalence and disease occurrence in livestock is a major concern. With the global spread of anthelmintic resistant trematodes, future control strategies must incorporate approaches focusing on avoidance of infection. The reliance of trematodes on intermediate snail hosts to successfully complete their life-cycle means livestock infections are linked to the availability of respective snail populations. By identifying intermediate snail host habitats, infection risk models may be strengthened whilst farmers may confidently apply pasture management strategies to disrupt the trematode life-cycle. However, accurately identifying and mapping these risk areas is challenging. Methods: In this study, environmental DNA (eDNA) assays were designed to reveal Galba truncatula, Fasciola hepatica and Calicophoron daubneyi presence within water sources on pasture land. eDNA was captured using a filter-based protocol, with DNA extracted using the DNeasy? PowerSoil? kit and amplified via PCR. In total, 19 potential G. truncatula habitats were analysed on four farms grazed by livestock infected with both F. hepatica and C. daubneyi. Results: Galba truncatula eDNA was identified in 10/10 habitats where the snail was detected by eye. Galba truncatula eDNA was also identified in four further habitats where the snail was not physically detected. Fasciola hepatica and C. daubneyi eDNA was also identified in 5/19 and 8/19 habitats, respectively. Conclusions: This study demonstrated that eDNA assays have the capabilities of detecting G. truncatula, F. hepatica and C. daubneyi DNA in the environment. Further assay development will be required for a field test capable of identifying and quantifying F. hepatica and C. daubneyi infection risk areas, to support future control strategies. An eDNA test would also be a powerful new tool for epidemiological investigations of parasite infections on farms.publishersversionPeer reviewe

    Unsupervised Machine Learning for Identifying Challenging Behavior Profiles to Explore Cluster-Based Treatment Efficacy in Children With Autism Spectrum Disorder: Retrospective Data Analysis Study

    Background: Challenging behaviors are prevalent among individuals with autism spectrum disorder; however, research exploring the impact of challenging behaviors on treatment response is lacking. Objective: The purpose of this study was to identify types of autism spectrum disorder based on engagement in different challenging behaviors and evaluate differences in treatment response between groups. Methods: Retrospective data on challenging behaviors and treatment progress for 854 children with autism spectrum disorder were analyzed. Participants were clustered based on 8 observed challenging behaviors using k means, and multiple linear regression was performed to test interactions between skill mastery and treatment hours, cluster assignment, and gender. Results: Seven clusters were identified, which demonstrated a single dominant challenging behavior. For some clusters, significant differences in treatment response were found. Specifically, a cluster characterized by low levels of stereotypy was found to have significantly higher levels of skill mastery than clusters characterized by self-injurious behavior and aggression (P\u3c.003). Conclusions: These findings have implications on the treatment of individuals with autism spectrum disorder. Self-injurious behavior and aggression were prevalent among participants with the worst treatment response, thus interventions targeting these challenging behaviors may be worth prioritizing. Furthermore, the use of unsupervised machine learning models to identify types of autism spectrum disorder shows promise

    Uric Acid-Induced Adipocyte Dysfunction Is Attenuated by HO-1 Upregulation: Potential Role of Antioxidant Therapy to Target Obesity

    Increased uric acid levels have been implicated in the pathogenesis of metabolic syndrome. To examine the mechanisms by which this occurs, we hypothesized that an increase in heme oxygenase 1, a potent antioxidant gene, will decrease uric acid levels and adipocyte dysfunction via suppression of ROS and xanthine oxidase (XO) levels. We examined the effect of uric acid on adipogenesis in human mesenchymal stem cells (MSCs) in the presence and absence of cobalt protoporphyrin (CoPP), an HO-1 inducer, and tin mesoporphyrin (SnMP), an HO activity inhibitor. Uric acid increased adipogenesis by increasing NADPH oxidase expression and elevation in the adipogenesis markers C/EBPα, PPARγ, and Mest, while decreasing small lipid droplets and Wnt10b levels. We treated MSCs with fructose, a fuel source that increases uric acid levels. Our results showed that fructose increased XO expression as compared to the control and concomitant treatment with CoPP significantly decreased XO expression and uric acid levels. These beneficial effects of CoPP were reversed by SnMP, supporting a role for HO activity in mediating these effects. These findings demonstrate that increased levels of HO-1 appear crucial in modulating the phenotype of adipocytes exposed to uric acid and in downregulating XO and NADPH oxidase levels

    Stone artefacts on the seabed at a submerged freshwater spring confirm a drowned cultural landscape in Murujuga, Western Australia

    We report the discovery and identification of five ancient stone artefacts associated with a submerged freshwater spring at the underwater archaeological site WH1 in Murujuga (Dampier Archipelago), Western Australia. A limiting date applied to the site based on timing of inundation suggests it was occupied in the Late Pleistocene or Early Holocene. The site is situated well below the intertidal zone having been recorded at 14 m depth in Flying Foam Passage. This discovery highlights the high potential of these submerged springs as archaeological survey targets. We discuss results of a recent survey that expands the number of confirmed artefacts located at WH1 and the geomorphological context in a large calcareous depression associated with a freshwater source. This study demonstrates how submerged landscape research using a suite of technologies can reveal archaeological assemblages in this tropical geomorphological environment, and that adapted techniques could be applied to other tropical conditions such as mangrove coasts, large deltaic plains, or reef-building environments. There are likely thousands of drowned archaeological sites on the continental shelf of the tropics, extending from the intertidal zone to the lowest point of the culturally occupied landmass, at approximately 130 m below modern sea level

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy

    TESS delivers its first Earth-sized planet and a warm sub-Neptune

    The future of exoplanet science is bright, as TESS once again demonstrates with the discovery of its longest-period confirmed planet to date. We hereby present HD 21749b (TOI 186.01), a sub-Neptune in a 36-day orbit around a bright (V = 8.1) nearby (16 pc) K4.5 dwarf. TESS measures HD21749b to be 2.610.16+0.17^{+0.17}_{-0.16} RR_{\oplus}, and combined archival and follow-up precision radial velocity data put the mass of the planet at 22.71.9+2.222.7^{+2.2}_{-1.9} MM_{\oplus}. HD 21749b contributes to the TESS Level 1 Science Requirement of providing 50 transiting planets smaller than 4 RR_{\oplus} with measured masses. Furthermore, we report the discovery of HD 21749c (TOI 186.02), the first Earth-sized (Rp=0.8920.058+0.064RR_p = 0.892^{+0.064}_{-0.058} R_{\oplus}) planet from TESS.
