    Quality of Life of Patients with Head and Neck Cancer Receiving Cetuximab, Fluorouracil, Cisplatin Comparing to Cetuximab, Fluorouracil, Cisplatin, and Docetaxel within the CEFCID Trial

    Introduction: CeFCiD was a multicenter phase II study comparing the efficacy of cetuximab (C), 5-flourouracil, and cisplatin with the same regimen adding docetaxel (D) in recurrent/metastatic head and neck cancer. The primary analysis trial did not demonstrate survival benefit from therapy intensification in first-line recurrent and/or metastatic squamous cell carcinoma of the head and neck (SCCHN). The current analysis of the trial assessed the impact of treatment on quality of life (QoL). Methods: The European Organization for Research and Treatment of Cancer Quality of life Questionnaire QLQ-C30 and the tumor-specific module for head and neck cancer (QLQ-H&N35) were used to assess QoL at baseline (visit 1), after 2 (visit 3), 4 (visit 5), and 6 (visit 7) cycles of chemotherapy. Results: Of 180 patients included in this study, 86 patients (47.8%) completed the questionnaires at baseline. Considering selected scores over treatment time, there was no difference in global QoL, dyspnea, swallowing, and speech between the treatment arms in the course. For fatigue, a significant increase from baseline to visit 3 (p = 0.02), visit 5 (p = 0.002), and to visit 7 (p = 0.003) was observed for patients receiving D, cisplatin or carboplatin (P), 5-fluorouracil (F), and C. At the end of chemotherapy, the manifestation of fatigue was similar compared in the 2 treatment arms. Discussion/Conclusion: Therapy intensification not adversely affects selected scores of QoL of patients with recurrent and/or metastatic SCCHN.     Examination of Late Palaeolithic archaeological sites in northern Europe for the preservation of cryptotephra layers

    We report the first major study of cryptotephra (non-visible volcanic ash layers) on Late Palaeolithic archaeological sites in northern Europe. Examination of 34 sites dating from the Last Termination reveals seven with identifiable cryptotephra layers. Preservation is observed in minerogenic and organic deposits, although tephra is more common in organic sediments. Cryptotephra layers normally occur stratigraphically above or below the archaeology. Nearby off-site palaeoclimate archives (peat bogs and lakes <0.3 km distant) were better locations for detecting tephra. However in most cases the archaeology can only be correlated indirectly with such cryptotephras. Patterns affecting the presence/absence of cryptotephra include geographic position of sites relative to the emitting volcanic centre; the influence of past atmospherics on the quantity, direction and patterns of cryptotephra transport; the nature and timing of local site sedimentation; sampling considerations and subsequent taphonomic processes. Overall, while tephrostratigraphy has the potential to improve significantly the chronology of such sites many limiting factors currently impacts the successful application

    Saliva as a biomarker for head and neck squamous cell carcinoma: In vitro detection of cytotoxic effects by using the plating efficiency index

    Worldwide, the incidence of head and neck squamous cell carcinoma (HNSCC) is increasing. The development of an effective prevention program would provide a promising opportunity to control this disease. We applied the 'plating efficiency index' of Chinese hamster lung fibroblasts to demonstrate the presence of cytotoxic effects in saliva samples from cancer patients as well as from healthy probands. Correlations between individual risk factors and the cytotoxic effects of saliva specimens were analysed and evaluated. Saliva samples were obtained from male patients (n=43) with carcinomas of the upper aerodigestive tract, and from a healthy age-matched male control group (n=131) with different tobacco and alcohol consumption habits. The extraction of non-stimulated sober saliva was chosen for this investigation. In vitro cytotoxicity of the saliva was detected using the permanent mammalian cell line V79 (lung fibroblasts) of Chinese hamsters. The determination of the plating efficiency index was made in the logarithmic growth phase of the initial cell culture. A reduction in cell colonies (plating efficiency) of at least 50% was defined as strongly cytotoxic. A significant direct correlation was found between daily tobacco consumption and the reduction of plating efficiency (p < 0.0001). We found only a moderate increase in the cytotoxicity of tested saliva samples in correlation with daily alcohol uptake. The difference between tumor patients and healthy probands was highly significant (X-2 test; p=0.001). Using the method of logistic regression analysis, we found a 3.6-fold increased cancer risk in probands with cytotoxic saliva (p < 0.001). According to our results, the plating efficiency index seems to be a suitable method for the detection of increased cancer risk. In combination with several effective biomarkers on cytogenetic end-points, it may help to establish biomonitoring programs for secondary cancer prevention

    Human-environment interaction in the hinterland of Ephesos - As deduced from an in-depth study of Lake Belevi, west Anatolia

    Lacustrine sediments are important archives for high resolution palaeoenvironmental reconstructions of the Holocene. Despite the density of ancient cities and settlements along the western coast of Turkey, the archives from coastal lakes in this area have until now not been recognized to their fullest potential and are, therefore, only poorly studied. The exceptional geo-bio-archive of Lake Belevi is located close to the ancient city of Ephesos in western Turkey. Two sediment cores have been analysed using geochemical, sedimentological, microfaunal, palynological, and archaeoparasitological methods. The in-depth study of these Holocene deposits is supported by a robust age-depth model that used 33 radiocarbon dates and tephrochronology. The results reveal the existence of a freshwater lake in the Early Holocene which turned brackish when the rising sea level connected it with the sea. The delta evolution of the Kucuk Menderes led to the reestablishment of a freshwater lake. The natural vegetation was represented by open oak woodlands. There are hints for first agricultural activities in the environs of Belevi as early as 8000 cal yr BP. Intensive cultivation of Olea is proven since 3000 cal yr BP. Starting during the 3rd millennium BP, the human impact with enhanced deforestation activities and correlative high sedimentation rates is attested for sites such as Belevi (Ephesos), Elaia (Pergamon) and Miletos. For the first time, tephra from the eruption of Minoan Santorini has been identified in the environs of Ephesos. This ash covered the vegetation by a thick layer, wherefore low-growing plants were strongly affected. The comparison between the results from the quasi natural area of Lake Belevi and the area around the city of Ephesos gives insights into the development and use of the landscapes, the environmental changes as well as the duration and intensity of the human impact.
