    Reduced referral and case fatality rates for severe symptomatic hyperlactataemia in a South African public sector antiretroviral programme: a retrospective observational study

    <p>Abstract</p> <p>Background</p> <p>Interventions to promote prevention and earlier diagnosis of severe symptomatic hyperlactataemia (SHL) were implemented in the Western Cape provincial antiretroviral programme (South Africa) from 2004. Interventions included clinician education, point-of-care lactate meters, switch from stavudine to zidovudine in high risk patients and stavudine dose reduction. This study assessed trends in referral rate, severity at presentation and case fatality rate for severe SHL.</p> <p>Methods</p> <p>Retrospective study of severe SHL cases diagnosed at a referral facility from 1 January 2003 to 31 December 2008. Severe SHL was defined as patients with compatible symptoms and serum lactate ā‰„ 5 mmol/l attributable to antiretroviral therapy (ART). Cumulative ART exposure at referring ART clinics was used to calculate referral rates.</p> <p>Results</p> <p>There were 254 severe SHL cases. The referral rate (per thousand patient years [py] ART exposure) peaked in 2005 (20.4/1000py), but fell to 1.3/1000py by 2008 (incidence rate ratio [IRR] = 0.07, 95%CI 0.04-0.11). In 2003, 66.7% of cases presented with a standard bicarbonate (SHCO<sub>3</sub>) level <15 mmol/l, but this fell to 12.5% by 2008 (p for trend < 0.001). Case fatality rate fell from a peak of 33.3% in 2004 to 0% in 2008 (p for trend = 0.002).</p> <p>Conclusions</p> <p>These trends suggest the interventions were associated with reduced referral, less severe metabolic acidosis at presentation and improved survival.</p

    Delayed Goblet Cell Hyperplasia, Acetylcholine Receptor Expression, and Worm Expulsion in SMC-Specific IL-4RĪ±ā€“Deficient Mice

    Interleukin 4 receptor Ī± (IL-4RĪ±) is essential for effective clearance of gastrointestinal nematode infections. Smooth muscle cells are considered to play a role in the type 2 immune responseā€“driven expulsion of gastrointestinal nematodes. Previous studies have shown in vitro that signal transducer and activator of transcription 6 signaling in response to parasitic nematode infection significantly increases smooth muscle cell contractility. Inhibition of the IL-4RĪ± pathway inhibits this response. How this response manifests itself in vivo is unknown. In this study, smooth muscle cell IL-4RĪ±ā€“deficient mice (SM-MHC(Cre)IL-4RĪ±(āˆ’/lox)) were generated and characterized to uncover any role for IL-4/IL-13 in this nonā€“immune cell type in response to Nippostrongylus brasiliensis infection. IL-4RĪ± was absent from Ī±-actinā€“positive smooth muscle cells, while other cell types showed normal IL-4RĪ± expression, thus demonstrating efficient cell-typeā€“specific deletion of the IL-4RĪ± gene. N. brasiliensisā€“infected SM-MHC(Cre)IL-4RĪ±(āˆ’/lox) mice showed delayed ability to resolve infection with significantly prolonged fecal egg recovery and delayed worm expulsion. The delayed expulsion was related to a delayed intestinal goblet cell hyperplasia, reduced T helper 2 cytokine production in the mesenteric lymph node, and reduced M3 muscarinic receptor expression during infection. Together, these results demonstrate that in vivo IL-4RĪ±ā€“responsive smooth muscle cells are beneficial for N. brasiliensis expulsion by coordinating T helper 2 cytokine responses, goblet hyperplasia, and acetylcholine responsiveness, which drive smooth muscle cell contractions

    Rediscovery of Anacampsis lupinella Busck (Lepidoptera: Gelechiidae) in Ontario

    We rediscovered populations of Anacampsis lupinella Busck (Lepidoptera: Gelechiidae) in three remnant populations of sundial lupines (Lupinus perennis L. (Fabaceae)) in southern Ontario. Here we provide a detailed description of the species and illustrate it with figures of adult moths and both male and female genitalia. Photographs of the moths taken recently in High Park, Toronto, match photos of the type specimen that was collected at High Park in 1901. The male genitalia of contemporary males match those of a male from the original series of specimens collected in Toronto. CO1DNA barcodes of eight contemporary specimens were very similar to the CO1barcode recovered from a specimen from the original series. The CO1sequences demonstrated slight genetic differentiation of the three Ontario populations, but collectively were clearly delimited from the seven congeneric species known from Ontario

    This Executive Summary of the Global Strategy for the Diagnosis, Management, and Prevention of COPD (GOLD) 2017 Report focuses primarily on the revised and novel parts of the document. The most significant changes include: i) the assessment of COPD has been refined to separate the spirometric assessment from symptom evaluation. ABCD groups are now proposed to be derived exclusively from patient symptoms and their history of exacerbations; ii) for each of the groups A to D, escalation strategies for pharmacological treatments are proposed; iii) the concept of de-escalation of therapy is introduced in the treatment assessment scheme; iv) nonpharmacologic therapies are comprehensively presented and; v) the importance of comorbid conditions in managing COPD is reviewed
