Magnetic Phase Transitions in the double spin-chains compound
We report high-resolution x-ray diffraction, muon-spin-rotation spectroscopic
and specific heat measurements in the double spin-chains compound . The x-ray diffraction results show that the crystal structure of
~is orthorhombic down to T=10K. Anisotropic line-broadening of
the diffraction peaks is observed, indicating disorder along the spin chains.
Muon spin relaxation and specific heat measurements show that
\~undergoes a phase transition to a magnetic ordered state at .
The specific heat data exhibits a second -like peak at , which increases with increasing magnetic field similarly way to

Outcome calculations based on nursing documentation in the first generation of electronic health records in the Netherlands
OBJECTIVES: Previous studies regarding nursing documentation focused primarily on documentation quality, for instance, in terms of the accuracy of the documentation. The combination between accuracy measurements and the quality and frequencies of outcome variables such as the length of the hospital stay were only minimally addressed. METHOD: An audit of 300 randomly selected digital nursing records of patients (age of >70 years) admitted between 2013-2014 for hip surgery in two orthopaedic wards of a general Dutch hospital was conducted. RESULTS: Nursing diagnoses: Impaired tissue perfusion (wound), Pressure ulcer, and Deficient fluid volume had significant influence on the length of the hospital stay. CONCLUSION: Nursing process documentation can be used for outcome calculations. Nevertheless, in the first generation of electronic health records, nursing diagnoses were not documented in a standardized manner (First generation 2010-2015; the first generation of electronic records implemented in clinical practice in the Netherlands)
Reversible cerebral ischemia in patients with pheochromocytoma
Cerebral ischemia and symptoms of stroke can occur as a rare manifestation in patients with pheochromocytoma. We describe a 45-year-old woman who was admitted because of a right-sided hemiparesis due to an ischemic lesion in the left hypothalamus. The clinical diagnosis of a pheochromocytoma was proven by highly elevated urinary catecholamines and confirmed histologically after operation. The successful removal of the tumor led to the almost complete recovery of the neurological deficiencies. It is of vital importance to know this atypical presentation of pheochromocytoma. The diagnosis of pheochromocytoma should be suspected in patients with focal cerebral symptoms, particularly in the presence of intermittent hypertension or other paroxysmal symptoms suggestive of pheochromocytom
High incidence of medication documentation errors in a Swiss university hospital due to the handwritten prescription process
BACKGROUND: Medication errors have been reported to be a leading cause of death in hospitalized patients. In this study we focused on identifying and quantifying errors in the handwritten drug ordering and dispensing documentation processes which could possibly lead to adverse drug events. METHODS: We studied 1,934 ordered agents (165 consecutive patients) retrospectively for medication documentation errors. Errors were categorized into: Prescribing errors, transcription errors and administration documentation errors on the nurses' medication lists. The legibility of prescriptions was analyzed to explore its possible influence on the error rate in the documentation process. RESULTS: Documentation errors occurred in 65 of 1,934 prescribed agents (3.5%). The incidence of patient charts showing at least one error was 43%. Prescribing errors were found 39 times (37%), transcription errors 56 times (53%), and administration documentation errors 10 times (10%). The handwriting readability was rated as good in 2%, moderate in 42%, bad in 52%, and unreadable in 4%. CONCLUSIONS: This study revealed a high incidence of documentation errors in the traditional handwritten prescription process. Most errors occurred when prescriptions were transcribed into the patients' chart. The readability of the handwritten prescriptions was generally bad. Replacing the traditional handwritten documentation process with information technology could potentially improve the safety in the medication process
Crystal Symmetry Lowering in Chiral Multiferroic BaTaFeSiO observed by X-Ray Magnetic Scattering
Chiral multiferroic langasites have attracted attention due to their
doubly-chiral magnetic ground state within an enantiomorphic crystal. We report
on a detailed resonant soft X-ray diffraction study of the multiferroic
BaTaFeSiO at the Fe and oxygen edges. Below
() we observe the satellite reflections ,
, and where . The dependence of the scattering intensity on X-ray polarization and
azimuthal angle indicate that the odd harmonics are dominated by the
out-of-plane (-axis) magnetic dipole while the
originates from the electron density distortions accompanying magnetic order.
We observe dissimilar energy dependences of the diffraction intensity of the
purely magnetic odd-harmonic satellites at the Fe edge. Utilizing
first-principles calculations, we show that this is a consequence of the loss
of threefold crystal symmetry in the multiferroic phase
Nonlinear electron-phonon coupling in doped manganites
We employ time-resolved resonant x-ray diffraction to study the melting of
charge order and the associated insulator-metal transition in the doped
manganite PrCaMnO after resonant excitation of a
high-frequency infrared-active lattice mode. We find that the charge order
reduces promptly and highly nonlinearly as function of excitation fluence.
Density functional theory calculations suggest that direct anharmonic coupling
between the excited lattice mode and the electronic structure drive these
dynamics, highlighting a new avenue of nonlinear phonon control
