Serum Albumin Is Inversely Associated With Portal Vein Thrombosis in Cirrhosis

We analyzed whether serum albumin is independently associated with portal vein thrombosis (PVT) in liver cirrhosis (LC) and if a biologic plausibility exists. This study was divided into three parts. In part 1 (retrospective analysis), 753 consecutive patients with LC with ultrasound-detected PVT were retrospectively analyzed. In part 2, 112 patients with LC and 56 matched controls were entered in the cross-sectional study. In part 3, 5 patients with cirrhosis were entered in the in vivo study and 4 healthy subjects (HSs) were entered in the in vitro study to explore if albumin may affect platelet activation by modulating oxidative stress. In the 753 patients with LC, the prevalence of PVT was 16.7%; logistic analysis showed that only age (odds ratio [OR], 1.024; P = 0.012) and serum albumin (OR, -0.422; P = 0.0001) significantly predicted patients with PVT. Analyzing the 112 patients with LC and controls, soluble clusters of differentiation (CD)40-ligand (P = 0.0238), soluble Nox2-derived peptide (sNox2-dp; P < 0.0001), and urinary excretion of isoprostanes (P = 0.0078) were higher in patients with LC. In LC, albumin was correlated with sCD4OL (Spearman's rank correlation coefficient [r(s)], -0.33; P < 0.001), sNox2-dp (r(s), -0.57; P < 0.0001), and urinary excretion of isoprostanes (r(s), -0.48; P < 0.0001) levels. The in vivo study showed a progressive decrease in platelet aggregation, sNox2-dp, and urinary 8-iso prostaglandin F2 alpha-III formation 2 hours and 3 days after albumin infusion. Finally, platelet aggregation, sNox2-dp, and isoprostane formation significantly decreased in platelets from HSs incubated with scalar concentrations of albumin. Conclusion: Low serum albumin in LC is associated with PVT, suggesting that albumin could be a modulator of the hemostatic system through interference with mechanisms regulating platelet activation

Lettere dai campi di concentramento e dalle prigioni, un esempio di raffigurazione della realtà concentrazionaria

L’articolo presenta il volume di Lucyna Sadzikowska intitolato Listy z lagrów i więzień 1939–1945. Wybrane zagadnienia (Lettere dai campi e dalle prigioni 1939–1945. Alcuni aspetti. Katowice 2019). Secondo l’autore dell’articolo, le lettere dei prigionieri dei campi di concentramento e di lavoro costituiscono un’interessante descrizione della realtà concentrazionaria che avvicina il lettore alla letteratura dei documenti personali. L’obiettivo del libro della Sadzikowska è quello di descrivere e codificare le lettere dai campi e dalle prigioni nel loro aspetto teorico e pratico. L’autrice prende in esame la corrispondenza ufficiale e privata dei campi e delle prigioni (per esempio i messaggi segreti e le lettere clandestine), e presenta uno specifico supplemento a questa epistolografia, ovvero le lettere di Gustaw Morcinek. Il volume della Sadzikowska illustra non solo il suo grande impegno nell’analizzare l’aspetto epistolografico dei documenti redatto nel periodo tra il 1939 e il 1945, ma anche la loro forza ispiratrice.The article discusses Lucyna Sadzikowska’s book entitled Listy z lagrów i więzień 1939–1945. Wybrane zagadnienia (Letters from the Concentration Camps and Prisons 1939–1945. Selected Issues. Katowice 2019). According to the author of the article, the published letters of the inmates kept in concentration and labour camps remain interesting descriptions of the camp reality that redirect the reader towards literature of the personal document. The purpose of Sadzikowska’s book is to describe and codify the prison and camp letter with regard to its theoretical and practical aspects. She analyses and elaborates on official and unofficial camp and prison correspondence (e.g. secret messages, letters smuggled in or out of camps and prisons), and presents a peculiar   supplement to this epistolography, that is, the literary letters of Gustaw Morcinek. The reviewed work not only presents the author’s commitment to elaborate on camp and prison epistolography written in the period between 1939 and 1945, but also points out the inspiring potential of personal documents

Аннотация: В этой статье представлено дискуссии о форме государства и демократии, в том числе между Н. Боббио и некоторыми марксистскими философами итальянскими, в 70-е гг. XX века. Этот период характеризует усиленная философская конфронтация связанная с политической обстановкой в мире. Дискуссии имели место посредством статей в журнале „Mondoperaio”. В нынешней статьи указано основные тезисы этих дискуссий, которые подчеркивают недостатки марксистской доктрины о государстве, последовательно отмечая конец всех гегемонистских амбиций итальянских интеллектуалов, связанных с международным коммунизмом
