Designing the Future, Engineering Reality: Prototyping in the Emergency Department

This case study covers the implementation of a live prototype aimed at addressing the issue of overcrowding at the emergency department (ED) of the Santa Maria Nuova Hospital (ASMN) in Reggio Emilia (Italy). It was facilitated by a team of service designers and management engineers from the University of Modena and Reggio Emilia (UNIMORE), and a working group of 15 professionals composed of doctors, nurses and auxiliaries. The live prototype involved the 150+ staff of the emergency department and over 3,750 patients over a period of 34 days. The end result of the service was a smoother patient flow that reduced waiting time by 38%, and had a patient satisfaction rating of 94% increased staff-patient communication. The service also carried negative effects on how doctor’s viewed their professional identity, and caused stress due to uncontrollable noise limits

Procuramos estabelecer os elementos formais e estruturais que possibilitam a emergência das identidades individuais e coletivas através das identificações significantes, e demonstrar como que se sustenta no sujeito uma identificação em sua relação com o desejo e afeto. Many of the poems in this collection deal with issues of immigration, exile and cultural loss. The emphasis, however, is not placed on ethnicity in any strict sense, but rather on taking the, by now, familiar premise of the immigrant subject and giving it back to the reader, as the Irish poet Seamus Heaney writes, "with a clean new music in it". The Italian-Canadian identity of the poems--or its Italianita --is enacted either through autobiographical narrative or by mediating on certain relevant cultural and societal artifacts. Neisseria gonorrhoeae strains of recent isolation in Italy: study of surface-exposed antigens and sera from infected patients

Scarsi sono i dati in letteratura relativi all'infezione da Neisseria gonorrhoeae (Ng) in Italia. E' stato, dunque, condotto uno studio, nel periodo compreso tra 2003 e il 2007 al fine di studiare le caratteristiche fenotipiche e genotipiche e dei ceppi circolanti nel nostro Paese. La gonorrea può essere trattata efficacemente con terapia antibiotica, tuttavia, l'aumento di ceppi antibiotico-resistenti rappresenta un fattore di rischio per l'insorgenza di malattia complicata e per la facilitazione della trasmissione dell'infezione da HIV. Inoltre, a tutt'oggi, non esiste ancora un vaccino adatto. Materiali e metodi. Sono stati campionati 481 ceppi di Ng provenienti da pazienti con o senza co-infezione da HIV. In alcuni pazienti, quando possibile, sono stati collezionati campioni di siero. Gli isolati clinici sono stati caratterizzati mediante sierotipizzazione e valutazione della sensibilità agli antibiotici, nonchè mediante analisi genotipiche. Inoltre, in un gruppo di pazienti opportunamente selezionati, è stata analizzata la presenza e la conservazione di due antigeni di superficie, recentemente identificati in Ng: Macrophage infectivity potentiator (Ng-Mip) e Outer membrane protein A (Ng-OmpA). E’ stata anche valutata la presenza di anticorpi contro queste due proteine in sieri di pazienti infetti Risultati. La sierotipizzazione ha presentato 3 diversi sierotipi: IA, IB e IA/IB. In particolare, IB è risultato il sierotipo prevalente (90%). Sono stati, inoltre, identificati ceppi di Ng resistenti alla ciprofloxacina (46%), tetraciclina (28.3%), penicillina (20.4%) e ceppi resistenti a due/tre antibiotici contemporaneamente. La maggior parte dei ceppi analizzati non ha mostrato correlazioni genotipiche, tuttavia è stato identificato un ampio cluster di ceppi resistenti: ST661. L’analisi delle sequenze aminoacidiche di Ng-Mip e Ng-OmpA ha messo in evidenza un alto grado di omologia tra i ceppi testati. Peraltro, anticorpi anti-Mip e anti-OmpA sono stati ritrovati in sieri di pazienti infetti. Conclusioni. La presenza di ceppi resistenti o multi-resistenti agli antibiotici suggerisce l’importanza di monitorare la suscettibilità al fine di attuare adeguati trattamenti terapeutici. Inoltre, considerando l’elevata variabilità antigenica delle componenti di superficie di Ng, i risultati di questo studio mettono in evidenza il possibile impiego di questi due antigeni di superficie, altamente conservati, in un potenziale vaccino.Reliable data concerning Neisseria gonorrhoeae (Ng) infections in Italy is lacking, as information about the phenotypic and genotypic characteristics of circulating strains. A study was undertaken from 2003-2007 to explore these issues. Gonorrhoea could be effectively treated with antibiotics, however the emergence of antibiotic resistance may increase the risk of complicated infections and the amplification of HIV transmission; in addition, yet no vaccine is available. Methods. A total of 481 viable gonococcal strains were collected from patients with or without concurrent HIV-infection. Sera samples from infected patients were also collected. Strains were characterized by serovar determination and antimicrobial susceptibility testing, as well as genotypic analyses. Macrophage infectivity potentiator (Ng-Mip) and Outer Membrane protein A (Ng-OmpA), two surface-exposed antigens, recently identified in Ng, have been investigated for their presence and their conservation in a selected group of isolates. Moreover, sera recognition ability also has been evaluated. Results. The strains were assigned to three different serovars: IA, IB and IA/IB with IB the most frequent (90%). Ciprofloxacin (46%), tetracycline (28.3%) and penicillin (20.4%) resistance phenotypes were detected as well as resistance to one or more drugs. Most of the strains were unrelated each others, however a large cluster of resistant isolates, ST661, was identified. Interestingly, amino acid sequences analysis revealed a high degree of homology for Ng-Mip, and 100% homology for Ng-OmpA proteins. Recombinant Ng-Mip and Ng-OmpA proteins were also recognized by sera from patients infected with gonorrhoea. Conclusions. Considering the presence of antimicrobial resistant as well as multi-drug resistant gonococci, is important to monitor the antibiotic susceptibility in order to apply effective treatment strategies. Most of the surface components in Ng show antigenic variability. Conversely, the high degree of conservation showed by Ng-Mip and Ng-OmpA suggest their potential use as candidates vaccine

O estatuto político do significante vazio: identidade coletiva, psicanálise e política

Neste artigo procuramos apresentar os elementos que possibilitam a constituição das identidades coletivas a partir de quatro conceitos fundamentais: a demanda, o significante vazio, a identificação e o gozo (la jouissance). Primeiro, revisitamos Freud trazendo a noção de identificação pensada em sua determinação política. Em seguida, buscamos justificar a especificidade da noção lacaniana de significante vazio, repensado por Ernesto Laclau enquanto um conceito político fundamental implicado na constituição e formação das identidades coletivas. Avançamos na lógica do significante e das demandas insatisfeitas que se organizam basilarmente na composição das identidades coletivas a partir de casos específicos e formalizações. UMA LÓGICA DA FANTASIA? LACAN ENTRE A CLÍNICA, O SOCIAL E A ONTOLOGIA

Uma lógica da fantasia? Trata-se da proposta lacaniana para aquilo que  parece severamente falho, em princípio, de algum ordenamento, o mais contrário possível a um parâmetro lógico. O presente artigo se propõe a discutir e estabelecer o valor teórico e clínico dessa lógica a partir de três momentos: Primeiro, é abordado a querela acerca do estatuto da realidade presente na obra freudiana e as formulações lacanianas frente aos impasses clínicos e ontológicos encontrados; o que tem por efeito a proposição de uma lógica amarrada a estrutura do matema $ ◊ a, e a delimitação fronteiriça da fantasia nos registros RSI. Segundo, se articula o ordenamento dessa lógica a partir dos elementos do matema e de proposições centrais do Seminário 14, a lógica da fantasia. Terceiro, a lógica da fantasia é articulada na dimensão clínica – e portanto social – sendo evocadas algumas formalizações lacanianas, a saber, os quatro discursos e o grafo do desejo. Preliminary Report on a Spiritually-Based PTSD Intervention for Military Veterans

Military veterans can experience spiritual/religious struggles such as weakening of beliefs, loss of meaning, increased guilt, difficulty forgiving, and moral challenges as a result of military trauma. While mainstream treatments (e.g., exposure therapy) have been shown to be effective for many, they often fail to address these issues adequately. This paper describes an 8-session spiritually-based group intervention designed to treat trauma-related spiritual wounds among military veterans. A program evaluation conducted with 24 veterans revealed significant reductions in PTSD symptoms, spiritual injury, and negative religious coping from pretest to posttest. The findings support the need for additional PTSD treatment approaches

Unlocking Creativity: 6-Part Story Method as an Imaginative Pedagogical Tool

This article emerges from an approach to transformative learning where students are challenged to explore taken-for-granted assumptions about their experiences in the world. We outline the 6-Part Story Method (6PSM), which uses abstract images to provide a structured storytelling process that enables reflexive learning. This is documented through conversations between a university teacher and three Masters students about the method used in their course on practical ethics in process consulting. Using individual stories that emerged from a common set of cards, we illustrate how the method enabled us to critically explore our practices as teacher and student consultants
