    Towards understanding terrorism: a theoretical examination of internal cohesion in terrorist groups and the negative dynamic of violence

    Terrorism, like other forms of political violence, has an organizational context. Few studies, however, have considered the influence of organizational life upon the outward behavior of the terrorist group. This thesis explores the possibility that terrorism, in addition to its political context, reflects the internal dynamics of the terrorist group. Assuming that action is what binds the terrorist group together, the use of violence may oftentimes be dictated more by the need to satisfy the internal goal of group survival than to directly further the group's external political agenda. Focusing upon internal cohesion as the critical mediating variable for group survival, this paper examines how the terrorist group's efforts to maintain itself drives violent behavior that transcends political considerations and operational prudence. When external and internal requirements become contradictory, the terrorist group faces a dilemma. Caught in a vicious cycle of reacting to strategic failure with more violent action in order to maintain itself, the terrorist group generates a negative dynamic of violence that not only undermines its chances of achieving stated long-term goals but also accelerates its decline.http://archive.org/details/towardsunderstan1094535086NANAU.S. Navy (U.S.N.) author

    Psychological aspects of doctors who takes care of patients with cancer

No entanto, existem poucos serviços com práticas regulares, que ofereçam espaços de reflexão, partilha e crescimento na difícil tarefa do cuidar em saúde. Ao lado disso, há ainda pouco investimento na elaboração de projetos que auxiliem na saúde do cuidador. O trabalho em oncologia remete ao intenso contato do médico com questões subjetivas como a dor, o sofrimento, a vida e a morte. Há estudos que mostram a existência de sentimentos de angústia, tensões, impotência e onipotência do médico, ao lidar com as limitações da própria doença, relação com o paciente e familiares, por não conseguir solucionar muitas questões referentes ao câncer, uma doença grave, que remete à finitude humana. Esse trabalho objetivou conhecer quais aspectos psicológicos estão presentes no médico que cuida de pacientes com câncer, além de identificar quais estratégias pessoais e institucionais existem para apoiá-los, a partir de sua perspectiva. A metodologia utilizada foi a pesquisa qualitativa. Foram realizadas entrevistas com médicos que cuidam de pacientes oncológicos do Hospital de Clínicas da Unicamp. Os resultados foram analisados a partir da análise temática de conteúdo, utilizando referenciais teóricos da psicanálise e da Saúde Coletiva. O estudo pretendeu contribuir com a Saúde Coletiva, na medida em que investigou as necessidades de cuidado desses trabalhadores e poderá suscitar novas questões para futuros projetos que auxiliem a saúde do trabalhador no âmbito do Sistema Único de Saúde, repercutindo no cuidado em oncologia mais ampliado e integral.Abstract: There are several works about the care related to the medical professional, regarding the sickness risk that can happen due their practice. However, still few services exist with regular practices that offer to these professionals opportunities to reflect, share and growth in the difficult task of taking care in health environment, besides that, there is still little investment in the elaboration of projects regarding the health care professionals. The work in oncology makes the doctor be in intense contact with subjective subjects as pain, suffering, life and death. There are studies that show the existence of anguish feelings, tensions, impotence and onipotence by the doctor when working with limitations of their own disease, relationship with the patient and their relatives and with the fact of not solving many subjects regarding cancer, a serious disease that is related to the human finiteness. This work had the objective to know which psychological aspects are present in the doctor who takes care of patients with cancer, and also to identify which personal and institutional strategies are used to support them, starting from that professional's perspective. The methodology used was a qualitative research. Interviews were accomplished with doctors who take care of oncology patients in the General Hospital of Unicamp. The results were analyzed from the thematic content analysis, using theoretical references of the psychoanalysis and the Public Health. The study intended to contribute with the Collective Health by investigating the needs of those medical workers and it can rise new subjects for projects that can help the health care workers who work for the public health system in Brazil (called Unique System of Health), reflecting in a more extended and full oncology care.MestradoSaude ColetivaMestre em Saude Coletiv

    Kinetic Analysis of Substrate Utilization by Native and TNAP-, NPP1-, or PHOSPHO1-Deficient Matrix Vesicles

    During the process of endochondral bone formation, chondrocytes and osteoblasts mineralize their extracellular matrix by promoting the formation of hydroxyapatite seed crystals in the sheltered interior of membrane-limited matrix vesicles (MVs). Here, we have studied phosphosubstrate catalysis by osteoblast-derived MVs at physiologic pH, analyzing the hydrolysis of ATP, ADP, and PPi by isolated wild-type (WT) as well as TNAP-, NPP1- and PHOSPHO1-deficient MVs. Comparison of the catalytic efficiencies identified ATP as the main substrate hydrolyzed by WT MVs. The lack of TNAP had the most pronounced effect on the hydrolysis of all physiologic substrates. The lack of PHOSPHO1 affected ATP hydrolysis via a secondary reduction in the levels of TNAP in PHOSPHO1-deficient MVs. The lack of NPP1 did not significantly affect the kinetic parameters of hydrolysis when compared with WT MVs for any of the substrates. We conclude that TNAP is the enzyme that hydrolyzes both ATP and PPi in the MV compartment. NPP1 does not have a major role in PPi generation from ATP at the level of MVs, in contrast to its accepted role on the surface of the osteoblasts and chondrocytes, but rather acts as a phosphatase in the absence of TNAP. © 2010 American Society for Bone and Mineral Research

    Quality of life in elderly with dizziness

    Dizziness is frequent in elderly people. AIMS: To evaluate the Quality of Life (QoL) in elderly subjects with dizziness, relate it with gender and age. MATERIAL AND METHOD: A prospective study comprising 120 elderly patients with dizziness evaluated with Brazilian versions of the Whoqol-bref and the dizziness handicap inventory (DHI). The factor analysis (FA), the Mann Whitney and Kruskal Wallis tests, and the Spearman correlation were applied to study the results. RESULTS: The most compromised domains were the DHI physical domain and the Whoqol-bref physical and environment domains. FA resulted in 3 factors in the DHI and 5 factors in the Whoqol-bref. There was a moderate correlation (-0.596) in the total scores of both instruments. Males had a better QoL in the environment perception and introspectivity and health perception factors of the Whoqol-bref test. Females had a better QoL in the functionality perception factor of the Whoqol-bref test. There were no significant age differences. CONCLUSIONS: Elderly patients with dizziness have a worse QoL. Elderly females with dizziness have worse QoL scores in environment perception and introspectivity and health perception and better QoL in the functionality perception factor compared to elderly males.Tontura é muito frequente em idosos. OBJETIVOS: Avaliar a qualidade de vida (QV) em idosos vestibulopatas e relacioná-la com o gênero e a faixa etária. MATERIAL E MÉTODO: Estudo prospectivo que incluiu série de 120 casos de idosos com tontura que foram avaliados por meio das versões brasileiras dos questionários Whoqol-bref e Dizziness Handicap Inventory (DHI). Os resultados obtidos foram submetidos à Análise Fatorial (AF) e aos testes de Mann Whitney, Kruskal Wallis e correlação de Spearman. RESULTADOS: O domínio físico avaliado pelo DHI, e o físico e o ambiental do Whoqol-bref foram os mais comprometidos. A AF resultou em 3 fatores no DHI e 5 no Whoqol-bref: Houve moderada correlação (-0,596) entre o escore total de ambos os instrumentos. Os homens apresentaram melhor QV nos fatores percepção ambiental e introspectividade e percepção da saúde do Whoqol-bref. As idosas obtiveram melhor QV no fator percepção da funcionalidade do Whoqol-bref. Não houve diferenças significantes entre os fatores dos dois instrumentos por faixa etária. CONCLUSÕES: Idosos vestibulopatas apresentam QV prejudicada.     The general objective of this research is to analyze the importance of the use of ICT in techno-pedagogical methods that affect teaching management. The methodology used was based on a bibliographic development of a documentary type. ICTs in the educational system have been based on a boom in web 2.0 and the many tools that exist to facilitate virtual education. The use of ICT in the field of university education requires the commitment of the authorities and the teachers themselves. The latter must apply the pedagogical strategies according to the programmatic contents with a mastery and knowledge of the technological instruments. From here, the techno-pedagogical methods are born, which pursue the creation of learning objects, which are characterized by being the means of interaction between the teacher and the student and which are the creative domain of the teacher.La espondilitis anquilosante es una artritis que se caracteriza por presentar inflamación crónica y las articulaciones se van degenerando, que afecta en gran proporción la columna vertebral y la articulación sacro ilíaca, esta enfermedad es más común que se presente en hombre que en mujeres, alrededor de los 20 a 30 años, afectado la calidad de vida de los individuos, y generando malestar y limitación de su movilidad. El objetivo de esta investigación es describir las causas y el diagnóstico de la espondilitis anquilosante a través de una revisión sistemática. Como conclusión se menciona que, es evidente que existe una gran variedad de afecciones que pueden estar asociadas con la EA, sin embargo, se observa la coincidencia que una de las principales consecuencias que genera esta enfermedad es el deterioro de la calidad de vida de los individuos que padecen esta y otras enfermedades durante su vida, y que repercuten en el comportamiento psicológico de los mismos. Es importante destacar que se debe seguir realizando análisis y estudios genéticos y de casos de pacientes con esta patología que permita determinar de forma más clara las causas que generan esta enfermedad y la prevalencia de la misma en los individuos, así como poder determinar posibles nuevos tratamientos que permitan mejorar en gran medida los síntomas, como el dolor, la inflamación y la rigidez para que las personas con esta enfermedad puedan realizar sus actividades sin depender de la ayuda de otras personas

    Terapia intensiva: Atención de pacientes con diagnóstico de pancreatitis aguda severa

    The pancreas fulfills an important function within the digestive system, so it is not exempt from being able to suffer complications that alter its normal functioning. Among the complications is acute pancreatitis, which is caused by multiple factors including diabetes, obesity, excessive alcohol consumption, tobacco, among others. There are parameters that serve to classify the severity of temper, among which are APACHE II, Ranson, Atlanta International Consensus 2012, Balthazar, etc. Therefore, the present investigation aims to analyze the care of patients with acute pancreatitis evera treated in intensive care. The methodology used was based on a bibliographic design with documentary research. The results are based on the analysis of the characteristics of the pancreas, determination of severe acute pancreatitis and patients in intensive care due to complications of severe acute pancreatitis. In conclusion, the complications that allow patients with severe acute pancreatitis to be admitted to the ICU are due to surgery performed, so surgical treatment is recommended only when there is pancreatic necrosis, complications with internal effusion, or perforations; therefore, care within the ICU must be guided by a multidisciplinary team with knowledge in the area and efficient management of technological equipment. Keywords: Pancreas, severe acute pancreatitis, intensive therapy.El páncreas cumple una función importante dentro del aparato digestivo, por lo que no está exento de poder sufrir complicaciones que alteren su normal funcionamiento. Dentro de las complicaciones se encuentra la pancreatitis aguda, la cual es causada por múltiples factores que incluyen diabetes, obesidad, consumo excesivo de alcohol, tabaco, entre otros. Existen parámetros que sirven para clasificar la severidad de temple, entre los cuales se encuentra APACHE II, Ranson, Consenso Internacional de Atlanta 2012, Balthazar, etc. Por lo cual, la presente investigación tiene como objetivo analizar la atención de los pacientes con pancreatitis aguda evera atendidos en terapia intensiva. La metodología utilizada se basó en un diseño bibliográfico con una investigación de tipo documental. Los resultados están basados en el análisis de las características del páncreas, determinación de la pancreatitis aguda severa y pacientes en terapia intensiva producto de complicciones de la pancreatitis aguda severa. Como conclusión, las complicaciones que permiten el ingreso a la UCI a los pacientes con pancreatitis aguda severa son debida a cirugía realizadas, por lo que se recomienda tratamiento quirúrgico solo cuando existe necrosis pancreática, complicaciones con derrame interno, o perforaciones; por lo tanto, los cuidados dentro de la UCI deben ser guiados por un equipo multidisciplinario con conocimientos en el área y manejo eficiente de los equipos tecnológicos.
