    Utilizações e benefícios do PRF na regeneração tecidular na cirurgia da elevação do seio maxilar

    Introdução: A elevação do seio maxilar é comum para permitir a implantação num maxilar atrófico com proximidade do seio. O PRF, material autólogo que permite a libertação lenta de fatores de crescimento pode ser uma alternativa aos produtos existentes como material de enxerto. Objetivo: Avaliar as utilizações e benefícios do PRF na elevação do seio maxilar, a sua associação a outros materiais e sua capacidade de reparar perfurações da membrana Schneideriana. Materiais e métodos: Foi realizada uma pesquisa bibliográfica no dia 16 de janeiro 2023 na base de dados PubMed com a combinação de palavras-chave “(((maxillary sinus augmentation) OR (maxillary sinus lift)) AND (prf)) AND (implant)”, entre 2012 e 2023. Resultados: A pesquisa bibliográfica identificou um total de 50 artigos na PubMed. Destes artigos, 21 foram selecionados. Discussão: O PRF como material de enxerto isolado oferece bons resultados em angiogénse, ganho ósseo e estabilidade do implante. Sua combinação com outros materiais pode ou não trazer benefícios no tempo de cicatrização. O PRF é eficaz para cobrir aberturas laterais e tem excelentes resultados no tratamento de perfurações da membrana do seio. Conclusão: O PRF é um material ideal para elevar o seio maxilar, devido ao seu caracter autólogo, preparação simples e preço acessível. Ele pode ser usado para preencher o espaço elevado e cobrir a janela lateral. Pode ser associado a outros materiais para reduzir o tempo de cicatrização, especialmente quando combinado com DBBM. E também eficaz no tratamento de perfurações da membrana Schneideriana de grandes dimensões.Introduction: Maxillary sinus elevation is common to enable implantation in an atrophied posterior maxilla with proximity to the sinus. PRF may be an alternative to existing materials as a graft material. Objective: Evaluate the uses and benefits of PRF in maxillary sinus elevation, its association with other materials, and its ability to repair Schneiderian membrane perforations. Materials and Methods: A literature search was conducted on January 16, 2023, in the PubMed database using the keyword combination “(((maxillary sinus augmentation) OR (maxillary sinus lift)) AND (prf)) AND (implant)” between 2012 and 2023. Results: The literature search identified a total of 50 articles in PubMed. Of these articles, 21 were selected for this study. Discussion: PRF as a graft material yields positive outcomes in vasculogenesis, bone gain, and implant stability. Its combination with other materials may or may not offer benefits in terms of healing time. PRF is effective for covering lateral openings and exhibits remarkable efficacy in treating sinus membrane perforations. Conclusion: PRF appears to be a first-choice material in maxillary sinus elevation, as it has many advantages due to its autologous character, simple preparation, and low cost. It can be used to fill the elevated space and cover the lateral window. It can be used in combination with other materials and reduces healing time when associated with DBBM. It also allows for the treatment of Schneiderian membrane perforations, even of large dimensions

    X-ray multilayer monochromator with enhanced performance

    International audienceAn x-ray multilayer monochromator with improved resolution and a low specular background is presented. The monochromator consists of a lamellar multilayer amplitude grating with appropriate parameters used at the zeroth diffraction order. The device is fabricated by means of combining deposition of thin films on a nanometer scale, UV lithography, and reactive ion etching. The performance of this new monochromator at photon energies near 1500 eV is shown

    Orthopaedic management of Hurler’s disease after hematopoietic stem cell transplantation: a systematic review

    The introduction of hematopoietic stem cell transplantation (HSCT) has significantly improved the life-span of Hurler patients (mucopolysaccharidosis type I-H, MPS I-H). Yet, the musculoskeletal manifestations seem largely unresponsive to HSCT. In order to facilitate evidence based management, the aim of the current study was to give a systematic overview of the orthopaedic complications and motor functioning of Hurler's patients after HSCT. A systematic review was conducted of the medical literature published from January 1981 to June 2010. Two reviewers independently assessed all eligible citations, as identified from the Pubmed and Embase databases. A pre-developed data extraction form was used to systematically collect information on the prevalence of radiological and clinical signs, and on the orthopaedic treatments and outcomes. A total of 32 studies, including 399 patient reports were identified. The most frequent musculoskeletal abnormalities were odontoid hypoplasia (72%), thoracolumbar kyphosis (81%), genu valgum (70%), hip dysplasia (90%) and carpal tunnel syndrome (63%), which were often treated surgically during the first decade of life. The overall complication rate of surgical interventions was 13.5%. Motor functioning was further hampered due to reduced joint mobility, hand dexterity, motor development and longitudinal growth. Stem cell transplantation does not halt the progression of a large range of disabling musculoskeletal abnormalities in Hurler's disease. Although prospective data on the quantification, progression and treatment of these deformities were very limited, early surgical intervention is often advocated. Prospective data collection will be mandatory to achieve better evidence on the effect of treatment strategies

    Neurocognition across the spectrum of mucopolysaccharidosis type I: Age, severity, and treatment

    OBJECTIVES: Precise characterization of cognitive outcomes and factors that contribute to cognitive variability will enable better understanding of disease progression and treatment effects in mucopolysaccharidosis type I (MPS I). We examined the effects on cognition of phenotype, genotype, age at evaluation and first treatment, and somatic disease burden. METHODS: Sixty patients with severe MPS IH (Hurler syndrome treated with hematopoietic cell transplant and 29 with attenuated MPS I treated with enzyme replacement therapy), were studied with IQ measures, medical history, genotypes. Sixty-seven patients had volumetric MRI. Subjects were grouped by age and phenotype and MRI and compared to 96 normal controls. RESULTS: Prior to hematopoietic cell transplant, MPS IH patients were all cognitively average, but post-transplant, 59% were below average, but stable. Genotype and age at HCT were associated with cognitive ability. In attenuated MPS I, 40% were below average with genotype and somatic disease burden predicting their cognitive ability. White matter volumes were associated with IQ for controls, but not for MPS I. Gray matter volumes were positively associated with IQ in controls and attenuated MPS I patients, but negatively associated in MPS IH. CONCLUSIONS: Cognitive impairment, a major difficulty for many MPS I patients, is associated with genotype, age at treatment and somatic disease burden. IQ association with white matter differed from controls. Many attenuated MPS patients have significant physical and/or cognitive problems and receive insufficient support services. Results provide direction for future clinical trials and better disease management
