    Stress compensation by gap monolayers for stacked InAs/GaAs quantum dots solar cells

    In this work we report the stacking of 10 and 50 InAs quantum dots layers using 2 monolayers of GaP for stress compensation and a stack period of 18 nm on GaAs (001) substrates. Very good structural and optical quality is found in both samples. Vertical alignment of the dots is observed by transmission electron microscopy suggesting the existence of residual stress around them. Photocurrent measurements show light absorption up to 1.2 μm in the nanostructures together with a reduction in the blue response of the device. As a result of the phosphorus incorporation in the barriers, a very high thermal activation energy (431 meV) has also been obtained for the quantum dot emission

    Structural, dermal and ungual characteristics of the foot in patients with type II diabetes

    [Abstract] Background and Objectives: Diabetes is a chronic and metabolic disease, considered as an important public health problem. The objective of this study was to determine the prevalence of podiatric pathology in type II diabetic patients. Materials and Methods: An observational descriptive study of prevalence in the endocrinology service of Complexo Hospitalario Universitario A Coruña (CHUAC) (A Coruña-Spain) was carried out (n = 153). Type II diabetic patients included, of legal age who signed the informed consent. Sociodemographic variables were studied (age, sex, body mass index (BMI), smoking habit, alcohol consumption, family history), disease variables (time of evolution of diabetes, treatments, low-density lipoprotein (LDL), high-density lipoprotein (HDL), glucose), podiatric variables: measurement of the footprint, metatarsal and digital formula, nail, skin, hindfoot and forefoot alterations. The data collection was done in 2018 and the data analysis was carried out in 2019. Results: The patients with type II diabetes had greater age, obesity and arterial hypertension it compared to the general population. Diabetic patients had a higher prevalence of flat feet than the general population (71.2% vs. 20.7%, p < 0.001), with a predominance of normal foot according to the podoscope. The predominant podological pathology was the presence of claw toes (94.8%), followed by dermal (78.4%) and nail (71.9%) alterations, and the Hallux Valgus (66.0%). The Clarke angle and the Chippaux index showed a Kappa concordance index of 0.26 with the type of footprint measured with the podoscope. The Staheli index showed a Kappa index of 0.27 associated with an observed agreement of 54%. Conclusions: This study shows that foot problems continue to be prevalent in subjects with type II diabetes mellitus and for this reason, podiatry is essential in its treatment

    Monitoring System Analysis for Evaluating a Building’s Envelope Energy Performance through Estimation of Its Heat Loss Coefficient

    The present article investigates the question of building energy monitoring systems used for data collection to estimate the Heat Loss Coefficient (HLC) with existing methods, in order to determine the Thermal Envelope Performance (TEP) of a building. The data requirements of HLC estimation methods are related to commonly used methods for fault detection, calibration, and supervision of energy monitoring systems in buildings. Based on an extended review of experimental tests to estimate the HLC undertaken since 1978, qualitative and quantitative analyses of the Monitoring and Controlling System (MCS) specifications have been carried out. The results show that no Fault Detection and Diagnosis (FDD) methods have been implemented in the reviewed literature. Furthermore, it was not possible to identify a trend of technology type used in sensors, hardware, software, and communication protocols, because a high percentage of the reviewed experimental tests do not specify the model, technical characteristics, or selection criteria of the implemented MCSs.     Valoración del proceso docente de la asignatura de Genética Médica por estudiantes de una facultad de medicina cubana.

    Background: The Medical Genetics subject is taught in the fourth semester of the Medicine degree. During the 2022-2023 academic year, a strategy was implemented to improve the educational teaching process. What assessment do medical students have of the educational teaching process of the subject of medical genetics during the 2022-2023 academic year?. Objective: to present the assessment of the teaching process of the Medical Genetics subject by Cuban students of the Manuel Fajardo Faculty of Medicine in the 2022-2023 academic year. Methods: A cross-sectional analytical observational study was carried out on a sample of 115 students of the fourth semester of the Medicine career of the Manuel Fajardo Faculty of Medical Sciences in the capital of Cuba. Fifteen variables related to the educational teaching process previously validated by expert teachers were characterized. Results: 68.5% of the students had a favorable expectation. 60.5% of the students stated that it was difficult to understand the language used in the subject. There was a good student-teacher relationship (95.5%), support from the study group in clarifying doubts (88.5%) team study (69.8%) and the class schedule contributed to the study of the subject (64.0%). More than 94% of the students perceived the teacher's preparation, the quality of the teaching aids, the student-teacher communication, among other aspects, as satisfactory. Conclusions: The evaluation of the teaching-educational process of the subject of Medical Genetics by Cuban students of the Manuel Fajardo Faculty of Medicine in the 2022-2023 academic year was favorable.Antecedentes: La asignatura de Genética Médica se imparte en el cuarto semestre de la carrera de Medicina. Durante el curso académico 2022-2023 se implementó una estrategia para perfeccionar el proceso docente educativo. Qué valoración tienen los estudiantes de medicina sobre el proceso docente educativo de la asignatura de genética médica durante el curso 2022-2023?&nbsp; Objetivo: exponer la valoración del proceso docente de la asignatura de Genética Médica por estudiantes cubanos de la Facultad de Medicina Manuel Fajardo en el curso académico 2022-2023. Métodos: Se realizó un estudio observacional analítico transversal a una muestra de 115 alumnos del cuarto semestre de la carrera de Medicina de la Facultad de Ciencias Médicas Manuel Fajardo de la capital de Cuba. Se caracterizaron 15 variables relacionadas con el proceso docente educativo previamente validadas por profesores expertos. Resultados: El 68,5% de los estudiantes tuvieron una expectativa favorable. El 60,5% de los estudiantes afirmaron dificultad para entender el lenguaje usado en la asignatura. Hubo buena relación alumno-profesor (95,5%), apoyo del colectivo de estudio en la aclaración de dudas (88,5%) estudio en equipo (69,8%) y el horario de las clases contribuyó para el estudio individual de la asignatura (64,0%). Más del 94% de los alumnos percibieron como satisfactoria la preparación del profesor, la calidad en los medios de enseñanza, la comunicación alumno-profesor, entre otros aspectos.&nbsp; Conclusiones: Resultó favorable la valoración del proceso docente-educativo de la asignatura de Genética Médica por estudiantes cubanos de la Facultad de Medicina Manuel Fajardo en el curso académico 2022-2023

    Responsiveness of the Spanish Version of Newcastle Stroke-Specific Quality of Life (NEWSQOL)

    A prospective observational study was conducted to assess the responsiveness of the Spanish version of NEWSQOL. The sample contained 128 patients who filled in the question-naires before and after a physical therapy intervention. The responsiveness was assessed with p-values using the effect size (ES) and the standardized response means (SRMs) of the change. Besides, two other external criteria were used to distinguish patients who improved with the treatment from those who remained stable. This classification was based on one functional independence measure (the Barthel Index) and one disability measure (the modified Rankin Scale). Results: There was a statistically significant correlation (Spearman?s coefficient = p < 0.01) between the domains of the Spanish version of NEWSQOL in relation to the Barthel Index and the modified Rankin Scale. All domains showed between marked-to-mild change responsiveness except sleep and relationships; mobility (ES 0.66 and SRM 0.92) and activities of daily living (ES 0.75 and SRM 0.87) were markedly responsive; communication (ES 0.38 and SRM 0.61) was moderately responsive; and pain, vision, cognition, feelings, emotions and fatigue were mildly responsive (ES 0.21-0.41 and SRM 0.23-0.44). Conclusion: The Spanish version of NEWSQOL shows between marked and mild responsiveness to measure the perception of QoL in post-stroke patients. Therefore, its use can be suitable for eval-uation studies, clinical trials and clinical practice

    Quick outpatient diagnosis in small district or general tertiary hospitals: A comparative observational study.

    While quick diagnosis units (QDUs) have expanded as an innovative cost-effective alternative to admission for workup, studies investigating how QDUs compare are lacking. This study aimed to comparatively describe the diagnostic performance of the QDU of an urban district hospital and the QDU of its reference general hospital.This was an observational descriptive study of 336 consecutive outpatients aged ≥18 years referred to the QDU of a urban district hospital in Barcelona (QDU1) during 2009 to 2016 for evaluation of suspected severe conditions whose physical performance allowed them to travel from home to hospital and back for visits and examinations. For comparison purposes, 530 randomly selected outpatients aged ≥18 years referred to the QDU of the reference tertiary hospital (QDU2), also in Barcelona, were included. Clinical and QDU variables were analyzed and compared.Mean age and sex were similar (61.97 (19.93) years and 55% of females in QDU1 vs 60.0 (18.81) years and 52% of females in QDU2; P values = .14 and .10, respectively). Primary care was the main referral source in QDU1 (69%) and the emergency department in QDU2 (59%). Predominant referral reasons in QDU1 and 2 were unintentional weight loss (UWL) (21 and 16%), anemia (14 and 21%), adenopathies and/or palpable masses (10 and 11%), and gastrointestinal symptoms (10 and 19%). Time-to-diagnosis was longer in QDU1 than 2 (12 [1-28] vs 8 [4-14] days; P 2 visits to be diagnosed were in general more likely to be males, to have UWL and adenopathies and/or palpable masses but less likely anemia, to undergo more examinations except endoscopy, and to be referred onward to specialist outpatient clinics.Despite some differences, results showed that, for diagnostic purposes, the overall performance and effectiveness of QDUs of urban district and reference general hospitals in evaluating patients with potentially serious conditions were similar. This study, the first to compare the performance of 2 hospital-based QDUs, adds evidence to the opportunity of producing standardized guidelines to optimize QDUs infrastructure, functioning, and efficiency

    Neurocognitive Function in Acromegaly after Surgical Resection of GH-Secreting Adenoma versus Naïve Acromegaly

    Patients with active untreated acromegaly show mild to moderate neurocognitive disorders that are associated to chronic exposure to growth hormone (GH) and insulin-like growth factor (IGF-I) hypersecretion. However, it is unknown whether these disorders improve after controlling GH/IGF-I hypersecretion. The aim of this study was to compare neurocognitive functions of patients who successfully underwent GH-secreting adenoma transsphenoidal surgery (cured patients) with patients with naive acromegaly. In addition, we wanted to determine the impact of different clinical and biochemical variables on neurocognitive status in patients with active disease and after long-term cure. A battery of six standardized neuropsychological tests assessed attention, memory and executive functioning. In addition, a quantitative electroencephalography with Low-Resolution Electromagnetic Tomography (LORETA) solution was performed to obtain information about the neurophysiological state of the patients. Neurocognitive data was compared to that of a healthy control group. Multiple linear regression analysis was also conducted using clinical and hormonal parameters to obtain a set of independent predictors of neurocognitive state before and after cure. Both groups of patients scored significantly poorer than the healthy controls on memory tests, especially those assessing visual and verbal recall. Patients with cured acromegaly did not obtain better cognitive measures than naïve patients. Furthermore memory deficits were associated with decreased beta activity in left medial temporal cortex in both groups of patients. Regression analysis showed longer duration of untreated acromegaly was associated with more severe neurocognitive complications, regardless of the diagnostic group, whereas GH levels at the time of assessment was related to neurocognitive outcome only in naïve patients. Longer duration of post-operative biochemical remission of acromegaly was associated with better neurocognitive state.
