    Leadership, job satisfaction and intention to leave among registered nurses in the North West and Free State provinces of South Africa

    Background: The nurse leadership of a hospital is identified as the single most important aspect of the practice environment that impacts nurse outcomes. When nurses are satisfied with their jobs, they tend to remain with their employers and become more productive in their workplaces. Objectives: This study aimed to investigate the relationship between leadership, job satisfaction and intentions to leave among registered nurses (RNs) working in hospitals in the North West and Free State provinces of South Africa. Methods: A cross-sectional survey design was adopted. The population (N = 680) with the sample (n = 204) included RNs in medical–surgical units in both private and public hospitals in the two provinces. Data were collected using the RN4CAST questionnaire. Results: RNs were satisfied with the items pertaining to leadership except for praise and recognition (55.7%). They also indicated high levels of overall job satisfaction (70.5%) but were dissatisfied with wages (50%), study leave (40.9%) and opportunities for advancement (40.1%). Furthermore, 46.1% of the RNs intended to leave their current hospitals. The results indicated a relationship between leadership and job satisfaction (r = 0.47; p = 0.00) and between intention to leave and job satisfaction (d = 0.50). Conclusion: The nurse managers played a significant role influencing RN’s level of job satisfaction, while job satisfaction was highly correlated with intention to leave. The nurse leadership can improve job satisfaction by giving praise and recognition to the RNs for jobs well done, and RNs should be afforded the opportunity to advance their careers through further studies.     Relationship of Human Immunodeficiency Virus Viral Load in Cerebrospinal Fluid and Plasma in Patients Co-infected With Cryptococcal Meningitis

    We measured human immunodeficiency virus (HIV) ribonucleic acid (RNA) in paired cerebrospinal fluid (CSF) and plasma samples in a prospective study of 91 HIV-infected, antiretroviral therapy-naive patients with cryptococcal meningitis. Cerebrospinal fluid HIV RNA was lower than in plasma (median 4.7 vs 5.2 log10 copies/mL, P < .0001) and positively correlated with plasma HIV RNA, peripheral CD4+ T-cell percentage, and CSF CXCL10. Plasma/CSF ratio of HIV RNA ranged widely from 0.2 to 265.5 with a median of 2.6. Cerebrospinal fluid quantitative cryptococcal culture positively correlated with CSF CCL2 and CCL3. CSF-plasma viral discordance was not associated with cryptococcal-associated immune reconstitution inflammatory syndrome

    More and more sensitive information is communicated digitally and with thatcomes the demand for security and privacy on the services being used. An accurateQoS metric for these services are of interest both for the customer and theservice provider. This thesis has investigated the impact of different parameterson the perceived voice quality for encrypted VoIP using a PESQ score as referencevalue. Based on this investigation a parametric prediction model has been developedwhich outputs a R-value, comparable to that of the widely used E-modelfrom ITU. This thesis can further be seen as a template for how to construct modelsof other equipments or codecs than those evaluated here since they effect theresult but are hard to parametrise. The results of the investigation are consistent with previous studies regarding theimpact of packet loss, the impact of jitter is shown to be significant over 40 ms.The results from three different packetizers are presented which illustrates theneed to take such aspects into consideration when constructing a model to predictvoice quality. The model derived from the investigation performs well withno mean error and a standard deviation of the error of a mere 1:45 R-value unitswhen validated in conditions to be expected in GSM networks. When validatedagainst an emulated 3G network the standard deviation is even lower.
