Improving health-related quality of life in single-sided deafness: a systematic review and meta-analysis
Unilateral severe-to-profound hearing loss, or single-sided deafness (SSD), impairs listening abilities supported by the use of two ears, including speech perception in background noise and sound localisation. Hearing-assistive devices can aid listening by re-routing sounds from the impaired to the non-impaired ear or by restoring input to the impaired ear. A systematic review of the literature examined the impact of hearing-assistive devices on the health-related quality of life (HRQoL) of adults with SSD as measured using generic and disease-specific instruments. A majority of studies used observational designs, and the quality of the evidence was low to moderate. Only two studies used generic instruments. A mixed-effect meta-analysis of disease-specific measures suggested that hearing-assistive devices have a small-to-medium impact on HRQoL. The Speech, Spatial and Qualities of Hearing Scale and the Health Utilities Index Mark 3 (HUI3) were identified as instruments that are sensitive to device-related changes in disease-specific and generic HRQoL, respectively
Estrogenic activity, race/ethnicity, and Indigenous American ancestry among San Francisco Bay Area women.
Estrogens play a significant role in breast cancer development and are not only produced endogenously, but are also mimicked by estrogen-like compounds from environmental exposures. We evaluated associations between estrogenic (E) activity, demographic factors and breast cancer risk factors in Non-Latina Black (NLB), Non-Latina White (NLW), and Latina women. We examined the association between E activity and Indigenous American (IA) ancestry in Latina women. Total E activity was measured with a bioassay in plasma samples of 503 women who served as controls in the San Francisco Bay Area Breast Cancer Study. In the univariate model that included all women with race/ethnicity as the independent predictor, Latinas had 13% lower E activity (p = 0.239) and NLBs had 35% higher activity (p = 0.04) compared to NLWs. In the multivariable model that adjusted for demographic factors, Latinas continued to show lower E activity levels (26%, p = 0.026), but the difference between NLBs and NLWs was no longer statistically significant (p = 0.431). An inverse association was observed between E activity and IA ancestry among Latina women (50% lower in 0% vs. 100% European ancestry, p = 0.027) consistent with our previously reported association between IA ancestry and breast cancer risk. These findings suggest that endogenous estrogens and exogenous estrogen-like compounds that act on the estrogen receptor and modulate E activity may partially explain racial/ethnic differences in breast cancer risk
Archiving Appalachia
Led by the staff of the Archives of Appalachia, this roundtable discussion will focus on how archival collections can capture and perpetuate the creative, dynamic force of Appalachia. The participants, along with Amy Collins, Director of the Archives of Appalachia, will briefly remark on the development and use of archival collections, media in the archives, and measures taken to ensure that collections reflect the diversity of the regionâs history and culture. The audience will be encouraged to share their perspectives on archival research, thereby creating dialogue about archival access and use, as well as archiving digital scholarship, and the potential for growing Appalachian collections. The intent of this discussion is to prompt ASA members to think about how archival research can contribute to their work on Appalachia and to consider the benefits of placing their own documents, photographs, books, and media in archival repositories. Radiative absorption enhancements by black carbon controlled by particle-to-particle heterogeneity in composition.
Black carbon (BC) absorbs solar radiation, leading to a strong but uncertain warming effect on climate. A key challenge in modeling and quantifying BC's radiative effect on climate is predicting enhancements in light absorption that result from internal mixing between BC and other aerosol components. Modeling and laboratory studies show that BC, when mixed with other aerosol components, absorbs more strongly than pure, uncoated BC; however, some ambient observations suggest more variable and weaker absorption enhancement. We show that the lower-than-expected enhancements in ambient measurements result from a combination of two factors. First, the often used spherical, concentric core-shell approximation generally overestimates the absorption by BC. Second, and more importantly, inadequate consideration of heterogeneity in particle-to-particle composition engenders substantial overestimation in absorption by the total particle population, with greater heterogeneity associated with larger model-measurement differences. We show that accounting for these two effects-variability in per-particle composition and deviations from the core-shell approximation-reconciles absorption enhancement predictions with laboratory and field observations and resolves the apparent discrepancy. Furthermore, our consistent model framework provides a path forward for improving predictions of BC's radiative effect on climate
Sexual Dimorphisms of Adrenal Steroids, Sex Hormones, and Immunological Biomarkers and Possible Risk Factors for Developing Rheumatoid Arthritis
Innate immunity and immunological biomarkers are believed to be interrelated with sex hormones and other neuroendocrine factors. Sexual dimorphism mechanisms may be operating in certain rheumatic and inflammatory diseases which occur more frequently in women than men, as rheumatoid arthritis (RA). Less data have been available on altered interrelations of the combined neuroendocrine and immune (NEI) systems as risk factors for development of certain diseases. In this study, serological interrelations of NEI biomarkers are analyzed before symptomatic onset of RA (pre-RA) versus control (CN) subjects, stratified by sex. Sexual dimorphism was found in serum levels of acute serum amyloid A (ASAA), soluble interleukin-2 receptor alpha (sIL-2Rα), and soluble tumor necrosis factor receptor 1 (sTNF-R1). Multiple steroidal and hormonal (neuroendocrine) factors also showed highly (p<0.001) significant sexual dimorphism in their assayed values, but less for cortisol (p=0.012), and not for 17-hydroxyprogesterone (p=0.176). After stratification by sex and risk of developing RA, differential NEI correlational patterns were observed in the interplay of the NEI systems between the pre-RA and CN groups, which deserve further investigation
Change in health-related quality of life in the context of pediatric obesity interventions: A meta-analytic review.
Objective: To quantitatively characterize change in health-related quality of life (HRQoL) in the context of behavioral (n = 16), surgical (n = 5), and pharmacological (n = 1) interventions for pediatric overweight and obesity. A secondary goal was to examine the relationship between change in HRQoL and change in body mass index (ÎBMI) by treatment type. The amount of weight loss necessary to observe a minimally clinically important difference (MCID) in HRQoL was determined. Method: Data were gathered from studies reporting on weight change and ÎHRQoL over the course of obesity interventions (N = 22) in youths (N = 1,332) with average ages between 7.4 and 16.5 years (M = 12.2). An overall effect size was calculated for ÎHRQoL. Moderation analyses were conducted using analysis of variance and weighted regression. MCID analyses were conducted by converting HRQoL data to standard error of measurement units. Results: The overall effect size for ÎHRQoL in the context of pediatric obesity interventions was medium (g = 0.51). A significant linear relationship was detected between ÎBMI and ÎHRQoL (R2 = 0.87). This relationship was moderated by treatment type, with medical (i.e., surgical) interventions demonstrating a stronger relationship. Results indicated that it takes a change of 0.998 BMI units to detect true change in HRQoL. Conclusion: This study provides the first known quantitative examination of changes in HRQoL associated with weight loss in pediatric interventions. Medical interventions appear to offer a more substantial increase in HRQoL per unit of BMI change. The BAT AGN Spectroscopic Survey -- XVIII. Searching for Supermassive Black Hole Binaries in the X-rays
Theory predicts that a supermassive black hole binary (SMBHB) could be
observed as a luminous active galactic nucleus (AGN) that periodically varies
on the order of its orbital timescale. In X-rays, periodic variations could be
caused by mechanisms including relativistic Doppler boosting and shocks. Here
we present the first systematic search for periodic AGNs using hard X-ray
light curves (14-195 keV) from the first 105 months of the Swift Burst Alert
Telescope (BAT) survey (2004-2013). We do not find evidence for periodic AGNs
in Swift-BAT, including the previously reported SMBHB candidate
MCG+1111032. We find that the null detection is consistent with the
combination of the upper-limit binary population in AGNs in our adopted model,
their expected periodic variability amplitudes, and the BAT survey
characteristics. We have also investigated the detectability of SMBHBs against
normal AGN X-ray variability in the context of the eROSITA survey. Under our
assumptions of a binary population and the periodic signals they produce which
have long periods of hundreds of days, up to % true periodic binaries can
be robustly distinguished from normal variable AGNs with the ideal uniform
sampling. However, we demonstrate that realistic eROSITA sampling is likely to
be insensitive to long-period binaries because longer observing gaps reduce
their detectability. In contrast, large observing gaps do not diminish the
prospect of detecting binaries of short, few-day periods, as 19% can be
successfully recovered, the vast majority of which can be identified by the
