    WstępRadioterapia odgrywa istotną rolę w leczeniu naczyniaków wątroby. Jako leczenie nowotworów złośliwych umiejscowionych w wątrobie pozostaje rzadkością. W niektórych ośrodkach paliatywna radioterapia zmian przerzutowych w wątrobie jest tematem prób klinicznych.O wiele częściej rozpatruje się i bada problem powikłań popromiennych wątroby po radioterapii górnego piętra jamy brzusznej. Przedstawiamy przypadek chorej z rakowiakiem wątroby, u której podjęto się paliatywnego napromieniania wątroby.Materiał i metodyChora lat 56 z rozpoznaniem rakowiaka wątroby po chemioterapii leczona byłą fotonami MV na okolicę wątroby z powodu utrzymujących się od ponad dwóch lat zmian nowotworowych. Dotychczasowym leczeniem cytostatykami (PDD, VP) uzyskano stabilizację procesu nowotworowego. Leczeniem objęto wątrobę wraz z węzłami chłonnymi wnęki. Pacjentka otrzymała 2800 cGy/T na okolice wątroby.WynikiW kontrolnym badaniu TK wykonanym 3 miesiące po zakończeniu radioterapii stwierdzono remisję większości zmian nowotworowych w wątrobie. W kolejnym badaniu wykonanym po 6 miesiącach stwierdzono ponowny wzrost ognisk nowotworu w wątrobie. Chora przeżyła 13 miesięcy od zakończenia radioterapii. Przez cały okres obserwacji zgłaszała liczne dolegliwości ze strony układu pokarmowego.WnioskiWskazania do radioterapii zmian nowotworowych w wątrobie pozostają kontrowersyjne w nielicznych przypadkach, po wyczerpaniu innych metod leczenia, radioterapię wątroby można zastosować w izolowanych ogniskach nowotworowych

    Processive Movement by a Kinesin Heterodimer with an Inactivating Mutation in One Head†

    ABSTRACT: A single molecule of the motor enzyme kinesin-1 keeps a tight grip on its microtubule track, making tens or hundreds of discrete, unidirectional 8 nm steps before dissociating. This high duty ratio processive movement is thought to require a mechanism in which alternating stepping of the two head domains of the kinesin dimer is driven by alternating, overlapped cycles of ATP hydrolysis by the two heads. The R210K point mutation in Drosophila kinesin heavy chain was reported to disrupt the ability of the enzyme active site to catalyze ATP P-O bond cleavage. We expressed R210K homodimers as well as isolated R210K heads and confirmed that both are essentially inactive. We then coexpressed tagged R210K subunits with untagged wild-type subunits and affinity purified R210K/wild-type heterodimers together with the inactive R210K homodimers. In contrast to the R210K head or homodimer, the heterodimer was a highly active (>50 % of wild-type) microtubule-stimulated ATPase, and the heterodimer displayed high duty ratio processive movement in single-molecule motility experiments. Thus, dimerization of a subunit containing the inactivating mutation with a functional subunit can complement the mutation; this must occur either by lowering or by bypassing kinetic barriers in the ATPase or mechanical cycles of the mutant head.     A Scoping Review of Home Produced Heroin and Amphetamine Type Stimulant Substitutes: Implications for Prevention, Treatment and Policy

    Several home-produced substances such as krokodil and boltushka are prevalent in many Eastern European countries. Anecdotal reports of its use have been circulating in Germany and Norway; however, this has not been confirmed. Its use has also been reported by the media in the USA, although only one confirmed report of its use exists. Home-produced drugs are associated with high levels of morbidity and a number of complex health issues such as the spread of blood borne viruses, gangrene, and internal organ damage. The high incidence of HIV rates amongst people who inject home-produced substances is a public health concern. The resulting physical health consequences of injecting these crude substances are very severe in comparison to heroin or amphetamine acquired in black markets. Due to this fact and the increased mortality associated with these substances, professionals in the area of prevention, treatment, and policy development need to be cognisant of the presentation, harms, and the dangers associated with home-produced substances globally. This scoping review aimed to examine existing literature on the subject of home-produced heroin and amphetamine-type stimulant substitutes. The review discussed the many implications such research may have in the areas of policy and practice. Data were gathered through the use of qualitative secondary resources such as journal articles, reports, reviews, case studies, and media reports. The home production of these substances relies on the utilisation of precursor drugs such as less potent stimulants, tranquillizers, analgesics, and sedatives or natural plant ingredients. The Internet underpins the facilitation of this practice as recipes, and diverted pharmaceutical sales are available widely online, and currently, ease of access to the Internet is evident worldwide. This review highlights the necessity of prevention, education, and also harm reduction related to home-produced drugs and also recommends consistent monitoring of online drug fora, online drug marketplaces, and unregulated pharmacies

    A global experiment on motivating social distancing during the COVID-19 pandemic

    Significance Communicating in ways that motivate engagement in social distancing remains a critical global public health priority during the COVID-19 pandemic. This study tested motivational qualities of messages about social distancing (those that promoted choice and agency vs. those that were forceful and shaming) in 25,718 people in 89 countries. The autonomy-supportive message decreased feelings of defying social distancing recommendations relative to the controlling message, and the controlling message increased controlled motivation, a less effective form of motivation, relative to no message. Message type did not impact intentions to socially distance, but people’s existing motivations were related to intentions. Findings were generalizable across a geographically diverse sample and may inform public health communication strategies in this and future global health emergencies. Controlled motivation was associated with more defiance and less long-term behavioral intention to engage in social distancing, whereas autonomous motivation was associated with less defiance and more short- and long-term intentions to social distance. Overall, this work highlights the potential harm of using shaming and pressuring language in public health communication, with implications for the current and future global health challenges

    Minimal information for studies of extracellular vesicles 2018 (MISEV2018): a position statement of the International Society for Extracellular Vesicles and update of the MISEV2014 guidelines

    The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles (“MISEV”) guidelines for the field in 2014. We now update these “MISEV2014” guidelines based on evolution of the collective knowledge in the last four years. An important point to consider is that ascribing a specific function to EVs in general, or to subtypes of EVs, requires reporting of specific information beyond mere description of function in a crude, potentially contaminated, and heterogeneous preparation. For example, claims that exosomes are endowed with exquisite and specific activities remain difficult to support experimentally, given our still limited knowledge of their specific molecular machineries of biogenesis and release, as compared with other biophysically similar EVs. The MISEV2018 guidelines include tables and outlines of suggested protocols and steps to follow to document specific EV-associated functional activities.     Female students’ perspective for maritime career

    The paper presents the outcome of the research that has been completed recently at the Faculty of Navigation (FoN); Maritime University of Szczecin, Poland. The aim of the research is to show the maritime society what female students – future maritime women – think about studying the navigation and future work at sea. The research has been done by a group of female students – prospective seafarers. It is based on a questionnaire directed to fellow female students which contains 29 questions concerning the motivation for commencing the study in the field of maritime navigation, aspirations, fears and obstacles faced by female seafarers related to present study,internship onboard seagoing ships and future work. 128 female students responded to the questionnaire. The answers have been gathered into groups of interest. The findings are presented in terms of diagrams and comments. The paper contains valuable information that may be used by maritime universities, ship owners and other potential employers of maritime female students – in particular maritime policy makers. One of the interesting findings is that 84% of the female students declare they would choose the same field of study having experience (including seagoing intership) gained so far. The paper presents also the change of the number of female students at FoN in the last 10 years. It is the sign of relatively high interest of young women in maritime navigation in Poland – the country that is one of the biggest suppliers of officers for seagoing ships
