    Incidência de tumores metastáticos na região buco-maxilo-facial: uma meta-análise

A proposta deste estudo foi determinar a melhor evidência disponível sobre o sítio da região buco-maxilo-facial mais acometido por metástases; assim como o tumor primário com maior ocorrência de metástase nestas regiões. Métodos. Foi realizada uma revisão sistemática da literatura considerando somente estudos escritos no alfabeto latino romano e sem restrições de período de publicação. Foram desenvolvidas estratégias de busca para cinco bases de dados: LILACS, LIVIVO, PubMed, Scopus, Web of Science; além de outras duas bases de dados da literatura cinzenta: Google Scholar e ProQuest. O risco de viés foi avaliado utilizando o "Meta-Analysis of Statistics Assessment and Review Instrument? (MASTARI). Os estudos foram selecionados de forma independente, por 2 examinadores, em 2 fases: leitura de resumos e leitura de texto completo. Resultados. A busca nas bases de dados resultou em 3403 artigos. Após a remoção dos duplicados, restaram 1754 artigos fase 1. A leitura dos resumos identificou 64 estudos para fase 2, dos quais 20 foram finalmente incluídos nesta revisão. Foram identificados tumores metastáticos oriundos de tumores primários da mama, próstata, tireoide, intestino, pulmão, rins, esôfago, estômago, pélvis, fígado, cérebro, pâncreas, ovário, laringe, pele, útero, testículos, entre outros. Foi encontrada uma variabilidade nos sítios da cavidade oral afetados, envolvendo onze diferentes regiões anatômicas, além da região maxilomandibular. Dezessete estudos foram classificados como de risco de viés moderado e três como de baixo risco de viés. O sítio da região buco-maxilo-facial mais acometido por metástases foi a mandíbula (54,92%), seguida pelos tecidos moles (40,16%) e maxila (11,69%). Conclusão. Com base nas evidências disponíveis, o tumor primário com maior ocorrência de metástase foi oriundo do pulmão, próstata e seguidos de igual incidência mama, fígado e rins. O sítio mais acometido por metástases foi a mandíbula, seguida pelos tecidos moles e maxila.Abstract : Objective. The purpose of this study was to determine the best available evidence on the site of the maxillofacial region which is mostly affected by metastases, as well as the primary tumor with a higher occurrence of metastasis in these regions. Methods. A systematic review of the literature was carried out considering only studies written in the Roman Latin alphabet and without any publication period restrictions. Searching strategies were developed for five databases: LILACS, LIVIVO, PubMed, Scopus, Web of Science and also two other gray-related literature databases: Google Scholar and ProQuest?. The risk of bias was evaluated by using the Meta-Analysis of Statistics Assessment and Review Instrument (MASTARI). The studies were selected in an independent manner, by two examiners, in two phases: abstracts? reading and full-texts reading. Results. The database search resulted in 3403 articles. After removing the duplicated ones, 1754 articles remained in phase 1. The abstracts? reading identified 64 studies for phase 2, from which 20 were ultimately included in the present review. Metastatic tumors from primary breast, prostate, thyroid, intestine, lung, kidneys, esophagus, stomach, pelvis, liver, brain, pancreas, ovary, larynx, skin, uterus and testicles tumors, among others, have been identified. The sites affected in the oral cavity widely varied, involving eleven different anatomical regions, besides the maxillary bones. Seventeen studies were classified as having a moderate bias risk and three as having a low risk of bias. The site of the oral cavity and maxillomandibular region which was mostly affected by metastases was the mandible (54.92%), followed by soft tissues (40.16%) and maxila (11.69%). Conclusion. Based on available evidence, the primary tumor with the highest occurrence of metastasis in the oral cavity and maxillary bones was from the lung and the prostate, followed by breast, kidneys and liver.     This article aims to discuss some aspects related to Narcissistic Personality Disorder associated with depression and suicide, as well as the comprehensive care of the multiple facets of a patient who presents this condition, through the experience report of a service in a psychiatric nursing office in the city of Rio de Janeiro, in the year 2022. Thus, it is intended to emphasize the importance of comprehensive and technical care, in a specialized and humanized way, to psychiatric patients to provide reception, treatment and reduction of the high suicide rate between individuals. The advances and challenges of the Psychiatric Reform within the scope of the National Mental Health Policy are discussed.Este artigo tem como objetivo discutir alguns aspectos relacionados ao Transtorno de Personalidade Narcisista associado à depressão e ao suicídio, bem como o atendimento integral às múltiplas facetas de um paciente que apresenta esse quadro, por meio do relato de experiência de um atendimento em um consultório de enfermagem psiquiátrica no Município do Rio de Janeiro, no ano de 2022.     Gingival fibroblasts behavior on bioactive zirconia and titanium dental implant surfaces produced by a functionally graded technique

    Adding a biological apatite layer to the implant surface enhances bone healing around the implant. Objective: This study aimed to characterize the mechanical properties and test human gingival fibroblasts behavior in contact with Zirconia and Titanium bioactive-modified implant materials. Methodology: 6 groups were considered: Titanium (Ti6Al4V), Ti6Al4V with 5% HA and 5% ßTCP, Zirconia (YTZP), YTZP with 5% HA and 5% ßTCP. For each group, we produced discs using a novel fabrication method for functionally graded materials, under adequate conditions for etching and grit-blasting to achieve equivalent surface microroughness among the samples. Surface roughness (Ra, Rz), water contact angle, shear bond strength, and Vickers hardness were performed. Human gingival fibroblasts immortalized by hTERT gene from the fourth passage, were seeded on discs for 14 days. Cell viability and proliferation were assessed using a resazurin-based method, and cellular adhesion and morphology using field emission gun scanning electron microscopy (FEG-SEM). After 3 days of culture, images of fluorescent nucleic acid stain were collected by confocal laser scanning microscopy (CLSM). Results: Results were presented as mean ± standard deviation (SD). We compared groups using one-way ANOVA with Tukey’s post-hoc test, and significance level was set at p<0.05. After 14 days of culture, cell viability and proliferation were significantly higher in YTZP group than in other groups (p<0.05). Samples of YTZP-ßTCP presented significantly higher wettability (p<0.05); yet, we observed no improvement in cell behavior on this group. Fibroblast spreading and surface density were more evident on YTZP specimens. Adding calcium-phosphate bioactive did not alter the tested mechanical properties; however, Ti6Al4V material shear bond strength was statistically higher than other groups (p<0.05). Conclusion: Adding bioactive materials did not improve soft-tissue cell behavior. When compared to other zirconia and titanium groups, pure zirconia surface improved adhesion, viability and proliferation of fibroblasts. Cell behavior seems to depend on surface chemical composition rather than on surface roughness

    Antimicrobial properties of membrane-active dodecapeptides derived from MSI-78

    Antimicrobial peptides (AMPs) are a class of broad-spectrum antibiotics known by their ability to disrupt bacterial membranes and their low tendency to induce bacterial resistance, arising as excellent candidates to fight bacterial infections. In this study we aimed at designing short 12-mer AMPs, derived from a highly effective and broad spectrum synthetic AMP, MSI-78 (22 residues), by truncating this peptide at the N- and/or C-termini while spanning its entire sequence with 1 amino add (aa) shifts. These designed peptides were evaluated regarding antimicrobial activity against selected gram-positive Staphylococcus strains and the gram-negative Pseudomonas aeruginosa (P. aeruginosa). The short 12-mer peptide CEM1 (GIGMFLKKAKICF) was identified as an excellent candidate to fight P. aeruginosa infections as it displays antimicrobial activity against this strain and selectivity, with negligible toxicity to mammalian cells even at high concentrations. However, in general most of the short 12-mer peptides tested showed a reduction in antimicrobial activity, an effect that was more pronounced for gram-positive Staphylococcus strains. Interestingly, CEM1 and a highly similar peptide differing by only one aa-shift (CEM2: IGKFLKKAKICFG), showed a remarkably contrasting AMP activity. These two peptides were chosen for a more detailed study regarding their mechanism of action, using several biophysical assays and simple membrane models that mimic the mammalian and bacterial lipid composition. We confirmed the correlation between peptide helicity and antimicrobial activity and propose a mechanism of action based on the disruption of the bacterial membrane permeability barrier

    Validity of self-reported measures of weight and height and its impact on nutritional status prediction by body mass index

    Introdução: medidas autorreferidas de massa e estatura têm sido utilizadas para avaliação do estado nutricional em estudos epidemiológicos. Objetivo: verificar a validade da massa e da estatura autorreferidas, bem como do índice de massa corporal (IMC) obtido com estes valores. Método: participaram 395 homens e 549 mulheres . Os dados foram coletados entre estudantes da Universidade Santa Cecília - Santos. Todos os sujeitos referiam a sua massa e estatura, para a realização das medidas em seguida.Utilizou-se o teste t de Student para amostras dependentes para comparação das médias, a correlação linear de Pearson (r) e Coeficiente de Correlação Intraclasse (R) entre as duas formas de obtenção dos resultados. Resultados: os homens autorreferiram de forma válida tanto sua massa (p=0,125; r=0,963; R=0,912) quanto sua estatura (p=0,058; r=0,974; R=0,921), validando a avaliação do estado nutricional. Já as mulheres tenderam a subestimar sua massa corporal (p=0,000; r=0,882; R=0,888) e superestimar sua estatura (p=0,000; r=0,854; R=0,814), causando subestimativa no valor de IMC. Conclusão: para o grupo estudado não seria válido utilizar as medidas autorreferidas de massa e estatura das mulheres para estimativa do seu estado nutricional pelo índice de massa corporal.INTRODUCTION: the nursing staff must inform parents with children suffering from congenital heart defects regarding their needs related to this situation. The most common needs are the seven, as follows: Information related to cardiopathy, promotion of physical activities, adequate diet, care with buccal health, prevention of infectious endocarditis, care during cyanosis crisis and drug administration. OBJECTIVE: to characterize how much information these parents have regarding this disorder. METHOD: systematic mapping on the literature in MEDLINE, Cochrane, CINAHL, LILACS and SciELO databases, from 1997 to 2007, obtaining 17 papers. RESULTS: the following needs, cyanosis crisis, promotion of physical activities and drug administration, were not adequately studied. The majority amongst the 17 papers was concentrated on the remaining four needs. Beside this, parent's knowledge is not satisfactory and is fragmented. It happens either in developed countries as well as in the underdeveloped ones. In general, care is performed by nurses, dental physicians and physicians. Training programs for parents are scarce and only one is described as successful. Changes are mandatory in terms of reorganization of services involving training and support for parents. Besides, validation of programs and protocols of care to promote training and development is required. These programs must be flexible to allow adaptation to clinical situation and to social, cultural and economic determinant factors acting on the family

    A 17-mer Membrane-Active MSI-78 Derivative with Improved Selectivity toward Bacterial Cells

    Antimicrobial peptides are widely recognized as an excellent alternative to conventional antibiotics. MSI-78, a highly effective and broad spectrum AMP, is one of the most promising AMPs for clinical application. In this study, we have designed shorter derivatives of MSI-78 with the aim of improving selectivity while maintaining antimicrobial activity. Shorter 17-mer derivatives were created by truncating MSI-78 at the N- and/or C-termini, while spanning MSI-78 sequence. Despite the truncations made, we found a 17-mer peptide, MSI-78(4-20) (KFLKKAKKFGKAFVKIL), which was demonstrated to be as effective as MSI-78 against the Gram-positive Staphylococcus strains tested and the Gram-negative Pseudomonas aeruginosa. This shorter derivative is more selective toward bacterial cells as it was less toxic to erythrocytes than MSI-78, representing an improved version of the lead peptide. Biophysical studies support a mechanism of action for MSI-78(4-20) based on the disruption of the bacterial membrane permeability barrier, which in turn leads to loss of membrane integrity and ultimately to cell death. These features point to a mechanism of action similar to the one described for the lead peptide MSI-78
