evidence from a systematic review and pooled lesion analysis
Despite claims that lesional mania is associated with right-hemisphere lesions, supporting evidence is scarce, and association with specific brain areas has not been demonstrated. Here, we aimed to test whether focal brain lesions in lesional mania are more often right- than left-sided, and if lesions converge on areas relevant to mood regulation. We thus performed a systematic literature search (PROSPERO registration CRD42016053675) on PubMed and Web-Of-Science, using terms that reflect diagnoses and structures of interest, as well as lesional mechanisms. Two researchers reviewed the articles separately according to PRISMA Guidelines, selecting reports of adult-onset hypomania, mania or mixed state following a focal brain lesion, for pooled-analyses of individual patient data. Eligible lesion images were manually traced onto the corresponding MNI space slices, and lesion topography analyzed using standard brain atlases. Using this approach, data from 211 lesional mania patients was extracted from 114 reports. Among 201 cases with focal lesions, more patients had lesions involving exclusively the right (60.7%) than exclusively the left (11.4%) hemisphere. In further analyses of 56 eligible lesion images, while findings should be considered cautiously given the potential for selection bias of published lesion images, right-sided predominance of lesions was confirmed across multiple brain regions, including the temporal lobe, fusiform gyrus and thalamus. These, and several frontal lobe areas, were also identified as preferential lesion sites in comparisons with control lesions. Such pooled-analyses, based on the most comprehensive dataset of lesional mania available to date, confirm a preferential association with right-hemisphere lesions, while suggesting that several brain areas/circuits, relevant to mood regulation, are most frequently affected.publishersversionpublishe
Early impact of abdominal compartment syndrome on liver, kidney and lung damage in a rodent model
Background: Abdominal compartment syndrome (ACS) sometimes occurs in critically ill patients following
damage control surgery. The purpose of the present study was to develop a model of ACS and to evaluate its
pathologic impact on liver, kidney, and lung morphology.
Methods: Twenty Wistar rats (mass 300\u2013350 g) were randomly divided into four groups: 1) intra-abdominal
hypertension (IAH): a laparotomy was performed and the abdomen packed with cotton until an intra-abdominal
pressure (IAP) of 15 mm Hg was reached; 2) hypovolemia (HYPO): blood was withdrawn until a mean arterial
pressure ~60 mm Hg was reached; 3) IAH + HYPO (to resemble clinical ACS); and 4) sham surgery. After 3
hours of protective mechanical ventilation, the animals were euthanized and the liver, kidney and lungs removed
to examine the degree of tissue damage.
Results: IAH resulted in the following: oedema and neutrophil infiltration in the kidney; necrosis, congestion,
and microsteatosis in the liver; and alveolar collapse, haemorrhage, interstitial oedema, and neutrophil
infiltration in the lungs. Furthermore, IAH was associated with greater cell apoptosis in the kidney, liver and
lungs compared to sham surgery. HYPO led to oedema and neutrophil infiltration in the kidney. The
combination of IAH and HYPO resulted in all the aforementioned changes in lung, kidney and liver tissue, as
well as exacerbation of the inflammatory process in the kidney and liver and kidney cell necrosis and apoptosis.
Conclusions: Intra-abdominal hypertension by itself is associated with kidney, liver and lung damage; when
combined with hypovolemia, it leads to further impairment and organ damage

Mapping mania symptoms based on focal brain damage
BACKGROUND. Although mania is characteristic of bipolar disorder, it can also occur following focal brain damage. Such cases may provide unique insight into brain regions responsible for mania symptoms and identify therapeutic targets. METHODS. Lesion locations associated with mania were identified using a systematic literature search (n = 41) and mapped onto a common brain atlas. The network of brain regions functionally connected to each lesion location was computed using normative human connectome data (resting-state functional MRI, n = 1000) and contrasted with those obtained from lesion locations not associated with mania (n = 79). Reproducibility was assessed using independent cohorts of mania lesions derived from clinical chart review (n = 15) and of control lesions (n = 490). Results were compared with brain stimulation sites previously reported to induce or relieve mania symptoms. RESULTS. Lesion locations associated with mania were heterogeneous and no single brain region was lesioned in all, or even most, cases. However, these lesion locations showed a unique pattern of functional connectivity to the right orbitofrontal cortex, right inferior temporal gyrus, and right frontal pole. This connectivity profile was reproducible across independent lesion cohorts and aligned with the effects of therapeutic brain stimulation on mania symptoms. CONCLUSION. Brain lesions associated with mania are characterized by a specific pattern of brain connectivity that lends insight into localization of mania symptoms and potential therapeutic targets. FUNDING. IMPACT OF ROUX-EN-Y GASTRIC BYPASS SURGERY (RYGB) ON METABOLIC SYNDROME COMPONENTS AND ON THE USE OF ASSOCIATED DRUGS IN OBESE PATIENTS
ABSTRACT BACKGROUND The prevalence of obesity and metabolic syndrome is increasing worldwide and both behavior modification and drug therapy have low adherence. Gastric bypass has shown effective results in both reducing weight and improving comorbidities. OBJECTIVE To evaluate the impact of Roux-en-Y Gastric Bypass Surgery (RYGB) on both metabolic syndrome components and the use of associated drugs in obese patients. METHODS Historical cohort of patients subjected to Roux-en-Y Gastric Bypass Surgery (RYGB) between January 2007 and March 2014 in a private clinic. The sample consisted of 273 obese class II and III individuals, 86.4% of whom were female, with age ≥20 years, followed up for 2 months after surgery. Sociodemographic, anthropometric, biochemical, clinical, and drug-use data were collected from patients’ medical records. RESULTS Significant differences were found in weight, body mass index and waist circumference, after 60 postoperative days. Components for metabolic syndrome diagnosis (hypertension P=0.001; hyperglycemia P<0.001; hypertriglyceridemia P=0.006) were reduced after 60 days of postoperative, with the exception HDL-c (P=0.083). There was a significant reduction in the use of antihypertensive (P<0.001), hypoglycemic (P=0.013), lipid lowering (P<0.001), and antiobesity (P=0.010) drugs and increased use of gastroprotective drugs, vitamins, and minerals (P<0.001) after 60 postoperative days. CONCLUSION Patients subjected to Roux-en-Y Gastric Bypass Surgery exhibited both weight loss and significant improvement not only in metabolic syndrome components (except for HDL-c) but in the use of drugs associated with obesity and metabolic syndrome
Estudo de Caso: Recuperação e reforço estrutural em edifício empresarial no meio urbano / Case Study: Recovery and structural reinforcement in business building in the urban environment
O estudo da durabilidade das estruturas de concreto é um tema complexo que trata de uma quantidade significante de variáveis que afetarão a vida útil de uma construção. Um dos principais motivos de deterioração do concreto é a corrosão, que pode ser iniciada por íons cloreto ou através de dióxido de carbono. Este estudo tem como método um estudo de caso realizado através de inspeções visuais em edifício sede de uma empresa estatal regional, inaugurado em 20 de julho de 1977, localizado na cidade do Recife – PE. Cuja estrutura foi executada toda em concreto armado, com suas fachadas em concreto aparente e placas pré-moldadas. O objeto de estudo sofre em virtude de corrosão nas armaduras do concreto e nele foram identificados os indicadores da manifestação patológica, que são caracterizados por fissuras, trincas, exposição de ferragem e eventualmente desplacamentos, e detalhado o meio de recuperação e reforço da estrutura. Após a inspeção da edificação, constataram-se anomalias relacionadas à corrosão em diversos elementos estruturais. Dentre eles, destaca-se a laje do 1º pavimento do Bloco C, pois apresentava um grau avançado de deterioração, com desplacamento de concreto e perda de seção do aço. Por fim, a recuperação e os reforços empregados na edificação não eliminam a necessidade de inspeção periódica da estrutura, tendo em vista que o processo de corrosão reiniciará após o tratamento
Characterization of the Earwig, Doru lineare, as a Predator of Larvae of the Fall Armyworm, Spodoptera frugiperda: A Functional Response Study
Spodoptera frugiperda Smith (Lepidoptera: Noctuidae) is considered as the most important pest of maize in almost all tropical America. In Argentina, the earwig Doru lineare Eschscholtz (Dermaptera: Forficulidae) has been observed preying on S. frugiperda egg masses in corn crops, but no data about its potential role as a biocontrol agent of this pest have been provided. The predation efficiency of D. lineare on newly emerged S. frugiperda larva was evaluated through a laboratory functional response study. D. lineare showed type II functional response to S. frugiperda larval density, and disc equation estimations of searching efficiency and handling time were (a) = 0.374 and (t) = 182.9 s, respectively.
