Global variation in diabetes diagnosis and prevalence based on fasting glucose and hemoglobin A1c

\ua9 2023, The Author(s).Fasting plasma glucose (FPG) and hemoglobin A1c (HbA1c) are both used to diagnose diabetes, but these measurements can identify different people as having diabetes. We used data from 117 population-based studies and quantified, in different world regions, the prevalence of diagnosed diabetes, and whether those who were previously undiagnosed and detected as having diabetes in survey screening, had elevated FPG, HbA1c or both. We developed prediction equations for estimating the probability that a person without previously diagnosed diabetes, and at a specific level of FPG, had elevated HbA1c, and vice versa. The age-standardized proportion of diabetes that was previously undiagnosed and detected in survey screening ranged from 30% in the high-income western region to 66% in south Asia. Among those with screen-detected diabetes with either test, the age-standardized proportion who had elevated levels of both FPG and HbA1c was 29–39% across regions; the remainder had discordant elevation of FPG or HbA1c. In most low- and middle-income regions, isolated elevated HbA1c was more common than isolated elevated FPG. In these regions, the use of FPG alone may delay diabetes diagnosis and underestimate diabetes prevalence. Our prediction equations help allocate finite resources for measuring HbA1c to reduce the global shortfall in diabetes diagnosis and surveillance

Chemical Analysis of a Diffuse Cloud along a Line of Sight Toward W51: Molecular Fraction and Cosmic-Ray Ionization Rate

    Get PDF
Absorption lines from the molecules OH+, H2O+, and H3+ have been observed in a diffuse molecular cloud along a line of sight near W51 IRS2. We present the first chemical analysis that combines the information provided by all three of these species. Together, OH+ and H2O+ are used to determine the molecular hydrogen fraction in the outskirts of the observed cloud, as well as the cosmic-ray ionization rate of atomic hydrogen. H3+ is used to infer the cosmic-ray ionization rate of H2 in the molecular interior of the cloud, which we find to be zeta_2=(4.8+-3.4)x10^-16 per second. Combining the results from all three species we find an efficiency factor---defined as the ratio of the formation rate of OH+ to the cosmic-ray ionization rate of H---of epsilon=0.07+-0.04, much lower than predicted by chemical models. This is an important step in the future use of OH+ and H2O+ on their own as tracers of the cosmic-ray ionization rate.Comment: 21 pages, 1 figure, 4 table

Immunoprophylaxis against AIDS in macaques with a lentiviral DNA vaccine

    Get PDF
AbstractWe earlier reported that immunization of macaques with a reverse transcriptase-deleted SHIVKU2 (ΔrtSHIVKU2) plasmid that contained HIV-1(HXB2) env and SIV gag–nef induced protection against AIDS caused by challenge virus SHIV89.6P with a heterologous env. We further deleted vif and integrase from ΔrtSHIVKU2 and substituted the 3′LTR with SV40 poly A sequences, creating Δ4SHIVKU2 (M) and a parallel construct containing gag–nef of HIV-1SF2, Δ4SHIVKU2 (H). Six macaques received two intramuscular injections of the (M) DNA, and another six received three injections of the (H) DNA. Three of the latter group received two post-challenge boosts with (M) DNA vaccine. Seven virus control macaques were inoculated with SHIV89.6P. All twelve immunized macaques were challenged with SHIV89.6P virus, and CMI responses were measured by ELISPOT assays.Virus control animals all developed progressive infection, whereas vaccinated macaques from both groups controlled virus replication, with plasma viral loads dropping to undetectable levels between weeks 6 and 126 p.i. This DNA vaccine was efficacious even though it encoded Env, Gag, and Nef that were genetically distinct from the proteins in the challenge virus. The DNA vaccine induced broad-based protection without using viral proteins to boost the immunity

Continuous monitoring of pulse period variations in Her X-1 using Swift/BAT

Context: Monitoring of pulse period variations in accreting binary pulsars is an important tool to study the interaction between the magnetosphere of the neutron star and the accretion disk. While the X-ray flux of the brightest X-ray pulsars have been successfully monitored over many years (e.g. with RXTE/ASM, CGRO/BATSE, Swift/BAT), the possibility to monitor their pulse timing properties continuously has so far been very limited. Aims: In our work we show that the Swift/BAT observations can be used to monitor coherent pulsations of bright X-ray sources and use the Swift archival data to study one of the most enigmatic X-ray pulsars, Hercules X-1. A quasi-continuous monitoring of the pulse period and the pulse period derivative of an X-ray pulsar, here Her X-1, is achieved over a long time (<~ 4 yrs). We compare our observational results with predictions of accretion theory and use them to test different aspects of the physical model of the system. Methods: In our analysis we use the data accumulated with Swift/BAT starting from the beginning of 2005 (shortly after launch) until the present time. To search for pulsations and for their subsequent analysis we used the count rate measured by the BAT detector in the entire field of view. Results: The slope of the correlation between the locally determined spin-up rate and the X-ray luminosity is measured for Her X-1 and found to be in agreement with predictions of basic accretion torque theory. The observed behaviour of the pulse period together with the previously measured secular decrease of the system's orbital period is discussed in the frame of a model assuming ejection of matter close to the inner boundary of the accretion disk.Comment: 7 pages, 5 figures, accepted for publication in Astronomy & Astrophysic

Pseudogap temperature as a Widom line in doped Mott insulators

The pseudogap refers to an enigmatic state of matter with unusual physical properties found below a characteristic temperature T∗T^* in hole-doped high-temperature superconductors. Determining T∗T^* is critical for understanding this state. Here we study the simplest model of correlated electron systems, the Hubbard model, with cluster dynamical mean-field theory to find out whether the pseudogap can occur solely because of strong coupling physics and short nonlocal correlations. We find that the pseudogap characteristic temperature T∗T^* is a sharp crossover between different dynamical regimes along a line of thermodynamic anomalies that appears above a first-order phase transition, the Widom line. The Widom line emanating from the critical endpoint of a first-order transition is thus the organizing principle for the pseudogap phase diagram of the cuprates. No additional broken symmetry is necessary to explain the phenomenon. Broken symmetry states appear in the pseudogap and not the other way around.Comment: 6 pages, 4 figures and supplementary information; published versio

Nigerian scam e-mails and the charms of capital

    Get PDF
So-called '419' or 'advance-fee' e-mail frauds have proved remarkably successful. Global losses to these scams are believed to run to billions of dollars. Although it can be assumed that the promise of personal gain which these e-mails hold out is part of what motivates victims, there is more than greed at issue here. How is it that the seemingly incredible offers given in these unsolicited messages can find an audience willing to treat them as credible? The essay offers a speculative thesis in answer to this question. Firstly, it is argued, these scams are adept at exploiting common presuppositions in British and American culture regarding Africa and the relationships that are assumed to exist between their nations and those in the global south. Secondly, part of the appeal of these e-mails lies in the fact that they appear to reveal the processes by which wealth is created and distributed in the global economy. They thus speak to their readers’ attempts to map or conceptualise the otherwise inscrutable processes of that economy. In the conclusion the essay looks at the contradictions in the official state response to this phenomena
