    Differences in Circulating Dendritic Cell Subtypes in Pregnant Women, Cord Blood and Healthy Adult Women

    Different subtypes of dendritic cells (DC) influence the differentiation of naíve T lymphocytes into T helper type 1 (Th1) and Th2 effector cells. We evaluated the percentages of DC subtypes in peripheral blood from pregnant women (maternal blood) and their cord blood compared to the peripheral blood of healthy non pregnant women (control). Circulating DC were identified by flow cytometry as lineage (CD3, CD14, CD16, CD19, CD20, and CD56)-negative and HLA-DR-positive cells. Subtypes of DC were further characterized as myeloid DC (CD11c+/CD123±), lymphoid DC (CD11c-/CD123+++) and less differentiated DC (CD11c-/CD123±). The frequency of DC out of all nucleated cells was significantly lower in maternal blood than in control (P<0.001). The ratio of myeloid DC/lymphoid DC was significantly higher in maternal blood than in control (P<0.01). HLA-DR expressions of myeloid DC as mean fluorescence intensity (MFI) were significantly less in maternal blood and in cord blood than in control (P<0.001, respectively). The DC differentiation factors, TNF-α and GM-CSF, released from mononuclear cells after lipopolysaccharide stimulation were significantly lower in maternal blood than in control (P<0.01). The distribution of DC subtypes was different in maternal and cord blood from those of non-pregnant women. Their role during pregnancy remains to be determined

    Prevalence and characteristics of foodborne pathogens from slaughtered pig carcasses in Korea

    The introduction of bacteria into slaughterhouses can lead to microbial contamination in carcasses during slaughter, and the initial level of bacteria in carcasses is important because it directly affects spoilage and the shelf life. This study was conducted to investigate the microbiological quality, and the prevalence of foodborne pathogens in 200 carcasses from 20 pig slaughterhouses across Korea. Distribution of microbial counts were significantly higher for aerobic bacteria at 3.01–4.00 log10 CFU/cm2 (42.0%) and 2.01–3.00 log10 CFU/cm2 (28.5%), whereas most of Escherichia coli showed the counts under 1.00 log10 CFU/cm2 (87.0%) (P &lt; 0.05). The most common pathogen isolated from 200 carcasses was Staphylococcus aureus (11.5%), followed by Yersinia enterocolitica (7.0%). In total, 17 S. aureus isolates from four slaughterhouses were divided into six pulsotypes and seven spa types, and showed the same or different types depending on the slaughterhouses. Interestingly, isolates from two slaughterhouses carried only LukED associated with the promotion of bacterial virulence, whereas, isolates from two other slaughterhouses carried one or more toxin genes associated with enterotoxins including sen. In total, 14 Y. enterocolitica isolates from six slaughterhouses were divided into nine pulsotypes, 13 isolates belonging to biotype 1A or 2 carried only ystB, whereas one isolate belonging to bio-serotype 4/O:3 carried both ail and ystA. This is the first study to investigate microbial quality and the prevalence of foodborne pathogens in carcasses from slaughterhouses nationally, and the findings support the need for ongoing slaughterhouse monitoring to improve the microbiological safety of pig carcasses

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Ontology Development for Cyber Violence Victimization in Korean Adolescents

    Since the dynamics and forms of cyber violence evolve at the same rapid pace as technology platforms, we used social big data to develop an ontology for cyber violence victimization in South Korea. Following the seven-stage ontology development process, we extracted keywords from 3,075 social media data sources including online articles, comments, and blogs. Focus group interview data was also obtained from 40 adolescents and 5 cyber violence experts. The analysis identified domains for (1) participants subdivided by age, school level, sex, form, and relationship; (2) protective and risk factors by individuals, family, friends, school, community, and online systems; (3) types by form and method of violence; (4) characteristics by media and distinct features; (5) outcomes by individual and relational/social; and (6) interventions and policies by personal coping strategies, services/interventions, and laws/policies. These findings provide a multi-faceted perspective on cyber violence victimization experiences in South Korea. In addition, the results elucidate the newest types of cyber violence in real-life terms

    Fidelity assessment of the suicide reporting guidelines in Korean newspapers

    Abstract Background With the dishonor of being the highest suicide rated country in Organisation for Economic Co-operation and Development, South Korea should take more initiatives in suicide prevention. Although the role of the media and its relation to actual suicide attempts has been tested and supported by many studies, the suicide reporting guidelines are not well followed. The purpose of this study is to examine how well Korean newspapers adhere to existing guidelines and to suggest limitation and improvements for the current guidelines. Methods Five mainstream newspapers in South Korea, namely, Kyunghyang Shinmun, Hankyoreh, Chosun Ilbo, JoongAng Daily, and Dong-A Ilbo, were chosen for the analysis. Using the Naver news search engine, articles dated from January 1, 2014, to December 31, 2017, were selected with the keyword “suicide” and advanced option “printed newspaper.” However, articles, columns, opinions, and reviews that utilized the word “suicide” in a general context were excluded from the final analysis. Finally, the number of cases was narrowed down to 368. Each article was analyzed using the guideline framework consisting of 13 items: sensational coverage, overstatement, direct wording, method used, details about site/location, photographs, suicide note, generalization, speculation, romanticization, interviews with the bereaved, help-seeking information, and public education. Results More than 60% of the articles included direct wording (63.9%), mentioned the method used (68.2%), and provided details about the site or location (74.5%). Nearly half of the articles revealed the contents of the suicide note (44.6%). Less than 3% of the suicide reports had information about hotline logo or phone numbers (1.4%) and facts regarding suicide and suicide prevention (2.2%). Conclusions Our study revealed that the guidelines were ineffective in their monitoring role and that most of the newspapers were incompliant with many significant guideline items in South Korea. Our findings not only explore the limitations of the current guidelines but also provide an important rationale as to why there should be stronger suicide monitoring regulation or an agency with sufficient authority to prevent suicide in a nation-wide scale

    A Social-Ecological Approach to Understanding the Relationship between Cyberbullying Victimization and Suicidal Ideation in South Korean Adolescents: The Moderating Effect of School Connectedness

    Background: Cyberbullying victimization and suicidal ideation are both ongoing deleterious social problems in South Korea. Using the social-ecological approach, this study examined the association between cyberbullying victimization and suicidal ideation as well as the buffering role of school connectedness in this relationship. Methods: A nationally representative sample of 7333 adolescents from the 2016 Korean Children and Youth Right Study participated in the study. Data were analyzed using descriptive statistics, Wald chi-square test, bivariate correlations, and multivariate logistic regression analyses. Results: Nearly 17.7% of adolescents were cyberbullied, and 28.4% had suicidal ideation in the past 12 months. Cyberbullying victims were at an increased risk of suicidal ideation. The results also found that parental abuse, family dysfunction, and perceived peer relationship stress were positively associated with suicidal ideation, while parental support for autonomy was negatively associated with suicidal ideation. Further, school connectedness moderated on the relationship between cyberbullying victimization and adolescent suicidal ideation. Conclusions: These findings suggest that various stakeholders should consider interventions and preventive programs that address school connectedness when working with adolescents who are victims of cyberbullying and exhibit suicidal behavior

    Complete chloroplast genome of Exochorda serratifolia (Rosaceae)

    Exochorda serratifolia (pearlbush) is a rosid shrub found in northeast Asia, including the Korean peninsula. This ornamental plant has white inflorescences and strong insect resistance; however, its genetic diversity is poorly understood and a complete plastid genome is unavailable. Here, we determined the complete chloroplast genome of E. serratifolia through de novo assembly using next-generation sequencing. The E. serratifolia chloroplast genome was 160,558 bp, comprising a large single-copy (LSC) region of 88,514 bp, a small single-copy (SSC) region of 19,308 bp, and two inverted repeat regions (IRs) of 26,368 bp each. We annotated 112 genes: 78 protein-coding genes, 30 tRNA genes, and four rRNA genes. This reference plastid genome increases our understanding of the phylogenetic relationships of E. serratifolia among Rosaceae plants

    Levels of Indicator Bacteria and Characteristics of Foodborne Pathogens from Carcasses of Cattle Slaughterhouses in Korea

    The initial microbial contamination of carcasses during slaughtering adversely affects spoilage and shelf life and is of global concern for food safety and meat quality. This study evaluated the hygiene and quality using the prevalence of foodborne pathogens and the level of indicator bacteria on 200 carcasses, collecting 10 from each of 20 cattle slaughterhouses in Korea. The distribution of aerobic bacterial count in carcasses was significantly highest at 2.0–3.0 log10 CFU/cm2 (34.1%), whereas the Escherichia coli count was significantly highest at under 1.0 log10 CFU/cm2 (94.0%) (P < 0.05). Clostridium perfringens was most prevalent (60.0% of slaughterhouses; 17.5% of carcasses), followed by Yersinia enterocolitica (30.0% of slaughterhouses; 6.5% of carcasses), Staphylococcus aureus (15.0% of slaughterhouses; 4.0% of carcasses), Listeria monocytogenes 1/2a (5.0% of slaughterhouses; 1.0% of carcasses), Salmonella enterica subsp. enterica serovar Infantis (5.0% of slaughterhouses; 1.0% of carcasses), and Shiga toxin-producing E. coli O:66 (5.0% of slaughterhouses; 0.5% of carcasses). Although 28 C. perfringens isolates from 11 slaughterhouses were divided into 21 pulsotypes, all isolates showed the same toxinotype as type A and only carried the cpa. Interestingly, 83.3% of isolates from two slaughterhouses located in the same province showed resistance to tetracycline. Furthermore, 13 Y. enterocolitica isolates from six slaughterhouses were divided into seven pulsotypes that were divided into biotypes 1A and 2 and serotypes O:5 and O:8, except for isolates that could not be typed. Twelve (92.3%) isolates only carried ystB, but one (7.7%) isolate carried ail and ystA. Moreover, 46.2% of Y. enterocolitica isolates showed multidrug resistance against ampicillin, cefoxitin, and amoxicillin/clavulanic acid. This study supports the need for continuous monitoring of slaughterhouses and hygiene management to improve the microbiological safety of carcasses
