Performance of A1C for the Classification and Prediction of Diabetes
OBJECTIVE Although A1C is now recommended to diagnose diabetes, its test performance for diagnosis and prognosis is uncertain. Our objective was to assess the test performance of A1C against single and repeat glucose measurements for diagnosis of prevalent diabetes and for prediction of incident diabetes. RESEARCH DESIGN AND METHODS We conducted population-based analyses of 12,485 participants in the Atherosclerosis Risk in Communities (ARIC) study and a subpopulation of 691 participants in the Third National Health and Nutrition Examination Survey (NHANES III) with repeat test results. RESULTS Against a single fasting glucose â„126 mg/dl, the sensitivity and specificity of A1C â„6.5% for detection of prevalent diabetes were 47 and 98%, respectively (area under the curve 0.892). Against repeated fasting glucose (3 years apart) â„126 mg/dl, sensitivity improved to 67% and specificity remained high (97%) (AUC 0.936). Similar results were obtained in NHANES III against repeated fasting glucose 2 weeks apart. The accuracy of A1C was consistent across age, BMI, and race groups. For individuals with fasting glucose â„126 mg/dl and A1C â„6.5% at baseline, the 10-year risk of diagnosed diabetes was 88% compared with 55% among those individuals with fasting glucose â„126 mg/dl and A1C 5.7â<6.5%. CONCLUSIONS A1C performs well as a diagnostic tool when diabetes definitions that most closely resemble those used in clinical practice are used as the âgold standard.â The high risk of diabetes among individuals with both elevated fasting glucose and A1C suggests a dual role for fasting glucose and A1C for prediction of diabetes. Although A1C is now recommended for the diagnosis of diabetes (1,2), its precise test performance is uncertain. The lack of a single, clear âgold standardâ poses a challenge for determining the performance of A1C. Previous diagnostic studies of A1C have relied exclusively on a single elevated fasting or 2-h glucose values as gold standards (3â5). However, because glucose determinations are inherently more variable than A1C (6), these convenient gold standards are likely to reduce the apparent accuracy of A1C as a diagnostic test. A stronger gold standard would rely on repeated glucose determinations on different days (2), i.e., the recommended approach to diagnosis of diabetes in clinical practice. Alternatively, A1C and fasting glucose can be compared head-to-head against the subsequent development of clinically diagnosed diabetes as the gold standard. We hypothesized that 1) A1C would perform well as a diagnostic and prognostic test for diabetes across its full range and at the American Diabetes Associationârecommended threshold of 6.5% and 2) that its performance would be best when judged against stronger, most clinically relevant gold standards
Scaffolding School Pupilsâ Scientific Argumentation with Evidence-Based Dialogue Maps
This chapter reports pilot work investigating the potential of Evidence-based Dialogue Mapping to scaffold young teenagersâ scientific argumentation. Our research objective is to better understand pupilsâ usage of dialogue maps created in Compendium to write scientific ex-planations. The participants were 20 pupils, 12-13 years old, in a summer science course for âgifted and talentedâ children in the UK. Through qualitative analysis of three case studies, we investigate the value of dialogue mapping as a mediating tool in the scientific reasoning process during a set of learning activities. These activities were published in an online learning envi-ronment to foster collaborative learning. Pupils mapped their discussions in pairs, shared maps via the online forum and in plenary discussions, and wrote essays based on their dialogue maps. This study draws on these multiple data sources: pupilsâ maps in Compendium, writings in science and reflective comments about the uses of mapping for writing. Our analysis highlights the diversity of ways, both successful and unsuccessful, in which dialogue mapping was used by these young teenagers

The role of microtubule movement in bidirectional organelle transport
We study the role of microtubule movement in bidirectional organelle
transport in Drosophila S2 cells and show that EGFP-tagged peroxisomes in cells
serve as sensitive probes of motor induced, noisy cytoskeletal motions.
Multiple peroxisomes move in unison over large time windows and show
correlations with microtubule tip positions, indicating rapid microtubule
fluctuations in the longitudinal direction. We report the first high-resolution
measurement of longitudinal microtubule fluctuations performed by tracing such
pairs of co-moving peroxisomes. The resulting picture shows that
motor-dependent longitudinal microtubule oscillations contribute significantly
to cargo movement along microtubules. Thus, contrary to the conventional view,
organelle transport cannot be described solely in terms of cargo movement along
stationary microtubule tracks, but instead includes a strong contribution from

Social deprivation and exposure to health promotion. A study of the distribution of health promotion resources to schools in England

It is also known that area deprivation is associated with lower impact health promotion. It is less well known, however, whether deprived areas are less responsive to health promotion, or whether they are less exposed. Using data from a national, school-based campaign to promote vaccination against the human papilloma virus (HPV), the relationship between area deprivation and exposure was examined.
METHODS: Taking advantage of a health promotion campaign to provide information to schools about HPV vaccination, a cross sectional study was conducted to examine the relationship between area level, social deprivation, and take-up of (i.e., exposure to) available health promotion material. The sample was 4,750 schools across England, including government maintained and independent schools. The relationship between area deprivation and exposure was examined using bi- and multivariate logistic regression.
RESULTS: It was found that schools in the least deprived quintile had 1.32 times the odds of requesting health promotion materials than schools in the most deprived areas (p = .01). This effect was independent of the school size, the type of school, and the geographic region.
Conclusion
The relationship between area deprivation and the impact of health promotion may be due, at least in part, to differential levels of exposure. The study was limited in scope, pointing to the need for more research, but also points to potentially important policy implications

only hard logistics, but âmessyâ factors such as the social or political implications of a decision. The Collaborative Operations for Personnel Recovery (Co-OPR) project has
developed decision-support for sensemaking in such scenarios, seeking to exploit the
complementary strengths of human and machine reasoning. Co-OPR integrates the Compendium sensemaking-support tool for real time information and argument mapping, with the I-X artificial intelligence planning and execution framework to support group activity and collaboration. Both share a common model for dealing with issues, the refinement of options for the activities to be performed, handling constraints and recording other information. The tools span the spectrum from being very flexible with few constraints on terminology and content, to knowledge-based relying on rich domain models and formal conceptual models (ontologies). In a personnel recovery experimental simulation of an UN peacekeeping operation, with roles played by military planning staff, the Co-OPR tools were judged by external evaluators to have been very effective
Interparticle interactions:Energy potentials, energy transfer, and nanoscale mechanical motion in response to optical radiation
In the interactions between particles of material with slightly different electronic levels, unusually large shifts in the pair potential can result from photoexcitation, and on subsequent electronic excitation transfer. To elicit these phenomena, it is necessary to understand the fundamental differences between a variety of optical properties deriving from dispersion interactions, and processes such as resonance energy transfer that occur under laser irradiance. This helps dispel some confusion in the recent literature. By developing and interpreting the theory at a deeper level, one can anticipate that in suitable systems, light absorption and energy transfer will be accompanied by significant displacements in interparticle separation, leading to nanoscale mechanical motion
The impact of diabetes on multiple avoidable admissions: a cross-sectional study
Background
Multiple admissions for ambulatory care sensitive conditions (ACSC) are responsible for an important proportion of health care expenditures. Diabetes is one of the conditions consensually classified as an ACSC being considered a major public health concern. The aim of this study was to analyse the impact of diabetes on the occurrence of multiple admissions for ACSC.
Methods
We analysed inpatient data of all public Portuguese NHS hospitals from 2013 to 2015 on multiple admissions for ACSC among adults aged 18 or older. Multiple ACSC users were identified if they had two or more admissions for any ACSC during the period of analysis. Two logistic regression models were computed. A baseline model where a logistic regression was performed to assess the association between multiple admissions and the presence of diabetes, adjusting for age and sex. A full model to test if diabetes had no constant association with multiple admissions by any ACSC across age groups.
Results
Among 301,334 ACSC admissions, 144,209 (47.9%) were classified as multiple admissions and from those, 59,436 had diabetes diagnosis, which corresponded to 23,692 patients. Patients with diabetes were 1.49 times (pâ<â0,001) more likely to be admitted multiple times for any ACSC than patients without diabetes. Younger adults with diabetes (18â39âyears old) were more likely to become multiple users.
Conclusion
Diabetes increases the risk of multiple admissions for ACSC, especially in younger adults. Diabetes presence is associated with a higher resource utilization, which highlights the need for the implementation of adequate management of chronic diseases policies.NOVASaudeinfo:eu-repo/semantics/publishedVersio
