Mirroring everyday clinical practice in clinical trial design: a new concept to improve the external validity of randomized double-blind placebo-controlled trials in the pharmacological treatment of major depression
Background: Randomized, double-blind, placebo-controlled trials constitute the gold standard in clinical research when testing the efficacy of new psychopharmacological interventions in the treatment of major depression. However, the blinded use of placebo has been found to influence clinical trial outcomes and may bias patient
selection.
Discussion: To improve clinical trial design in major depression so as to reflect clinical practice more closely we propose to present patients with a balanced view of the benefits of study participation irrespective of their assignment to placebo or active treatment. In addition every participant should be given the option to finally
receive the active medication. A research agenda is outlined to evaluate the impact of the proposed changes on the efficacy of the drug to be evaluated and on the demographic and clinical characteristics of the enrollment fraction with regard to its representativeness of the eligible population.
Summary: We propose a list of measures to be taken to improve the external validity of double-blind, placebocontrolled trials in major depression. The recommended changes to clinical trial design may also be relevant for other psychiatric as well as medical disorders in which expectations regarding treatment outcome may affect the
outcome itself
ACHADOS BUCAIS EM PACIENTES USUÁRIOS DE CRACK – RELATO DE CASOS
O crack é uma droga ilícita derivada da cocaína e o seu uso freqüente causa uma rápida dependência química. Atualmente, o consumo desta droga tornou-se muito comum no Brasil e é considerado um problema de saúde pública. O efeito imediato do crack é uma intensa euforia e se usado freqüentemente pode causar parada cardíaca, hemorragia pulmonar, pneumonia, infarto de músculos e da pele, úlceras no trato gastrointestinal, lesões bucais e de orofaringe. O seguinte trabalho visa apresentar dois relatos de casos de alguns achados bucais relacionados ao consumo desta droga. Nos dois casos, os pacientes são homens, dependentes químicos do crack, internados no Instituto de Pesquisa e Tratamento do Alcoolismo para tratamento de desintoxicação. No caso 1, o paciente apresentava o dente 11 fraturado e uma hiperceratose reacional no vermelhão do lábio inferior causados pelo trauma contínuo do cachimbo nestas regiões. No paciente do caso 2 foi observada uma tatuagem na mucosa do lábio inferior, agenesia e a permanência de dentes decíduos. O cirurgião dentista deve estar atento a alguns achados bucais que possam estar relacionados ao consumo de drogas ilícitas e, com isso, estar preparado para atender este tipo de paciente, cada vez mais comum nos consultórios odontológicos
Detection, identification, and significance of phytoplasmas in wild grasses in East Africa
Plant-pathogenic phytoplasmas found in wild grasses in East Africa could pose a serious threat to the cultivation of Napier grass, Pennisetum purpureum, the most important livestock fodder in the region. To asses this threat, leaves from plants of 33 grass species were sampled from Mbita, Bungoma, and Busia districts in western Kenya; Tarime district in northern Tanzania; and Busia and Bugiri districts in the eastern Uganda to determine which species host phytoplasmas, the identity of the phytoplasmas, and their relationship with disease symptoms. Phytoplasmas were detected using universal primers based on conserved phytoplasma-specific 16S rDNA sequences from 11 grass species collected. Sequence and phylogenetic analysis revealed the presence of Napier grass stunt-related phytoplasmas in 11 grass species, ‘Candidatus Phytoplasma cynodontis’ in three, and goosegrass white leaf phytoplasma in 2 wild grass species. This study showed that the geographical distribution, diversity of phytoplasmas, and their grass host species in East Africa is greater than antecedently thought and that typical disease symptoms, including white leaf or stunting alone, are not reliable indicators of the presence of phytoplasma. It also shows the need to identify insect vectors responsible for phytoplasma transmission from native grasses to Napier grass or other cereals present in the region
Rethinking Proteasome Evolution: Two Novel Bacterial Proteasomes
The proteasome is a multisubunit structure that degrades proteins. Protein degradation is an essential component of regulation because proteins can become misfolded, damaged, or unnecessary. Proteasomes and their homologues vary greatly in complexity: from HslV (heat shock locus v), which is encoded by 1 gene in bacteria, to the eukaryotic 20S proteasome, which is encoded by more than 14 genes. Despite this variation in complexity, all the proteasomes are composed of homologous subunits. We searched 238 complete bacterial genomes for structures related to the proteasome and found evidence of two novel groups of bacterial proteasomes. The first, which we name Anbu, is sparsely distributed among cyanobacteria and proteobacteria. We hypothesize that Anbu must be very ancient because of its distribution within the cyanobacteria, and that it has been lost in many more recent species. We also present evidence for a fourth type of bacterial proteasome found in a few β-proteobacteria, which we call β-proteobacteria proteasome homologue (BPH). Sequence and structural analyses show that Anbu and BPH are both distinct from known bacterial proteasomes but have homologous structures. Anbu is encoded by one gene, so we postulate a duplication of Anbu created the 20S proteasome. Anbu’s function appears to be related to transglutaminase activity, not the general stress response associated with HslV. We have found different combinations of Anbu, BPH, and HslV within these bacterial genomes, which raises questions about specialized protein degradation systems
Tea Polyphenols and Their Roles in Cancer Prevention and Chemotherapy
Many plant-derived, dietary polyphenols have been studied for their chemopreventive and chemotherapeutic properties against human cancers, including green tea polyphenols, genistein (found in soy), apigenin (celery, parsley), luteolin (broccoli), quercetin (onions), kaempferol (broccoli, grapefruits), curcumin (turmeric), etc. The more we understand their involved molecular mechanisms and cellular targets, the better we could utilize these “natural gifts” for the prevention and treatment of human cancer. Furthermore, better understanding of their structure-activity relationships will guide synthesis of analog compounds with improved bio-availability, stability, potency and specificity. This review focuses on green tea polyphenols and seeks to summarize several reported biological effects of tea polyphenols in human cancer systems, highlight the molecular targets and pathways identified, and discuss the role of tea polyphenols in the prevention and treatment of human cancer. The review also briefly describes several other dietary polyphenols and their biological effects on cancer prevention and chemotherapy
Crustins: enigmatic WAP domain-containing antibacterial proteins from crustaceans
Crustins are antibacterial proteins of ca. 7�14 kDa with a characteristic four-disulphide core-containing whey acidic protein (WAP) domain, expressed by the circulating haemocytes of crustaceans. Over 50 crustin sequences have been now reported from a variety of decapods, including crabs, lobsters, shrimp and crayfish. Three main types seem to occur but all possess a signal sequence at the amino terminus and a WAP domain at the carboxyl end. Differences between types lie in the structure of the central region. Those crustins purified as the native protein or expressed recombinantly all kill Gram-positive bacteria, and gene studies have shown that they are constitutively expressed, often at high levels, but show no consistent patterns of change in expression following injection ofbacteria. This variable response to infection is enigmatic but indicates that these proteins could perform additional functions, perhaps as immune regulators in recovery from wounding, trauma or physiological stress
Antidepressant Response in Major Depressive Disorder: A Meta-Regression Comparison of Randomized Controlled Trials and Observational Studies
To compare response to antidepressants between randomized controlled trials
(RCTs) and observational trials.Published and unpublished studies (from 1989 to 2009) were searched for by 2
reviewers on Medline, the Cochrane library, Embase, clinicaltrials.gov,
Current Controlled Trial, bibliographies and by mailing key organisations
and researchers. RCTs and observational studies on fluoxetine or venlafaxine
in first-line treatment for major depressive disorder reported in English,
French or Spanish language were included in the main analysis. Studies
including patients from a wider spectrum of depressive disorders (anxious
depression, minor depressive episode, dysthymia) were added in a second
analysis. The main outcome was the pre-/post-treatment difference on
depression scales standardised to 100 (17-item or 21-item Hamilton Rating
Scale for Depression or Montgomery and Åsberg Rating Scale) in each
study arm. A meta-regression was conducted to adjust the comparison between
observational studies and RCTs on treatment type, study characteristics and
average patient characteristics. 12 observational studies and 109 RCTs
involving 6757 and 11035 patients in 12 and 149 arms were included in the
main analysis. Meta-regression showed that the standardised treatment
response in RCTs is greater by a magnitude of 4.59 (2.61 to 6.56). Study
characteristics were related to standardised treatment response, positively
(study duration, number of follow-up assessments, outpatients versus
inpatients, per protocol analysis versus intention to treat analysis) or
negatively (blinded design, placebo design). At patient level, response
increased with baseline severity and decreased with age. Results of the
second analysis were consistent with this.Response to antidepressants is greater in RCTs than in observational studies.
Observational studies should be considered as a necessary complement to
RCTs
