    Light from the Cosmic Frontier: Gamma-Ray Bursts

    Gamma-Ray Bursts (GRBs) are the most powerful cosmic explosions since the Big Bang, and thus act as signposts throughout the distant Universe. Over the last 2 decades, these ultra-luminous cosmological explosions have been transformed from a mere curiosity to essential tools for the study of high-redshift stars and galaxies, early structure formation and the evolution of chemical elements. In the future, GRBs will likely provide a powerful probe of the epoch of reionisation of the Universe, constrain the properties of the first generation of stars, and play an important role in the revolution of multi-messenger astronomy by associating neutrinos or gravitational wave (GW) signals with GRBs. Here, we describe the next steps needed to advance the GRB field, as well as the potential of GRBs for studying the Early Universe and their role in the up-coming multi-messenger revolution.Comment: White paper submitted to ESA as a contribution to the deliberations on the science themes for the L2 and L3 mission opportunitie

    Preparative fractionation of a random copolymer (SAN) with respect to either chain length or chemical composition

    The possibilities to fractionate copolymers with respect to their chemical composition on a preparative scale by means of the establishment of liquid/liquid phase equilibria were studied for random copolymers of styrene and acrylonitrile (san). Experiments with solutions of san in toluene have shown that fractionation does in this quasi-binary system, where demixing results from marginal solvent quality, take place with respect to the chain length of the polymer only. On the other hand, if phase separation is induced by a second, chemically different polymer one can find conditions under which fractionation with respect to composition becomes dominant. This opportunity is documented for the quasi-ternary system dmac/san/polystyrene, where the solvent dimethyl acetamide is completely miscible with both polymers. The theoretical reasons for the different fractionation mechanisms are discussed

    Long-term efficiency of infliximab in patients with ankylosing spondylitis : real life data confirm the potential for dose reduction

    Objective: To analyse the treatment outcome of patients with ankylosing spondylitis (AS) in the European AS infliximab cohort (EASIC) study after a total period of 8 years with specific focus on dosage and the duration of intervals between infliximab infusions. Methods: EASIC included patients with AS who had received infliximab for 2 years as part of the ASSERT trial. After that period, rheumatologists were free to change the dose or the intervals of infliximab. Clinical data were status at baseline, end of ASSERT and for a total of 8 years of follow-up. Results: Of the initially 71 patients with AS from EASIC, 55 patients (77.5%) had completed the 8th year of anti-tumour necrosis factor (TNF) treatment. Of those, 48 patients (87.3%) still continued on infliximab. The mean infusion interval increased slightly from 6 to 7.1 +/- 1.5 weeks, while 45.8% patients had increased the intervals up to a maximum of 12 weeks. The mean infliximab dose remained stable over time, with a minimum of 3.1 mg/kg and a maximum of 6.4 mg/kg. In patients receiving <5 mg/kg infliximab, the mean infusion interval increased to 7.0 +/- 1.2 weeks. In total, the mean cumulative dose per patient and per year decreased from 3566.30 to 2973.60 mg. Conclusions: We could observe that over a follow-up of 8 years of treatment with infliximab, >85% patients still remained on the same treatment, without any major safety events. Furthermore, both the infusion intervals and also the mean infliximab dose were modestly reduced in >= 70% of the patients without the loss of clinical efficiency

    Acceleration of lead ions in the CERN PS booster and the CERN PS

    The new CERN Heavy Ion Accelerating Facility also requires besides a new Linac substantial modifications of existing accelerators. They are imposed by the low speed and the low intensity of the ion beam and, crucially at low energy, by the short lifetime of the partially stripped ions due to charge exchange with the atoms of the residual gas. The upgraded vacuum system hits the limits of a non-bakeable machine and consequently the acceleration had to be sped up by all means. In the Booster this led to injection and RF capture on a fast-rising magnet cycle and a new digital RF beam control system. Beam current transformers had to be replaced by new, heavily shielded ones. Other modifications include a new staircase magnet to distribute ions over the four Booster rings, lengthening of septa and kicker pulses, plus new, bakeable extraction septa and an energy stabilizing RF loop on the flat top in the CPS, and a stripper in the transfer line to the SPS

    Healthcare Utilization and Costs in Sepsis Survivors in Germany-Secondary Analysis of a Prospective Cohort Study

    Background: Survivors of sepsis often face long-term sequelae after intensive care treatment. Compared to the period of hospitalization, little is known about the ambulatory healthcare utilization in sepsis patients. The study evaluated healthcare utilization and associated costs of sepsis care including allied health professions after initial hospitalization. Methods: Secondary analysis was performed on data in 210 sepsis patients prospectively enrolled from nine intensive care study centers across Germany. Data was collected via structured surveys among their Primary care (Family-) physicians (PCPs) within the first month after discharge from ICU (baseline) and again at 6, 12 and 24 months after discharge, each relating to the period following the last survey. Costs were assessed by standardized cost unit rates from a health care system’s perspective. Changes in healthcare utilization and costs over time were calculated using the Wilcoxon rank-sum test. Results: Of the 210 patients enrolled, 146 (69.5%) patients completed the 24 months follow-up. In total, 109 patients were hospitalized within the first 6 months post-intensive care. Mean total direct costs per patient at 0–6 months were €17,531 (median: €6047), at 7–12 months €9029 (median: €3312), and at 13–24 months €18,703 (median: €12,828). The largest contributor to the total direct costs within the first 6 months was re-hospitalizations (€13,787 (median: €2965). After this first half year, we observed a significant decline in inpatient care costs for re-hospitalizations (p ≤ 0.001). PCPs were visited by more than 95% of patients over 24 months. Conclusions: Sepsis survivors have high health care utilization. Hospital readmissions are frequent and costly. Highest costs and hospitalizations were observed in more than half of patients within the first six months post-intensive care. Among all outpatient care providers, PCPs were consulted most frequently. Clinical impact: Sepsis survivors have a high healthcare utilization and related costs which persist after discharge from hospital. Within outpatient care, possible needs of sepsis survivors as physiotherapy or psychotherapy seem not to be met appropriately. Development of sepsis aftercare programs for early detection and treatment of complications should be prioritized

    The Novel Object and Unusual Name (NOUN) database: a collection of novel images for use in experimental research

    Many experimental research designs require images of novel objects. Here we introduce the Novel Object and Unusual Name (NOUN) Database. This database contains 64 primary novel object images and additional novel exemplars for ten basic- and nine global-level object categories. The objects’ novelty was confirmed by both self-report and a lack of consensus on questions that required participants to name and identify the objects. We also found that object novelty correlated with qualifying naming responses pertaining to the objects’ colors. Results from a similarity sorting task (and subsequent multidimensional scaling analysis on the similarity ratings) demonstrated that the objects are complex and distinct entities that vary along several featural dimensions beyond simply shape and color. A final experiment confirmed that additional item exemplars comprise both sub- and superordinate categories. These images may be useful in a variety of settings, particularly for developmental psychology and other research in language, categorization, perception, visual memory and related domains
