    Toxoplasmosis in Kosovo pregnant women

    This study presents the initial results of a collaborative project aimed at the evaluation of Toxoplasma seroprevalence in a population of Kosovar pregnant women. The serum samples of 334 pregnant women were tested to detect IgG, IgM, IgG avidity for toxoplasmosis. Data regarding age, occupation, area of origin and education were also obtained for the pregnant women examined; 97/334 (29.4%) resulted positive for IgG antibodies, four of whom (4.1%) were also positive for IgM, (1.2% of the total population examined). All four IgM-positive pregnant women also demonstrated low avidity tests. The rate of IgG seroprevalence found in our study was lower than that observed in various European countries, especially those of western Europe. Conversely, the percentage of recent infections was higher than expected. The higher rate of infections could be the result of a recent toxoplasmosis epidemic in Kosovo, most likely due to the altered hygienic conditions caused by the forced transfer of the ethnic-Albanian population from an area of low (Serbia) to high (Kosovo) toxoplasmosis prevalence

    Mental, Social and Visual Alienation in D’Alessandro’s Photography

    This chapter analyzes the first of several photobooks that illustrated the reform of psychiatric health care in Italy in the 1960s and 1970s: Luciano D’Alessandro’s 1969 Gli esclusi. In 1967, D’Alessandro was invited by the director of the asylum of Nocera Superiore, Sergio Piro, to document through photography the abysmal conditions of the “total institution” that was the pre-reform mental hospital. D’Alessandro first published a small selection of photos, in Popular Photography Italiana (1967), which he then expanded in Gli esclusi. This chapter claims that, in the evolution between the two publications, we can read the complex and multilayered notion of alienation that informed the work of reform, especially that of one of the most famous figures associated with it, Franco Basaglia. By analyzing D’Alessandro’s Gli esclusi through the notion of alienation, this chapter lets what Sekula calls the conditions of “readability” of the photographic message emerge

    Low dose rate brachytherapy (LDR-BT) as monotherapy for early stage prostate cancer in Italy: practice and outcome analysis in a series of 2237 patients from 11 institutions

    OBJECTIVE: Low-dose-rate brachytherapy (LDR-BT) in localized prostate cancer is available since 15 years in Italy. We realized the first national multicentre and multidisciplinary data collection to evaluate LDR-BT practice, given as monotherapy, and outcome in terms of biochemical failure. METHODS: Between May 1998 and December 2011, 2237 patients with early-stage prostate cancer from 11 Italian community and academic hospitals were treated with iodine-125 ((125)I) or palladium-103 LDR-BT as monotherapy and followed up for at least 2 years. (125)I seeds were implanted in 97.7% of the patients: the mean dose received by 90% of target volume was 145 Gy; the mean target volume receiving 100% of prescribed dose (V100) was 91.1%. Biochemical failure-free survival (BFFS), disease-specific survival (DSS) and overall survival (OS) were estimated using Kaplan-Meier method. Log-rank test and multivariable Cox regression were used to evaluate the relationship of covariates with outcomes. RESULTS: Median follow-up time was 65 months. 5- and 7-year DSS, OS and BFFS were 99 and 98%, 94 and 89%, and 92 and 88%, respectively. At multivariate analysis, the National Comprehensive Cancer Network score (p < 0.0001) and V100 (p = 0.09) were correlated with BFFS, with V100 effect significantly different between patients at low risk and those at intermediate/high risk (p = 0.04). Short follow-up and lack of toxicity data represent the main limitations for a global evaluation of LDR-BT. CONCLUSION: This first multicentre Italian report confirms LDR-BT as an excellent curative modality for low-/intermediate-risk prostate cancer. ADVANCES IN KNOWLEDGE: Multidisciplinary teams may help to select adequately patients to be treated with brachytherapy, with a direct impact on the implant quality and, possibly, on outcome

    Labour market effects of social programmes: evidence from Ethiopia’s productive safety net

    This thesis assesses how a large safety-net programme in rural Ethiopia affects local labour markets. Combining repeated cross-sections of the National Labour Force Survey with information on the geographical targeting of the programme, a differencein- differences approach is used to estimate whether employment outcomes have changed differently over time in targeted districts relative to those that did not receive the programme. The results suggest that a combination of public works employment and unconditional transfers to food-insecure households does not affect the local labour supply in this rural economy. I explore whether differential trends and population composition could explain these results, which are more likely due to programme design. Albeit scantily observed, the wages of private sector labourers also seem unaffected, in contrast to findings from similar studies focusing on labour markets from other developing countries outside Africa.</p

    Long-term survivors in an epidemic without recovery

    The analysis of epidemics without recovery often implies the inclusion of long-term survivors in the modeling process. Such individuals are characterized by a peculiarly long sojourn in a state of infection without any sign of progression towards the subsequent evolutionary state of the disease. A parametric modeling of their presence in an epidemic involves non-Markov staging of the infection process and of its development. In the present work, an approach involving survivor function is proposed, along with some remarks on epidemiological indicators and some numerical and graphical examples of its application
