    Automatic segmentation of wrist bone fracture area by K-means pixel clustering from X-ray image

    Early detection of subtle fracture is important particularly for the senior citizens’ quality of life. Naked eye examination from X-ray image may cause false negatives due to operator subjectivity thus computer vision based automatic detection software is much needed in practice.  In this paper, we propose an automatic extraction method for suspisious wrist fracture regions. We apply K-means in pixel clustering to form the candidate part of possible fracture from wrist X-ray image automatically. This method can recover previously detected patterned false cases with edge detection method after fuzzy stretching. The proposed method is successful in 16 out of 20 tested cases in experiment

    Subpleural Pulmonary Hyalinizing Granuloma Presenting as a Solitary Pulmonary Nodule

    AbstractWe introduce a case of pulmonary hyalinizing granuloma presented as a solitary pulmonary nodule located subpleurally. The patient was a 57-year-old man who had abnormal chest roentgenograms showing a solitary pulmonary nodule in the right lower lung field. The nodule was resected for definitive diagnosis and histopathologically proved to be pulmonary hyalinizing granuloma. In previously reported cases, most patients had ill-defined margins and usually bilateral, multiple lesions radiographically. In our case, the subpleural location is an uncommon location of this rare entity

    Generation of Polyclonal Rabbit Antisera Specific to the Zika Virus Capsid Protein

    Zika virus (ZIKV), a mosquito-borne flavivirus, is an emerging zoonotic pathogen closely related to Japanese encephalitis virus, West Nile virus, dengue virus, and yellow fever virus. Although ZIKV infection generally produces only mild symptoms in some infected individuals, it has recently been associated with a growing number of neurological diseases, including Guillain-Barré syndrome in ZIKV-infected adults and microcephaly in infants born to ZIKV-infected women. Like all flaviviruses, ZIKV has a plus-strand RNA genome encoding ten functional proteins (designated C, prM, E, NS1, NS2A, NS2B, NS3, NS4A, NS4B, and NS5). Of these ten, the C (capsid) protein is an essential structural protein required for the formation of infectious viral particles. In order to produce the antiserum specifically recognizing the ZIKV C protein in this study, we expressed and purified the ZIKV C protein as a glutathione-S-transferase (GST) fusion protein in E. coli. The ZIKV C protein-coding region was PCR-amplified using the genomic RNA of ZIKV PRVABC-59, and the amplicons were cloned into the pGEX-4T-1 E. coli expression vector. GST-C fusion proteins were purified using a glutathione sepharose column. Subsequently, the GST-C fusion proteins were used for immunization with rabbits. Western blot analysis using the ZIKV-infected Vero cell lysates were performed to examine the reactivity of the antisera to the ZIKV C protein. Thus, this study provides a useful reagent for the diagnosis and understanding of the viral morphogenesis in the ZIKV-infected cells

    Optimal Design Parameters of a Percussive Drilling System for Efficiency Improvement

    This paper aims to determine the optimal design parameters for percussive drilling systems considering the bit-rock interaction. First, the motion dynamics of a bit impacted by a dropped piston are modeled by impact stress propagation and a rock-breaking mechanism representing the penetration resistance coefficient and unloading constant. Next, the damping vibration behavior of the bit is investigated considering the impact duration and the rock loading/unloading condition. In addition, the proposed dynamics are simplified by adopting two dimensionless parameters representing the bit-piston mass ratio and the rock-piston stiffness ratio. Finally, the drilling efficiency, defined by the energy transmitted from the piston to the rock, is presented in terms of the proposed parameters. The use of optimal design parameters for percussive drilling systems improved the drilling efficiency. These results are applicable to the design and performance estimation of down-the-hole and top-hammer systems

    Development, Characterization, and Application of Two Reporter-Expressing Recombinant Zika Viruses

    Zika virus (ZIKV), a mosquito-borne transplacentally transmissible flavivirus, is an enveloped virus with an ~10.8 kb plus-strand RNA genome that can cause neurological disease. To facilitate the identification of potential antivirals, we developed two reporter-expressing ZIKVs, each capable of expressing an enhanced green fluorescent protein or an improved luminescent NanoLuc luciferase. First, a full-length functional ZIKV cDNA clone was engineered as a bacterial artificial chromosome, with each reporter gene under the cap-independent translational control of a cardiovirus-derived internal ribosome entry site inserted downstream of the single open reading frame of the viral genome. Two reporter-expressing ZIKVs were then generated by transfection of ZIKV-susceptible BHK-21 cells with infectious RNAs derived by in vitro run-off transcription from the respective cDNAs. As compared to the parental virus, the two reporter-expressing ZIKVs grew to lower titers with slower growth kinetics and formed smaller foci; however, they displayed a genome-wide viral protein expression profile identical to that of the parental virus, except for two previously unrecognized larger forms of the C and NS1 proteins. We then used the NanoLuc-expressing ZIKV to assess the in vitro antiviral activity of three inhibitors (T-705, NITD-008, and ribavirin). Altogether, our reporter-expressing ZIKVs represent an excellent molecular tool for the discovery of novel antivirals

    Staphylococcal enterotoxin sensitization in a community-based population : a potential role in adult-onset asthma

    Background: Recent studies suggest that Staphylococcus aureus enterotoxin sensitization is a risk factor for asthma. However, there is a paucity of epidemiologic evidence on adult-onset asthma in community-based populations. Objective: We sought to evaluate the epidemiology and the clinical significance of staphylococcal enterotoxin sensitization in community-based adult populations. Methods: The present analyses were performed using the baseline data set of Korean adult population surveys, consisting of 1080 adults (mean age=60.2years) recruited from an urban and a rural community. Questionnaires, methacholine challenge tests, and allergen skin tests were performed for defining clinical phenotypes. Sera were analysed for total IgE and enterotoxin-specific IgE using ImmunoCAP. Results: Staphylococcal enterotoxin sensitization (0.35kU/L) had a prevalence of 27.0%. Risk factors were identified as male sex, current smoking, advanced age (61years), and inhalant allergen sensitization. Current asthma was mostly adult onset (18years old) and showed independent associations with high enterotoxin-specific IgE levels in multivariate logistic regression tests. In multivariate linear regressions, staphylococcal enterotoxin-specific IgE level was identified as the major determinant factor for total IgE level. Conclusions and Clinical Relevance: Staphylococcal enterotoxin sensitization was independently associated with adult-onset asthma in adult community populations. Strong correlations between the enterotoxin-specific IgE and total IgE levels support the clinical significance. The present findings warrant further studies for the precise roles of staphylococcal enterotoxin sensitization in the asthma pathogenesis

    Tension Pneumothorax after Endoscopic Retrograde Pancreatocholangiogram

    We report a case of tension pneumothorax after an endoscopic sphincterotomy. A 78-yr-old woman presented with progressing dyspnea. She had undergone an endoscopic retrograde cholangiopancreatogram three days before due to acute cholecystitis. She underwent endoscopic sphincterotomy for stone extraction, but the procedure failed. On arrival to our hospital, she complained about severe dyspnea and she had subcutaneous emphysema. A computed tomogram scan revealed severe subcutaneous emphysema, right-side tension pneumothorax, and pneumoretroperitoneum. Contrast media injected through a transnasal biliary drainage catheter spilled from the second portion of the duodenum. A second abdominal computed tomogram showed multiple air densities in the retroperitoneum and peritoneal cavity, which were consistent with panperitonitis. We recommended an emergent laparotomic exploration, but the patient's guardians refused. She died eventually due to septic shock. Endoscopic retrograde cholangiopancreatogram is a popular procedure for biliary and pancreatic diseases, but it can cause severe complications such as intestinal perforation. Besides perforations, air can spread through the abdominal cavity, retroperitoneum, mediastinum, and the neck soft tissue, eventually causing pneumothorax. Early recognition and appropriate management is crucial to an optimal output of gastrointestinal perforation and pneumothorax
