    Mental deterioration in childhood epilepsy

    Mental retardation is detected in 20-30% of children with epilepsy at hospitals specializing in treatment of childhood epilepsy. However, the incidence of mental deterioration in childhood epilepsy is not high. In this study, mental deterioration was found in 52 (1.8%) of the 2,880 children with epilepsy at Okayama University Hospital. The patients showing mental deterioration mostly suffered from specific epileptic syndromes, such as West syndrome, Lennox-Gastaut syndrome, severe myoclonic epilepsy in infancy and epilepsy with continuous spike-waves during slow wave sleep. These types of epilepsy show generalized electroencephalographic (EEG) abnormalities. It is presumed that mental deterioration is caused by the total effects of prolonged diffuse EEG abnormalities and the age of the patients. Antiepileptic drugs exert a relatively minor effect on mental deterioration.</p

    Evoked potentials in patients with Angelman syndrome

    Evoked potentials were studied in four cases with Angelman syndrome. Chromosome 15ql1-13 deletion was proved in two cases and paternal uniparental disomy was proved in the rest. Prolonged photo-evoked eyelid microvibration latencies were noted in all four, while visual evoked potential latencies remained within normal limits in three of four. Interpeak latencies of wave I to wave V in auditory brainstem response were prolonged in two of four. Short latency somatosensory evoked potential was examined in two cases and prolonged interpeak latency of wave P3 to wave N1 was noted in one case. Brink reflex was examined in one case and prolonged R2 latency was noted. These findings suggest that the brainstem is disturbed in cases with Angelman syndrome

    Fabrication and catalytic activity of thermally stable gold nanoparticles on ultrastable Y (USY) zeolites

    Au was deposited on ultrastable Y (USY) zeolites using an ion-exchange method. Up to 5.5 wt% Au was introduced into the NH4-form of USY zeolites. In contrast, deposition of Au hardly took place on the H- and Na-forms of Y-type zeolites, NH4-forms of mordenite, and ZSM-5. Treatment of the Au-loaded USY zeolite in a H2 atmosphere, afforded Au0 nanoparticles. These particles were thermally stable even at 973 K, where their mean particle diameter was 3.7 nm. In contrast, highly aggregated Au particles were observed after thermal treatment at temperatures lower than 523 K, followed by storage in air for a month. The resulting particle sizes were in good correlation with the IR band intensity of the adsorbed CO and the catalytic activity of Au in the aerobic oxidation of benzyl alcohol. The Au nanoparticles showed highest activity when the Au/USY zeolite was thermally treated at 673–973 K. A negligible deactivation was observed after repeating the reaction at least 12 times. In the case of Au/TiO2 catalyst prepared by the deposition-precipitation method, the highest activity was observed at 573 K, which was lower than the temperature used for the Au/USY zeolites.     Proton Magnetic Resonance Spectroscopy in Patients with Migration Disorders

    Proton Magnetic Resonance Spectroscopy (1H-MRS) can be used to detect cerebral metabolites including N-acetylaspartate (NAA),creatine (Cr) and choline (Ch). Hence,clinical applications of this method for neuropediatric diseases can be expected. However,regarding neuronal migration disorders,there have been only a few reported studies. We therefore examined the lH-MRS in six patients with migration disorders,ages ranged from 8months to 28years 10months with a mean of 10years 10months. Investigation was performed using Magnetom H15 (Siemens) with a repetition time of 1500 msec and an echo time of 270msec. The ratio of NAA/Cr,Ch/Cr were examined. The volume of interest with the size of 2 × 2 × 2 ~ 3 × 3 × 5cm3 was chosen in the area including lesions,and a contralateral area without lesions was also investigated. Results were as follows. 1) The ratio of NAA/Cr was low in the area with lesions in all 6cases; 1.41,1.95,2.27 and 1.71 in cases with heterotopic gray matter,0.99 in one case with polymicrogyria,an d 1.30 in one case with hemimegalencephaly,contrasted with a contralataral area without lesions: 1.89, 2.89,2.87,2.55,3.26,2.03,respectively. 2) The ratio of Ch/Cr showed no consistent difference between the area including lesions and contralataral area without lesions. Our findings of a decreased NAA/Cr ratio can be inferred to reflect the decreased numbers of neuronal cell population,or reduced metabolism in the lesions

    Microscopic Aspects of Multipole Properties of Filled Skutterudites

    We discuss low-temperature multipole states of Nd-based filled skutterudites by analyzing a multiorbital Anderson model with the use of a numerical renormalization group method. In order to determine the multipole state, we take a procedure to maximize the multipole susceptibility matrix. Then, it is found that the dominant multipole state is characterized by the mixture of 4u magnetic and 5u octupole moments. The secondary state is specified by 2u octupole. When we further take into account the coupling between ff electrons in degenerate Γ67\Gamma_{67}^{-} (eue_{\rm u}) orbitals and dynamical Jahn-Teller phonons with EgE_{\rm g} symmetry, quadrupole fluctuations become significant at low temperatures in the mixed multipole state with 4u magnetic and 5u octupole moments. Finally, we briefly discuss possible relevance of the present results to actual Nd-based filled skutterudite compounds.Comment: 6 pages, 3 figures.     A Large Hepatic Cyst with Obstructive Jaundice Successfully Treated with Single-Incision Laparoscopic Deroofing

    We herein present a case of hepatic cysts causing obstructive jaundice that was treated with single-incision laparoscopic deroofing. A 72-year-old female patient was referred to hospital due to a large hepatic cyst that compressed the intrahepatic bile ducts. The patient was scheduled to undergo single-incision laparoscopic deroofing. The EZ ACCESS? oval type (Hakko Co. Ltd.) was placed at the umbilicus using a 25-mm incision with two 5-mm trocars. An additional 12-mm port was placed at the left epigastric region. We unroofed and excised the cyst wall using a vessel sealing system in liver segment 4. After surgery, the patient was found to be asymptomatic. The unroofed cysts were completely diminished. Notably, the remnant liver had fairly regenerated. The estimated regeneration volume of the normal liver was 153 cm3. To prevent surgical complications, clinicians should perform adequate management and use of devices. To prevent postoperative recurrence of cysts, performing complete deroofing is essential. Single-incision laparoscopic deroofing contributes to improving the quality of life of patients and should be considered a standard treatment
