    Confronting corruption in the health sector in Vietnam: patterns and prospects

    This repository item contains a single issue of the Health and Development Discussion Papers, an informal working paper series that began publishing in 2002 by the Boston University Center for Global Health and Development. It is intended to help the Center and individual authors to disseminate work that is being prepared for journal publication or that is not appropriate for journal publication but might still have value to readers.Corruption in Vietnam is a national concern which could derail health sector goals for equity, access, and quality. Yet, there is little research on vulnerabilities to corruption or associated factors at the sectoral level. This article examines current patterns of corruption in Vietnam’s health sector, identifies key corruption vulnerabilities, and reviews strategies for addressing corruption in the future. The article builds on the findings and discussion at the sixth Anti-Corruption Dialogue between the Vietnamese Government and the international donor community. Development partners, government agencies, Vietnamese and international non-governmental organizations, media representatives and other stakeholders explored what is known about important problems such as informal payments, procurement corruption, and health insurance fraud. The participants proposed corruption-reduction interventions in the areas of administrative oversight, transparency initiatives and civil society participation, and health reforms to change incentives. The analysis assesses the prospects for success of these interventions given the Vietnamese institutional context, and draws conclusions relevant to addressing health sector corruption in other countries

    Una variante del juego-orĂĄculo de la pichca, diferente de las versiones estudiadas por Gentile Lafaille (1998), facilita el augurio anual para el agropastoralismo en el centro de la provincia de HuarochirĂ­ (departamento de Lima, PerĂș). La nota lĂșdica predomina, pero las prĂĄcticas y creencias en torno al juego tambiĂ©n traen consecuencias prĂĄcticas. En la actualidad, como en el manuscrito quechua de HuarochirĂ­ de 1608 [?], el padrĂłn llamado dualismo alternante sirve para regenerar el orden polĂ­tico frente a los imponderables del clima y de la historia.Une variante du jeu d’oracle pichca, diffĂ©rente de celles dĂ©crites par Gentile Lafaille (1998), procure une augure annuelle pour l’agropastoralisme dans la rĂ©gion centrale de HuarochirĂ­ (dĂ©partement de Lima, PĂ©rou). L’aspect ludique prĂ©domine, mais les croyances et agissements associĂ©s Ă  ce jeu ont Ă©galement des consĂ©quences pratiques.     Melanocortin-4 receptor gene: case-control study and transmission disequilibrium test confirm that functionally relevant mutations are compatible with a major gene effect for extreme obesity

    We initially performed a mutation screen of the coding region of the MC4R in 808 extremely obese children and adolescents and 327 underweight or normal-weight controls allowing for a case-control study. A total of 16 different missense, nonsense, and frameshift mutations were found in the obese study group; five of these have not been observed previously. In vitro assays revealed that nine [the haplotype (Y35X; D37V) was counted as one mutation] of the 16 mutations led to impaired cAMP responses, compared with wild-type receptor constructs. In contrast, only one novel missense mutation was detected in the controls, which did not alter receptor function. The association test based on functionally relevant mutations was positive (P = 0.006, Fisher's exact test, one-sided). We proceeded by screening a total of 1040 parents of 520 of the aforementioned obese young index patients to perform transmission disequilibrium tests. The 11 parental carriers of functionally relevant mutations transmitted the mutation in 81.8% (P = 0.033; exact one-sided McNemar test). These results support the hypothesis that these MC4R mutations represent major gene effects for obesity
