    Oral lichenoid lesions related to contact with dental materials: A literature review

    Oral lichenoid lesions related to contact are defined as oral-cavity eruptions with an identifiable etiology, and are clinically and histologically similar to oral lichen planus. Within this group are found oral lichenoid lesions related to contact with dental materials (OLLC), the most common being those related to silver amalgam. Currently, it remains difficult to diagnose these lesions due to the clinical and histopathological similarity with oral lichen planus and other oral mucosa lesions of lichenoid characteristics. In the present paper, we carry out an updated review of the tests for, and the different characteristics of OLLC, which may aid the diagnosis. For this review, we made searches in the Pubmed® and Cochrane® databases. Among the literature we found several published papers, from which we have used review papers, case papers, cohort studies, case and control studies, and a meta-analysis study. After carrying out this review, we can conclude that the diagnosis of these lesions is still difficult and controversial. However, there are different aspects in the clinical presentation, pathological study and results obtained when replacing suspect materials, which, when taken together, may be useful when establishing the final diagnosis of OLLC

    [EN] The evaluation activities developed within the frame of a project on innovation and improvement of teaching, and some preliminary results, are presented in this paper. The main goal of this project is to work on the design of evaluation activities that truly force the students to put their soft skills (generic competences) into play and, at the same time, that allow the instructors to grade their students’ performance on the technical-scientific competences.An assessment for learning approach is used, so that the evaluation activities are useful for the students to improve their learning experience.[ES] En esta comunicación se presentan las actividades de evaluación desarrolladas en el marco de un Proyecto de Innovación y Mejora Educativa, junto con algunos resultados preliminares. El principal objetivo de este proyecto es el diseño de actividades de evaluación que fuercen a los estudiantes a desarrollar sus competencias transversales, al mismo tiempo que permitan a los profesores evaluar su desempeño en las competencias científico-técnicas.     [EN] This paper presents some activities developed in the last two years from the Innovation and Educational Quality Team of the Universitat Politècnica de València, INTEGRAL, to work and evaluate generic competences in subjects within the field of Mechanical Engineering and Materials Science . The idea is to show examples of learning activities that allow developing these competences in technical-scientific subjects, based on previous experiences of the components of the Team, and drawing conclusions and proposals for improvement after their implementation in recent courses.[ES] En esta comunicación se presentan las actividades más relevantes desarrolladas en los dos últimos cursos desde el Equipo de Innovación y Calidad Educativa de la Universitat Politècnica de València, INTEGRAL, para trabajar y evaluar competencias transversales en asignaturas del ámbito de la Ingeniería Mecánica y de Ciencia de los Materiales.     [EN] This paper presents the results of the experience carried out in the lab-sessions of several subjects of Mechanical Engineering. The aim of this work is to find a common methodology focused on the acquisition and evaluation of the Specific Instrumental Transversal Competence (CT13 in the UPV institutional project). The work has been applied in two types of lab-sessions: computer lab and laboratory. The instrumentation used in each of them has a different nature, while the former is focused on the use and comprehension of a numerical calculation software, in the latter the students must understand the management and purpose of some experimental measurement equipments. In the computer sessions, after an accompanied learning, the students must carry out and justify the steps followed in solving a numerical problem. The evaluation takes into account the level of adequacy of the steps followed throughout the process. In the laboratory practices, the students must demonstrate the ability to correctly select the proper equipment based on the parameter to be measured as well as to define the sequence of treatments in the measurement process. The measurement of the level of acquisition of the CT13 is done through an individual questionnaire.[ES] En este trabajo se presentan los resultados de la experiencia llevada a cabo en las prácticas de diversas asignaturas de Ingeniería Mecánica. El objetivo final es encontrar una metodología unificada enfocada a la adquisición y evaluación de la Competencia Transversal Instrumental Específica (CT13 en el proyecto institucional de la UPV).Se ha trabajado con dos tipos de prácticas: informáticas y de laboratorio. La instrumentación empleada en cada una de ellas tiene distinta naturaleza, mientras las primeras están enfocadas al manejo de un programa de cálculo numérico, en las segundas los estudiantes deben comprender el manejo y la finalidad de diversos equipos de medida experimental.En las prácticas informáticas los alumnos, tras un aprendizaje acompañado, deben ser capaces de realizar y justificar los pasos seguidos en la resolución de un problema numérico. En la evaluación se tiene en cuenta el nivel de adecuación de los pasos seguidos a lo largo del proceso. En las prácticas de laboratorio los alumnos deben demostrar la capacidad de seleccionar correctamente el equipo adecuado en función del parámetro a medir, así como definir la secuencia de tratamientos en el proceso experimental.     Hyperkalemia in Heart Failure Patients in Spain and Its Impact on Guidelines and Recommendations: ESC-EORP-HFA Heart Failure Long-Term Registry

    [Abstract] Introduction and objectives: Hyperkalemia is a growing concern in the treatment of patients with heart failure and reduced ejection fraction because it limits the use of effective drugs. We report estimates of the magnitude of this problem in routine clinical practice in Spain, as well as changes in potassium levels during follow-up and associated factors. Methods: This study included patients with acute (n=881) or chronic (n=3587) heart failure recruited in 28 Spanish hospitals of the European heart failure registry of the European Society of Cardiology and followed up for 1 year. Various outcomes were analyzed, including changes in serum potassium levels and their impact on treatment. Results: Hyperkalemia (K+> 5.4 mEq/L) was identified in 4.3% (95%CI, 3.7%-5.0%) and 8.2% (6.5%-10.2%) of patients with chronic and acute heart failure, respectively, and was responsible for 28.9% of all cases of contraindication to mineralocorticoid receptor antagonist use and for 10.8% of all cases of failure to reach the target dose. Serum potassium levels were not recorded in 291 (10.8%) of the 2693 chronic heart failure patients with reduced ejection fraction. During follow-up, potassium levels increased in 179 of 1431 patients (12.5%, 95%CI, 10.8%-14.3%). This increase was directly related to age, diabetes, and history of stroke and was inversely related to history of hyperkalemia. Conclusions: This study highlights the magnitude of the problem of hyperkalemia in patients with heart failure in everyday clinical practice and the need to improve monitoring of this factor in these patients due to its interference with the possibility of receiving optimal treatment.[Resumen] Introducción y objetivos. La hiperpotasemia es una preocupación creciente en el tratamiento de los pacientes con insuficiencia cardiaca y fracción de eyección reducida, pues limita el uso de fármacos eficaces. Este trabajo ofrece estimaciones de la magnitud de este problema en la práctica clínica habitual en España, los cambios en las concentraciones de potasio en el seguimiento y los factores asociados. Métodos. Pacientes con insuficiencia cardiaca aguda (n = 881) y crónica (n = 3.587) seleccionados en 28 hospitales españoles del registro europeo de insuficiencia cardiaca de la European Society of Cardiology y seguidos 1 año para diferentes desenlaces, incluidos cambios en las cifras de potasio y su impacto en el tratamiento. Resultados. La hiperpotasemia (K+ > 5,4 mEq/l) está presente en el 4,3% (IC95%, 3,7-5,0%) y el 8,2% (6,5-10,2%) de los pacientes con insuficiencia cardiaca crónica y aguda; causa el 28,9% de todos los casos en que se contraindica el uso de antagonistas del receptor de mineralocorticoides y el 10,8% de los que no alcanzan la dosis objetivo. Del total de 2.693 pacientes ambulatorios con fracción de eyección reducida, 291 (10,8%) no tenían registrada medición de potasio. Durante el seguimiento, 179 de 1.431 (12,5%, IC95%, 10,8-14,3%) aumentaron su concentración de potasio, aumento relacionado directamente con la edad, la diabetes mellitus y los antecedentes de ictus e inversamente con los antecedentes de hiperpotasemia. Conclusiones. Este trabajo destaca el problema de la hiperpotasemia en pacientes con insuficiencia cardiaca de la práctica clínica habitual y la necesidad de continuar y mejorar la vigilancia de este factor en estos pacientes por su interferencia en el tratamiento óptimo

    Introduction Two blood brain-derived biomarkers, glial fibrillar acidic protein (GFAP) and ubiquitin carboxy-terminal hydrolase L1 (UCH-L1), can rule out intracranial lesions in patients with mild traumatic brain injury (mTBI) when assessed within the first 12 hours. Most elderly patients were excluded from previous studies due to comorbidities. Biomarker use in elderly population could be affected by increased basal levels. This study will assess the performance of an automated test for measuring serum GFAP and UCH-L1 in elderly patients to predict the absence of intracranial lesions on head CT scans after mTBI, and determine both biomarkers reference values in a non-TBI elderly population.Methods and analysis This is a prospective multicentre observational study on elderly patients (≥65 years) that will be performed in Spain, France and Germany. Two patient groups will be included in two independent substudies. (1) A cohort of 2370 elderly patients (1185<80 years and 1185≥80 years; BRAINI2-ELDERLY DIAGNOSTIC AND PROGNOSTIC STUDY) with mTBI and a brain CT scan that will undergo blood sampling within 12 hours after mTBI. The primary outcome measure is the diagnostic performance of GFAP and UCH-L1 measured using an automated assay for discriminating between patients with positive and negative findings on brain CT scans. Secondary outcome measures include the performance of both biomarkers in predicting early (1 week) and midterm (3 months) neurological status and quality of life after trauma. (2) A cohort of 480 elderly reference participants (BRAINI2-ELDERLY REFERENCE STUDY) in whom reference values for GFAP and UCHL1 will be determined.Ethics and dissemination Ethical approval was obtained from the Institutional Review Boards of Hospital 12 de Octubre in Spain (Re#22/027) and Southeast VI (Clermont Ferrand Hospital) (Re# 22.01782.000095) in France.
