    Determinants of food insecurity among smallholder farmer households in Central America: recurrent versus extreme weather-driven events

    To ensure food security among rural communities under a changing climate, policymakers need information on the prevalence and determinants of food insecurity, the role of extreme weather events in exacerbating food insecurity,and the strategies that farmers use to cope with food insecurity. Using household surveys in Guatemala and Honduras, we explore the prevalence of food insecurity among smallholder farmers on both a recurrent (seasonal)and episodic (resulting from extreme weather events) basis, analyze the factors associated with both types of food insecurity, and document farmer coping strategies. Of the 439 households surveyed, 56% experienced recurrent foodinsecurity, 36% experienced episodic food insecurity due to extreme weather events, and 24% experienced both types. Food insecurity among smallholder farmers was correlated with sociodemographic factors (e.g., age, education,migration) and asset ownership. The factors affecting food insecurity differed between type and prevalence of food insecurity. Our results highlight the urgent need for policies and programs to help smallholder farmers improve theiroverall food security and resilience to extreme weather shocks. Such policies should focus on enhancing farmer education levels, securing land tenure, empowering women, promoting generational knowledge exchange, and providing emergency food support in the lean season or following extreme weather events

    Tamizaje de deleciones en pacientes con distrofia muscular de Duchenne (DMD) o Becker-Kiener (BMD) mediante PCR multiplex en Costa Rica, 1998-2000

    Artículo científico -- Universidad de Costa Rica, Instituto de Investigaciones en Salud. 2001Objetivo: Iniciar los estudios genético moleculares sobre las distrofinopatías en Costa Rica. Materiales y Métodos: Treinta y un pacientes varones, diagnosticados con distrofia muscular, que podrían ser distrofinopáticos fueron reevaluados clínicamente. Veintitrés mostraron un fenotipo de DMD y dos de BMD. Seis no mostraron síntomas definitivos de distrofinopatías. ADN de los pacientes fue analizado mediante PCR-múltiplex en búsqueda de deleciones en el gen de la distrofina. También se realizó un diagnóstico prenatal a una portadora obligada. Resultados: Diez pacientes presentaron deleciones: en uno abarca 23 exones (del 23 al 25), en dos ocho exones (del 45 al 52 y del 12 al 19), en otro siete exones (del 60 al 66), en dos seis exones (del 45 al 50); en cuatro pacientes la deleción comprende un único exón: el 52 en dos de ellos, el 19 y el 44. Los seis pacientes con diagnóstico dudoso no mostraron deleciones. Un hijo de la portadora obligada, afectado con DMD tiene una deleción de 37 exones (del 6 al 42), mientras que el feto no la tiene. Conclusión El traslape de síntomas entre las distintas distrofias musculares, la falta de pruebas diagnósticos de laboratorio y de especialistas en el diagnóstico diferencial evidencian la necesidad de implementar métodos diagnósticos más discriminantes en Costa Rica. La identificación de pacientes con deleciones permite buscar estas mutaciones en las mujeres con posible riesgo de ser portadoras y que así reciban mejor consejo genético.Universidad de Costa Rica.     Collapsing glomerulopathy in sickle cell disease: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Sickle cell disease has been associated with many renal structural and functional abnormalities. Collapsing glomerulopathy or the collapsing variant of focal segmental glomerulosclerosis is a rare clinicopathologic entity in patients with sickle cell disease that requires timely diagnosis and aggressive management.</p> <p>Case presentation</p> <p>In this case report we describe a 21-year-old African-American woman with a medical history of significant sickle cell disease and asthma. She was admitted for pain, decreased urine output, bilateral leg swelling and reported weight gain. During her period of hospitalisation she developed acute renal failure requiring dialysis. Further investigation revealed the collapsing variant of focal segmental glomerulosclerosis.</p> <p>Conclusions</p> <p>Although focal segmental glomerulosclerosis is a common feature of sickle cell nephropathy, the collapsing variant of focal segmental glomerulosclerosis or collapsing glomerulopathy has been rarely documented. Even when other risk factors are controlled, collapsing glomerulopathy has a very poor prognosis. This is a rare case of a patient with massive proteinuria presenting as acute renal failure with a very poor response to corticosteroids and a much faster rate of progression to end-stage renal disease.</p

    Adapting clonally propagated crops to climatic changes: a global approach for taro (Colocasia esculenta (L.) Schott)

    Clonally propagated crop species are less adaptable to environmental changes than those propagating sexually. DNA studies have shown that in all countries where taro (Colocasia esculenta (L.) Schott) has been introduced clonally its genetic base is narrow. As genetic variation is the most important source of adaptive potential, it appears interesting to attempt to increase genetic and phenotypic diversity to strengthen smallholders’ capacity to adapt to climatic changes. A global experiment, involving 14 countries from America, Africa, Asia and the Pacific was conducted to test this approach. Every country received a set of 50 indexed genotypes in vitro assembling significant genetic diversity. After onstation agronomic evaluation trials, the best genotypes were distributed to farmers for participatory on-farm evaluation. Results indicated that hybrids tolerant to taro leaf blight (TLB, Phytophthora colocasiae Raciborski), developed by Hawaii, Papua New Guinea and Samoa breeding programmes outperformed local cultivars in most locations. However, several elite cultivars from SE Asia, also tolerant to TLB, outperformed improved hybrids in four countries and in one country none of the introductions performed better than the local cultivars. Introduced genotypes were successfully crossed (controlled crossing) with local cultivars and new hybrids were produced. For the first time in the history of Aroids research, seeds were exchanged internationally injecting tremendous allelic diversity in different countries.     Performance of an Influenza Rapid Test in Children in a Primary Healthcare Setting in Nicaragua

    Background: Influenza is major public health threat worldwide, yet the diagnostic accuracy of rapid tests in developing country settings is not well described. Methodology/Principal Findings: To investigate the diagnostic accuracy of the QuickVue Influenza A+B test in a primary care setting in a developing country, we performed a prospective study of diagnostic accuracy of the QuickVue Influenza A+B test in comparison to reverse transcriptase-polymerase chain reaction (RT-PCR) in a primary healthcare setting in children aged 2 to 12 years in Managua, Nicaragua. The sensitivity and specificity of the QuickVue test compared to RT-PCR were 68.5 % (95 % CI 63.4, 73.3) and 98.1 % (95 % CI 96.9, 98.9), respectively, for children with a fever or history of a fever and cough and/or sore throat. Test performance was found to be lower on the first day that symptoms developed in comparison to test performance on days two or three of illness.     Higher Rates of Hemolysis Are Not Associated with Albuminuria in Jamaicans with Sickle Cell Disease

    BACKGROUND: Albuminuria is a marker of glomerular damage in Sickle Cell Disease (SCD). In this study, we sought to determine the possible predictors of albuminuria in the two more prevalent genotypes of SCD among the Jamaica Sickle Cell Cohort Study participants. METHODS: An age-matched cohort of 122 patients with HbSS or HbSC genotypes had measurements of their morning urine albumin concentration, blood pressure, body mass index, haematology and certain biochemistry parameters done. Associations of albuminuria with possible predictors including hematological parameters, reticulocyte counts, aspartate aminotransferase (AST) and lactate dehydrogenase (LDH) levels were examined using multiple regression models. RESULTS: A total of 122 participants were recruited (mean age 28.6 years ±2.5 years; 85 HbSS, 37 HbSC). 25.9% with HbSS and 10.8% with HbSC disease had microalbuminuria (urine albumin/creatinine ratio  =  30-300 mg/g of creatinine) whereas 16.5% of HbSS and 2.7% of HbSC disease had macroalbuminuria (urine albumin/creatinine ratio>300 mg/g of creatinine). Mean arterial pressure, hemoglobin levels, serum creatinine, reticulocyte counts and white blood cell counts were statistically significant predictors of albuminuria in HbSS, whereas white blood cell counts and serum creatinine predicted albuminuria in HbSC disease. Both markers of chronic hemolysis, i.e. AST and LDH levels, showed no associations with albuminuria in either genotype. CONCLUSIONS: Renal disease, as evidenced by excretion of increased amounts of albumin in urine due to a glomerulopathy, is a common end-organ complication in SCD. It is shown to be more severe in those with HbSS disease than in HbSC disease. Rising blood pressure, lower hemoglobin levels and higher white blood cell counts are hints to the clinician of impending renal disease, whereas higher rates of hemolysis do not appear to play a role in this complication of SCD

    There Is No Safe Dose of Prions

    Understanding the circumstances under which exposure to transmissible spongiform encephalopathies (TSEs) leads to infection is important for managing risks to public health. Based upon ideas in toxicology and radiology, it is plausible that exposure to harmful agents, including TSEs, is completely safe if the dose is low enough. However, the existence of a threshold, below which infection probability is zero has never been demonstrated experimentally. Here we explore this question by combining data and mathematical models that describe scrapie infections in mice following experimental challenge over a broad range of doses. We analyse data from 4338 mice inoculated at doses ranging over ten orders of magnitude. These data are compared to results from a within-host model in which prions accumulate according to a stochastic birth-death process. Crucially, this model assumes no threshold on the dose required for infection. Our data reveal that infection is possible at the very low dose of a 1000 fold dilution of the dose that infects half the challenged animals (ID50). Furthermore, the dose response curve closely matches that predicted by the model. These findings imply that there is no safe dose of prions and that assessments of the risk from low dose exposure are right to assume a linear relationship between dose and probability of infection. We also refine two common perceptions about TSE incubation periods: that their mean values decrease linearly with logarithmic decreases in dose and that they are highly reproducible between hosts. The model and data both show that the linear decrease in incubation period holds only for doses above the ID50. Furthermore, variability in incubation periods is greater than predicted by the model, not smaller. This result poses new questions about the sources of variability in prion incubation periods.
