    Adalimumab for Treating Moderate-to-Severe Hidradenitis Suppurativa: An Evidence Review Group Perspective of a NICE Single Technology Appraisal

    As part of its single technology appraisal (STA) process, the UK National Institute for Health and Care Excellence (NICE) invited the manufacturer of adalimumab (AbbVie) to submit evidence on the clinical effectiveness and cost effectiveness of adalimumab for the treatment of moderate-to-severe hidradenitis suppurativa (HS). The appraisal assessed adalimumab as monotherapy in adult patients with an inadequate response to conventional systemic HS therapy. The School of Health and Related Research Technology Appraisal Group was commissioned to act as the independent Evidence Review Group (ERG). The ERG produced a critical review of the evidence for the clinical effectiveness and cost effectiveness of the technology based on the company’s submission to NICE. The evidence was mainly derived from three randomised controlled trials comparing adalimumab with placebo in adults with moderate-to-severe HS. The clinical-effectiveness review found that significantly more patients achieved a clinical response in the adalimumab groups than in the control groups but that the treatment effect varied between trials and there was uncertainty regarding its impact on a range of other relevant outcomes as well as long-term efficacy. The company’s submitted Markov model assessed the incremental cost effectiveness of adalimumab versus standard care for the treatment of HS from the perspective of the UK NHS and Personal Social Services (PSS) over a lifetime horizon. The original submitted model, including a patient access scheme (PAS), suggested that the incremental cost-effectiveness ratio (ICER) for adalimumab versus standard care was expected to be £16,162 per quality-adjusted life-year (QALY) gained. Following a critique of the model, the ERG’s preferred base case, which corrected programming errors and structural problems surrounding discontinuation rules and incorporated a lower unit cost for HS surgery, resulted in a probabilistic ICER of £29,725 per QALY gained. Based on additional analyses undertaken by the company and the ERG following the publication of the appraisal consultation document (ACD), the Appraisal Committee concluded that the maximum possible ICER for adalimumab compared with supportive care was between £28,500 and £33,200 per QALY gained but was likely to be lower. The Appraisal Committee recommended adalimumab (with the PAS) for the treatment of active moderate-to-severe HS in adults whose disease has not responded to conventional systemic therapy

    Learning new sensorimotor contingencies:Effects of long-term use of sensory augmentation on the brain and conscious perception

    Theories of embodied cognition propose that perception is shaped by sensory stimuli and by the actions of the organism. Following sensorimotor contingency theory, the mastery of lawful relations between own behavior and resulting changes in sensory signals, called sensorimotor contingencies, is constitutive of conscious perception. Sensorimotor contingency theory predicts that, after training, knowledge relating to new sensorimotor contingencies develops, leading to changes in the activation of sensorimotor systems, and concomitant changes in perception. In the present study, we spell out this hypothesis in detail and investigate whether it is possible to learn new sensorimotor contingencies by sensory augmentation. Specifically, we designed an fMRI compatible sensory augmentation device, the feelSpace belt, which gives orientation information about the direction of magnetic north via vibrotactile stimulation on the waist of participants. In a longitudinal study, participants trained with this belt for seven weeks in natural environment. Our EEG results indicate that training with the belt leads to changes in sleep architecture early in the training phase, compatible with the consolidation of procedural learning as well as increased sensorimotor processing and motor programming. The fMRI results suggest that training entails activity in sensory as well as higher motor centers and brain areas known to be involved in navigation. These neural changes are accompanied with changes in how space and the belt signal are perceived, as well as with increased trust in navigational ability. Thus, our data on physiological processes and subjective experiences are compatible with the hypothesis that new sensorimotor contingencies can be acquired using sensory augmentation

    Förderung des selbstregulierten Lernens durch die Lehrkräftefortbildung «Lernen mit Plan»: Effekte auf fokale Trainingsinhalte und die allgemeine Unterrichtsqualität

    Lehrkräftefortbildungen gelten als nachhaltige Maßnahme der Selbstregulationsförderung bei Schülerinnen und Schülern. Noch immer besteht jedoch Forschungsbedarf zur Implementation der Selbstregulationsförderung im Unterricht und deren Auswirkung auf die allgemeine Unterrichtsqualität. Die viertägige Fortbildung «Lernen mit Plan» vermittelt Haupt- und Werkrealschullehrkräften Strategien zur Selbstregulationsförderung im Mathematikunterricht. Im vorliegenden Beitrag wird untersucht, inwieweit «Lernen mit Plan» die fokal trainierte Förderung der Selbstregulation (selbstregulationsspezifische Unterrichtsqualität) sowie die allgemeine Unterrichtsqualität beeinflusst. Insgesamt 75 Lehrkräfte wurden randomisiert einer Experimentalgruppe (47 Lehrkräfte) sowie einer Wartekontrollgruppe (28 Lehrkräfte) zugewiesen. Die selbstregulationsspezifische sowie allgemeine Unterrichtsqualität wurden in einem prä-, post- und followup- Design über Lehrer- und Schülerfragebögen erhoben. «Lernen mit Plan» ging aus Lehrer- und Schülerperspektive mit einer verstärkten Selbstregulationsförderung und aus Schülerperspektive zusätzlich mit positiven Effekten auf die allgemeine Unterrichtsqualität einher

    Explaining the enigmatic anchoring effect: Mechanisms of selective accessibility

    Results of 3 studies support the notion that anchoring is a special case of semantic priming; specifically, information that is activated to solve a comparative anchoring task will subsequently be more accessible when participants make absolute judgments. By using the logic of priming research, in Study 1 the authors showed that the strength of the anchor effect depends on the applicability of activated information. Study 2 revealed a contrast effect when the activated information was not representative for the absolute judgment and the targets of the 2 judgment tasks were sufficiently different. Study 3 demonstrated that generating absolute judgments requires more time when comparative judgments include an implausible anchor and can therefore be made without relevant target information that would otherwise be accessible.
