    Readmission Rates and Their Impact on Hospital Financial Performance: A Study of Washington Hospitals

    This longitudinal study examines whether readmission rates, made transparent through Hospital Compare, affect hospital financial performance by examining 98 hospitals in the State of Washington from 2012 to 2014. Readmission rates for acute myocardial infarction (AMI), pneumonia (PN), and heart failure (HF) were examined against operating revenues per patient, operating expenses per patient, and operating margin. Using hospital-level fixed effects regression on 276 hospital year observations, the analysis indicated that a reduction in AMI readmission rates is related with increased operating revenues as expenses associated with costly treatments related with unnecessary readmissions are avoided. Additionally, reducing readmission rates is related with an increase in operating expenses. As a net effect, increased PN readmission rates may show marginal increase in operating margin because of the higher operating revenues due to readmissions. However, as readmissions continue to happen, a gradual increase in expenses due to greater use of resources may lead to decreased profitability

    We study the effects of a scalar field, coupled only to neutrinos, on oscillations among weak interaction current eigenstates. The effect of a real scalar field appears as effective masses for the neutrino mass eigenstates, the same for \nbar as for \n. Under some conditions, this can lead to a vanishing of δm2\delta m^2, giving rise to MSW-like effects. We discuss some examples and show that it is possible to resolve the apparent discrepancy in spectra required by r-process nucleosynthesis in the mantles of supernovae and by Solar neutrino solutions.Comment: 9 pages, latex, 1 figur

    Multichannel oscillations and relations between LSND, KARMEN and MiniBooNE, with and without CP violation

    We show by examples that multichannel mixing can affect both the parameters extracted from neutrino oscillation experiments, and that more general conclusions derived by fitting the experimental data under the assumption that only two channels are involved in the mixing. Implications for MiniBooNE are noted and an example based on maximal CP violation displays profound implications for the two data sets (muon-neutrino and muon-antineutrino) of that experiment.Comment: 5 pages 4 figure

    A Model for the Analysis of Caries Occurrence in Primary Molar Tooth Surfaces

    Recently methods of caries quantification in the primary dentition have moved away from summary ‘whole mouth’ measures at the individual level to methods based on generalised linear modelling (GLM) approaches or survival analysis approaches. However, GLM approaches based on logistic transformation fail to take into account the time-dependent process of tooth/surface survival to caries. There may also be practical difficulties associated with casting parametric survival-based approaches in a complex multilevel hierarchy and the selection of an optimal survival distribution, while non-parametric survival methods are not generally suitable for the assessment of supplementary information recorded on study participants. In the current investigation, a hybrid semi-parametric approach comprising elements of survival-based and GLM methodologies suitable for modelling of caries occurrence within fixed time periods is assessed, using an illustrative multilevel data set of caries occurrence in primary molars from a cohort study, with clustering of data assumed to occur at surface and tooth levels. Inferences of parameter significance were found to be consistent with previous parametric survival-based analyses of the same data set, with gender, socio-economic status, fluoridation status, tooth location, surface type and fluoridation status-surface type interaction significantly associated with caries occurrence. The appropriateness of the hierarchical structure facilitated by the hybrid approach was also confirmed. Hence the hybrid approach is proposed as a more appropriate alternative to primary caries modelling than non-parametric survival methods or other GLM-based models, and as a practical alternative to more rigorous survival-based methods unlikely to be fully accessible to most researchers

    A relativistic quark potential model is used to do a systematic search for quasi-stable dibaryon states in the uu, dd, and ss three flavor world. Flavor symmetry breaking and channel coupling effects are included and an adiabatic method and fractional parentage expansion technique are used in the calculations.     Unusual Nucleophilic Addition of Grignard Reagents in the Synthesis of 4-Amino-pyrimidines

    Pyrimidines have always received considerable attention because of their importance in synthesis and elucidation of biochemical roles, in particular that of vitamin B1. Herein, we describe a reaction pathway in a Grignard reagent-based synthesis of substituted pyrimidines. A general synthesis of α-keto-2-methyl-4-amino pyrimidines and their C6-substituted analogues from 4-amino-5-cyano-2-methylpyrimidine is reported. The presence of the nitrile substituent in the starting material also results in an unusual reaction pathway leading to C6-substituted 1,2-dihydropyrimidines. Grignard reagents that give normal pyrimidine products under standard reaction conditions can be 14 switched to give dihydropyrimidines by holding the reaction at 0 °C before quenching
