    Delays in making initial treatment contact after the first onset of mental health disorders in the Argentinean Study of Mental Health Epidemiology

    Aims. While there are effective treatments for psychiatric disorders, many individuals with such disorders do not receive treatment and those that do often take years to get into treatment. Information regarding treatment contact failure and delay in Argentina is needed to guide public health policy and planning. Therefore, this study aimed to provide data on prompt treatment contact, lifetime treatment contact, median duration of treatment delays and socio-demographic predictors of treatment contact after the first onset of a mental disorder.Methods. The Argentinean Study of Mental Health Epidemiology (EAESM) is a multistage probability sample representative of adults (aged 18+) living in large urban areas of Argentina. A total of 2116 participants were evaluated with the World Mental Health Composite International Diagnostic Interview to assess psychiatric diagnosis, treatment contact and delay.Results. Projections of cases that will make treatment contact by 50 years taken from a survival curve suggest that the majority of individuals with a mood (100%) or anxiety disorder (72.5%) in Argentina whose disorder persist for a sufficient period of time eventually make treatment contact while fewer with a substance disorder do so (41.6%). Timely treatment in the year of onset is rare (2.6% for a substance disorder, 14.6% for an anxiety disorder and 31.3% of those with a mood disorder) with mean delays between 8 years for mood disorders and 21 years for anxiety disorders. Younger cohorts are more likely to make treatment contact than older cohorts, whereas those with earlier ages of disorder onset are least likely to make treatment contact. Those with anxiety disorders and major depressive disorder are more likely to make treatment contact when they have comorbid disorders, whereas those with substance use disorders are less likely.Conclusions. Argentina needs to implement strategies to get individuals with substance use disorders into treatment, and to reduce treatment delays for all, but particularly to target early detection and treatment among children and adolescents.Fil: Stagnaro, J.C..     O objetivo deste trabalho foi estabelecer alternativas para indução e sincronização do estro em cabras leiteiras manejadas semi-intensivamente. Foram conduzidos quatro experimentos com 411 cabras na Embrapa-Centro Nacional de Pesquisa deCaprinos, Sobral, CE. No protocolo básico, utilizaram-se esponjas intra-vaginais com 50 mg de acetato de medroxiprogesterona (MAP) por dez dias e aplicação intra-muscular de 100 mig de cloprostenol e 200 UI de gonadotropina coriônica eqüina (eCG) no 8º dia; a inseminação artificial (IA), com sêmen congelado foi feita 38 horas após remoção da esponja. No experimento1 substituiu-se a e CG pelo "efeito macho"; no experimento 2 substituiu-se a dose de MAP para 60 mg; no experimento3 compararam-se diferentes momentos de IA: 38, 44 e 50 horas e no experimento 4 substituiu-se a eCG pela gonadotropina humana (hCG). Nenhuma das alternativas testadas modificou (P>0,05) a prolificidade. A IA em cio natural gerou maior (P<0,05) índice de parição no experimento2(67,7%) e no experimento 4 (73,3%). A dose de 60 mg de MAP permitiu realizar a IA mais tarde (44 horas apósretirar a esponja) sem detrimento da fertilidade. A hCG equivaleu a eCG, se aplicada 48 horas antes de retirar a esponja.<br>The objective of this study was to establish alternatives to induce and synchronize estrus in dairy goats managed under semi-intensive conditions. Four experiments were carried out using 411 goats at the Embrapa-Centro Nacional de Pesquisa de Caprinos, Sobral, CE, Brazil. In the basic protocol, intra-vaginal sponges were used with 50 mg of medroxyprogesterone acetate (MAP) over ten days, associated with intra-muscular shots of cloprostenol, and equine corionic gonadotropin (eCG) at the 8th day. Artificial insemination (AI) with frozen semen took place 38 hours after sponge withdrawal. In the first experiment, eCG was replaced by "buck effect"; in the second experiment, 60 mg MAP replaced the usual dose; the third experiment compared different pre-fixed time for AI: 38, 44 and 50 hours and in the fourth experiment, hCG (human corionic gonadotropin) given at different moments, replaced eCG. Prolificacy was not influenced (P>0.05) by any changes of basic protocol.After natural estrus, AIprovided higher (P<0.05) parturition rates in the second (67.7%) and fourth experiment (73.3%). Sponge with 60mg MAP allowed to inseminate later (44 hours after removal) without impairing fertility. As long as hCG is given 48 hours before sponge removal it results equals to eCG ones

    Association of DSM-IV Posttraumatic Stress Disorder With Traumatic Experience Type and History in the World Health Organization World Mental Health Surveys

    Importance: Previous research has documented significant variation in the prevalence of posttraumatic stress disorder (PTSD) depending on the type of traumatic experience (TE) and history of TE exposure, but the relatively small sample sizes in these studies resulted in a number of unresolved basic questions. Objective: To examine disaggregated associations of type of TE history with PTSD in a large cross-national community epidemiologic data set. Design, Setting, and Participants: The World Health Organization World Mental Health surveys assessed 29 TE types (lifetime exposure, age at first exposure) with DSM-IV PTSD that was associated with 1 randomly selected TE exposure (the random TE) for each respondent. Surveys were administered in 20 countries (n = 34 676 respondents) from 2001 to 2012. Data were analyzed from October 1, 2015, to September 1, 2016. Main Outcomes and Measures: Prevalence of PTSD assessed with the Composite International Diagnostic Interview. Results: Among the 34 676 respondents (55.4% [SE, 0.6%] men and 44.6% [SE, 0.6%] women; mean [SE] age, 43.7 [0.2] years), lifetime TE exposure was reported by a weighted 70.3% of respondents (mean [SE] number of exposures, 4.5 [0.04] among respondents with any TE). Weighted (by TE frequency) prevalence of PTSD associated with random TEs was 4.0%. Odds ratios (ORs) of PTSD were elevated for TEs involving sexual violence (2.7; 95% CI, 2.0-3.8) and witnessing atrocities (4.2; 95% CI, 1.0-17.8). Prior exposure to some, but not all, same-type TEs was associated with increased vulnerability (eg, physical assault; OR, 3.2; 95% CI, 1.3-7.9) or resilience (eg, participation in sectarian violence; OR, 0.3; 95% CI, 0.1-0.9) to PTSD after the random TE. The finding of earlier studies that more general history of TE exposure was associated with increased vulnerability to PTSD across the full range of random TE types was replicated, but this generalized vulnerability was limited to prior TEs involving violence, including participation in organized violence (OR, 1.3; 95% CI, 1.0-1.6), experience of physical violence (OR, 1.4; 95% CI, 1.2-1.7), rape (OR, 2.5; 95% CI, 1.7-3.8), and other sexual assault (OR, 1.6; 95% CI, 1.1-2.3). Conclusion and Relevance: The World Mental Health survey findings advance understanding of the extent to which PTSD risk varies with the type of TE and history of TE exposure. Previous findings about the elevated PTSD risk associated with TEs involving assaultive violence was refined by showing agreement only for repeated occurrences. Some types of prior TE exposures are associated with increased resilience rather than increased vulnerability, connecting the literature on TE history with the literature on resilience after adversity. These results are valuable in providing an empirical rationale for more focused investigations of these specifications in future studies

Background: We report the results of International Nosocomial Infection Control Consortium (INICC) surveillance study from January 2010-December 2015 in 703 intensive care units (ICUs) in Latin America, Europe, Eastern Mediterranean, Southeast Asia, and Western Pacific. Methods: During the 6-year study period, using Centers for Disease Control and Prevention National Healthcare Safety Network (CDC-NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 861,284 patients hospitalized in INICC hospital ICUs for an aggregate of 3,506,562 days. Results: Although device use in INICC ICUs was similar to that reported from CDC-NHSN ICUs, DA-HAI rates were higher in the INICC ICUs: in the INICC medical-surgical ICUs, the pooled rate of central line-associated bloodstream infection, 4.1 per 1,000 central line-days, was nearly 5-fold higher than the 0.8 per 1,000 central line-days reported from comparable US ICUs, the overall rate of ventilator-associated pneumonia was also higher, 13.1 versus 0.9 per 1,000 ventilator-days, as was the rate of catheter-associated urinary tract infection, 5.07 versus 1.7 per 1,000 catheter-days. From blood cultures samples, frequencies of resistance of Pseudomonas isolates to amikacin (29.87% vs 10%) and to imipenem (44.3% vs 26.1%), and of Klebsiella pneumoniae isolates to ceftazidime (73.2% vs 28.8%) and to imipenem (43.27% vs 12.8%) were also higher in the INICC ICUs compared with CDC-NHSN ICUs. Conclusions: Although DA-HAIs in INICC ICU patients continue to be higher than the rates reported in CDC-NSHN ICUs representing the developed world, we have observed a significant trend toward the reduction of DA-HAI rates in INICC ICUs as shown in each international report. It is INICC's main goal to continue facilitating education, training, and basic and cost-effective tools and resources, such as standardized forms and an online platform, to tackle this problem effectively and systematically
