    Effect of pesticides applied in cowpea production on rumen microbial fermentation of cowpea haulms as reflected in in vitro gas production

    The present study assessed the effect of lambda cyhalothrin, cypermethrin and dimethoate residues in cowpea haulm on microbial fermentation using gas syringes as incubators. The lambda cyhalothrin, cypermethrin and dimethoate were applied at the vegetative, flowering and podding stages of the cowpea at the rate of 2.66 mg/L, 5.14 mg/L and 6.68 mg/L of water, respectively. Dimethoate was detected in the cowpea haulm at the highest concentration of 1.38 mg/kg. The haulm with no pesticide treatment was incubated with media containing rumen fluid, and pesticides were added at concentrations of 40 mg/kg, 80 mg/kg and 120 mg/kg. In vitro gas production was measured at 3 h, 6 h, 12 h, 24 h, 48 h, 72 h and 96 h to estimate the rate of gas evolution. Gas production in general was influenced by pesticide application. In general, gas evolution was reduced by increasing levels of lambda cyhalothrin up to 80 mg/kg. However, an increase in gas accumulation was observed with increasing levels of dimethoate, while the application of cypermethrin yielded no noticeable change in gas production. The study indicates that pesticide residues may function as toxins at concentrations greater than those encountered in the field or lethal dose (LD50) and may inhibit the growth of rumen microbes

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Comparison of Two Products of Direct-Fed Microbial Supplementation on the Nutrient Utilization and Ruminal Fermentation in Sheep

    Abstract This experiment was undertaken to evaluate the potential impacts of supplementing two direct-fed microbial (DFM) products, namely Bactozyme and Ru-max, to the diet of 12 male Barki sheep (live body weight 46.6 ± 2.9 kg) on dry matter intake (DMI), apparent total tract digestibility of nutrients, nitrogen balance and rumen fermentation characteristics. The Bactozyme or Ru-max were supplemented at a rate of 1.0 g/head/day, mixed with the concentrate mixture. Animals were randomly allocated into 3 equal groups (n=4) and were subjected to the digestibility trails. The results showed that the inclusion of either products of DFM had no positive impact on DMI, but non-significantly improved the apparent total tract digestibility of dry matter (DM), organic matter (OM) and crude protein (CP). However, the Bactozyme addition increased (P<0.05) the apparent total tract digestibility of neutral detergent fiber (NDF) and acid detergent fiber (ADF). The enhancement of the apparent total tract digestibility of cell wall was not significant for the two of DFM products and a non-significant improvement in cell wall digestion due to the Ru-max supplementation over the control group was found. The DFM products had positive impacts on the average of total digestible nutrients (TDN) and digestible crude protein (DCP) but non significant in comparison with the untreated animals. In addition, the nitrogen balance was improved (P>0.05) by 8 and 13% due to Ru-max and Bactozyme supplementation, respectively in comparison with the control group. The results also revealed that inclusion of DFM products had no impacts on rumen pH 3.0 and 6.0 h after feeding but Bactozyme reduced (P<0.05) the rumen pH 1.0 h after feeding compared to the control group. The inclusion of Bactozyme and Ru-max increased (P>0.05) the NH 3 -N concentration in the rumen at 1.0 and 3.0 h after feeding but the Bactozyme decreased (P<0.05) the NH 3 -N concentration and increased the volatile fatty acids (VFA) at 6.0 h after feeding compared to the control group. Overall, results indicated that the two DFM products had positive impacts on cell wall digestibility, which in turn improves metabolic energy supply and nutrients utilization in ruminants as well

    Effect of some essential oils on in vitro methane emission

    The objectives of this study were to characterise four essential oils (EO) chemically and to evaluate their effect on ruminal fermentation and methane emission in vitro. The investigated EO were isolated from Achillea santolina, Artemisia judaica, Schinus terebinthifolius and Mentha microphylla, and supplemented at four levels (0, 25, 50 and 75 l) to 75ml of buffered rumen fluid plus 0.5 g of substrate. The main components of the EO were piperitone (49.1%) and camphor (34.5%) in A. judaica, 16-dimethyl 15-cyclooactdaiene (60.5%) in A. santolina, piperitone oxide (46.7%) and cis-piperitone oxide (28%) in M. microphylla, and -muurolene (45.3%) and -thujene (16.0%) in S. terebinthifolius. The EO from A. santolina (at 25 and 50 l), and all levels of A. judaica increased the gas production significantly, but S. terebinthifolius (at 50 and 75 l), A. santolina (at 75 l) and all levels of M. microphylla decreased the gas production significantly in comparison with the control. The highest levels of A. santolina and A. judaica, and all doses from M. microphylla EO inhibited the methane production along with a significant reduction in true degradation of dry matter and organic matter, protozoa count and NH3-N concentration.     Effect of an Essential Oil Blend on Dairy Cow Performance during Treatment and Post-Treatment Periods

    Essential oils (EOs) and their active components are attracting a lot of attention as sustainable feed additives due to their known antimicrobial properties against a variety of microorganisms, including bacteria, protozoa, and fungi. They also have biological therapeutic, antioxidant, and flavor properties. They can be used for modifying the rumen environment and improving animal productivity. The present study was designed to investigate the effect of supplementation with 2.5 and 5 g/head/day of an essential oils blend (EOB) of clove, oregano, and juniper in equal proportions on milk production and quality, and physiological status of early lactating Holstein dairy cows for 8 weeks of treatment followed by 8 weeks of posttreatment. The results revealed an improvement in milk yield (p ≤ 0.02) and feed efficiency (p ≤ 0.001) with EOB at a dose of 2.5 g/head/day. The percentages of milk protein and lactose were increased (p ≤ 0.03) with a high dose of EOB inclusion during the treatment period as compared with the control group. Milk fat percentage, fat-corrected milk, and energy-corrected milk were not significantly affected by EOB supplementation. The supplementation of EOB tended to decrease milk total bacterial and somatic cell counts when compared to the control group. The supplementation of EOB at both doses increased serum total protein (p ≤ 0.02) and globulin (p ≤ 0.05), and decreased cholesterol level (p ≤ 0.001) and creatinine (p ≤ 0.001) as compared to the control group. Furthermore, the results show that the low dose of EOB had more desirable effects on milk yield, milk total bacterial count, milk somatic cell count, and feed efficiency than the high dose, suggesting that lower doses may not inhibit rumen microbial populations. Moreover, the low doses of EOB could be used as sustainable natural alternatives to antibiotics in livestock diets

    In vitro evaluation, in vivo quantification, and microbial diversity studies of nutritional strategies for reducing enteric methane production

    The main objective of the present work was to study nutritive strategies for lessening the CH4 formation associated to ruminant tropical diets. In vitro gas production technique was used for evaluating the effect of tannin-rich plants, essential oils, and biodiesel co-products on CH4 formation in three individual studies and a small chamber system to measure CH4 released by sheep for in vivo studies was developed. Microbial rumen population diversity from in vitro assays was studied using qPCR. In vitro studies with tanniniferous plants, herbal plant essential oils derived from thyme, fennel, ginger, black seed, and Eucalyptus oil (EuO) added to the basal diet and cakes of oleaginous plants (cotton, palm, castor plant, turnip, and lupine), which were included in the basal diet to replace soybean meal, presented significant differences regarding fermentation gas production and CH4 formation. In vivo assays were performed according to the results of the in vitro assays. , when supplemented to a basal diet (Tifton-85 hay sp, corn grain, soybean meal, cotton seed meal, and mineral mixture) fed to adult Santa Ines sheep reduced enteric CH4 emission but the supplementation of the basal diet with EuO did not affect ( > 0.05) methane released. Regarding the microbial studies of rumen population diversity using qPCR with DNA samples collected from the in vitro trials, the results showed shifts in microbial communities of the tannin-rich plants in relation to control plant. This research demonstrated that tannin-rich , essential oil from eucalyptus, and biodiesel co-products either in vitro or in vivo assays showed potential to mitigate CH4 emission in ruminants.     Physicians' guideline adherence is associated with long-term heart failure mortality in outpatients with heart failure with reduced ejection fraction: the QUALIFY international registry

    Background: Physicians' adherence to guideline-recommended therapy is associated with short-term clinical outcomes in heart failure (HF) with reduced ejection fraction (HFrEF). However, its impact on longer-term outcomes is poorly documented. Here, we present results from the 18-month follow-up of the QUALIFY registry. Methods and results: Data at 18 months were available for 6118 ambulatory HFrEF patients from this international prospective observational survey. Adherence was measured as a continuous variable, ranging from 0 to 1, and was assessed for five classes of recommended HF medications and dosages. Most deaths were cardiovascular (CV) (228/394) and HF-related (191/394) and the same was true for unplanned hospitalizations (1175 CV and 861 HF-related hospitalizations, out of a total of 1541). According to univariable analysis, CV and HF deaths were significantly associated with physician adherence to guidelines. In multivariable analysis, HF death was associated with adherence level [subdistribution hazard ratio (SHR) 0.93, 95% confidence interval (CI) 0.87–0.99 per 0.1 unit adherence level increase; P = 0.034] as was composite of HF hospitalization or CV death (SHR 0.97, 95% CI 0.94–0.99 per 0.1 unit adherence level increase; P = 0.043), whereas unplanned all-cause, CV or HF hospitalizations were not (all-cause: SHR 0.99, 95% CI 0.9–1.02; CV: SHR 0.98, 95% CI 0.96–1.01; and HF: SHR 0.99, 95% CI 0.96–1.02 per 0.1 unit change in adherence score; P = 0.52, P = 0.2, and P = 0.4, respectively). Conclusion: These results suggest that physicians' adherence to guideline-recommended HF therapies is associated with improved outcomes in HFrEF. Practical strategies should be established to improve physicians' adherence to guidelines. © 2019 The Authors. European Journal of Heart Failure © 2019 European Society of Cardiolog
