Do financial incentives for delivering health promotion counselling work? Analysis of smoking cessation activities stimulated by the quality and outcomes framework
<p>Abstract</p> <p>Background</p> <p>A substantial fraction of UK general practitioners' salaries is now intended to reflect the quality of care provided. This performance-related pay system has probably improved aspects of primary health care but, using the observational data available, disentangling the impacts of different types of targets set within this unique payment system is challenging.</p> <p>Discussion</p> <p>Financial incentives undoubtedly influence GPs' activities, however, those aimed at encouraging GPs' delivery of health promotion counselling may not always have the effects intended. There is strong, observational evidence that targets and incentives intended to increase smoking cessation counselling by GPs have merely increased their propensity to record this activity in patients' medical records. The limitations of using financial incentives to stimulate the delivery of counselling in primary care are discussed and a re-appraisal of their use within UK GPs' performance-related pay system is argued for.</p> <p>Summary</p> <p>The utility of targets employed by the system for UK General Practitioners' performance related pay may be inappropriate for encouraging the delivery of health promotion counselling interventions. An evaluation of these targets is essential before they are further developed or added to.</p
Modifying the Sum Over Topological Sectors and Constraints on Supergravity
The standard lore about the sum over topological sectors in quantum field
theory is that locality and cluster decomposition uniquely determine the sum
over such sectors, thus leading to the usual theta-vacua. We show that without
changing the local degrees of freedom, a theory can be modified such that the
sum over instantons should be restricted; e.g. one should include only
instanton numbers which are divisible by some integer p. This conclusion about
the configuration space of quantum field theory allows us to carefully
reconsider the quantization of parameters in supergravity. In particular, we
show that FI-terms and nontrivial Kahler forms are quantized. This analysis
also leads to a new derivation of recent results about linearized supergravity.Comment: 17 pages, minor change
Axion Protection from Flavor
The QCD axion fails to solve the strong CP problem unless all explicit PQ
violating, Planck-suppressed, dimension n<10 operators are forbidden or have
exponentially small coefficients. We show that all theories with a QCD axion
contain an irreducible source of explicit PQ violation which is proportional to
the determinant of the Yukawa interaction matrix of colored fermions.
Generically, this contribution is of low operator dimension and will
drastically destabilize the axion potential, so its suppression is a necessary
condition for solving the strong CP problem. We propose a mechanism whereby the
PQ symmetry is kept exact up to n=12 with the help of the very same flavor
symmetries which generate the hierarchical quark masses and mixings of the SM.
This "axion flavor protection" is straightforwardly realized in theories which
employ radiative fermion mass generation and grand unification. A universal
feature of this construction is that the heavy quark Yukawa couplings are

Effective Theory Approach to the Spontaneous Breakdown of Lorentz Invariance
We generalize the coset construction of Callan, Coleman, Wess and Zumino to
theories in which the Lorentz group is spontaneously broken down to one of its
subgroups. This allows us to write down the most general low-energy effective
Lagrangian in which Lorentz invariance is non-linearly realized, and to explore
the consequences of broken Lorentz symmetry without having to make any
assumptions about the mechanism that triggers the breaking. We carry out the
construction both in flat space, in which the Lorentz group is a global
spacetime symmetry, and in a generally covariant theory, in which the Lorentz
group can be treated as a local internal symmetry. As an illustration of this
formalism, we construct the most general effective field theory in which the
rotation group remains unbroken, and show that the latter is just the

Variability in Tuberculosis Granuloma T Cell Responses Exists, but a Balance of Pro- and Anti-inflammatory Cytokines Is Associated with Sterilization
Lung granulomas are the pathologic hallmark of tuberculosis (TB). T cells are a major cellular component of TB lung granulomas and are known to play an important role in containment of Mycobacterium tuberculosis (Mtb) infection. We used cynomolgus macaques, a non-human primate model that recapitulates human TB with clinically active disease, latent infection or early infection, to understand functional characteristics and dynamics of T cells in individual granulomas. We sought to correlate T cell cytokine response and bacterial burden of each granuloma, as well as granuloma and systemic responses in individual animals. Our results support that each granuloma within an individual host is independent with respect to total cell numbers, proportion of T cells, pattern of cytokine response, and bacterial burden. The spectrum of these components overlaps greatly amongst animals with different clinical status, indicating that a diversity of granulomas exists within an individual host. On average only about 8% of T cells from granulomas respond with cytokine production after stimulation with Mtb specific antigens, and few “multi-functional” T cells were observed. However, granulomas were found to be “multi-functional” with respect to the combinations of functional T cells that were identified among lesions from individual animals. Although the responses generally overlapped, sterile granulomas had modestly higher frequencies of T cells making IL-17, TNF and any of T-1 (IFN-γ, IL-2, or TNF) and/or T-17 (IL-17) cytokines than non-sterile granulomas. An inverse correlation was observed between bacterial burden with TNF and T-1/T-17 responses in individual granulomas, and a combinatorial analysis of pair-wise cytokine responses indicated that granulomas with T cells producing both pro- and anti-inflammatory cytokines (e.g. IL-10 and IL-17) were associated with clearance of Mtb. Preliminary evaluation suggests that systemic responses in the blood do not accurately reflect local T cell responses within granulomas
Theoretical analysis of the mechanisms of a gender differentiation in the propensity for orthostatic intolerance after spaceflight
<p>Abstract</p> <p>Background</p> <p>A tendency to develop reentry orthostasis after a prolonged exposure to microgravity is a common problem among astronauts. The problem is 5 times more prevalent in female astronauts as compared to their male counterparts. The mechanisms responsible for this gender differentiation are poorly understood despite many detailed and complex investigations directed toward an analysis of the physiologic control systems involved.</p> <p>Methods</p> <p>In this study, a series of computer simulation studies using a mathematical model of cardiovascular functioning were performed to examine the proposed hypothesis that this phenomenon could be explained by basic physical forces acting through the simple common anatomic differences between men and women. In the computer simulations, the circulatory components and hydrostatic gradients of the model were allowed to adapt to the physical constraints of microgravity. After a simulated period of one month, the model was returned to the conditions of earth's gravity and the standard postflight tilt test protocol was performed while the model output depicting the typical vital signs was monitored.</p> <p>Conclusions</p> <p>The analysis demonstrated that a 15% lowering of the longitudinal center of gravity in the anatomic structure of the model was all that was necessary to prevent the physiologic compensatory mechanisms from overcoming the propensity for reentry orthostasis leading to syncope.</p
Notes on Operator Equations of Supercurrent Multiplets and the Anomaly Puzzle in Supersymmetric Field Theories
Recently, Komargodski and Seiberg have proposed a new type of supercurrent
multiplet which contains the energy-momentum tensor and the supersymmetry
current consistently. In this paper we study quantum properties of the
supercurrent in renormalizable field theories. We point out that the new
supercurrent gives a quite simple resolution to the classic problem, called the
anomaly puzzle, that the Adler-Bardeen theorem applied to an R-symmetry current
is inconsistent with all order corrections to functions. We propose an
operator equation for the supercurrent in all orders of perturbation theory,
and then perform several consistency checks of the equation. The operator
equation we propose is consisitent with the one proposed by Shifman and
Vainshtein, if we take some care in interpreting the meaning of non-conserved

Goldstino superfields for spontaneously broken N=2 supersymmetry
We examine spontaneously broken N=2 supersymmetry in four dimensions and
associate a spinor superfield with each Goldstino via a finite supersymmetry
transformation with parameters that are the Grassmann coordinates of N=2
superspace. Making use of a special choice of coset parametrization allows us
to develop a version of nonlinearly realized N=2 supersymmetry for which the
associated Goldstino superfields are defined on harmonic superspace, thereby
providing a natural mechanism for construction of a Goldstino action. The
corresponding superfield Lagrangian is an O(4) multiplet. This property is used
to reformulate the Goldstino action in projective superspace and in
conventional N=2 superspace. We show how to generate matter couplings of the
Goldstinos to supersymmetric matter using the N=2 harmonic, projective and full
superspaces. As a bi-product of our consideration, we also derive an N=2 chiral

Screening for coping style increases the power of gene expression studies
Background: Individuals of many vertebrate species show different stress coping styles and these have a striking influence on how gene expression shifts in response to a variety of challenges. Principal Findings: This is clearly illustrated by a study in which common carp displaying behavioural predictors of different coping styles (characterised by a proactive, adrenaline-based or a reactive, cortisol-based response) were subjected to inflammatory challenge and specific gene transcripts measured in individual brains. Proactive and reactive fish differed in baseline gene expression and also showed diametrically opposite responses to the challenge for 80% of the genes investigated. Significance: Incorporating coping style as an explanatory variable can account for some the unexplained variation that is common in gene expression studies, can uncover important effects that would otherwise have passed unnoticed and greatly enhances the interpretive value of gene expression data
Development and implementation of the Structured Training Trainer Assessment Report (STTAR) in the English National Training Programme for laparoscopic colorectal surgery
Background: There is a lack of educational tools available for surgical teaching critique, particularly for advanced laparoscopic surgery. The aim was to develop and implement a tool that assesses training quality and structures feedback for trainers in the English National Training Programme for laparoscopic colorectal surgery. Methods: Semi-structured interviews were performed and analysed, and items were extracted. Through the Delphi process, essential items pertaining to desirable trainer characteristics, training structure and feedback were determined. An assessment tool (Structured Training Trainer Assessment Report—STTAR) was developed and tested for feasibility, acceptability and educational impact. Results: Interview transcripts (29 surgical trainers, 10 trainees, four educationalists) were analysed, and item lists created and distributed for consensus opinion (11 trainers and seven trainees). The STTAR consisted of 64 factors, and its web-based version, the mini-STTAR, included 21 factors that were categorised into four groups (training structure, training behaviour, trainer attributes and role modelling) and structured around a training session timeline (beginning, middle and end). The STTAR (six trainers, 48 different assessments) demonstrated good internal consistency (α = 0.88) and inter-rater reliability (ICC = 0.75). The mini-STTAR demonstrated good inter-item reliability (α = 0.79) and intra-observer reliability on comparison of 85 different trainer/trainee combinations (r = 0.701, p = <0.001). Both were found to be feasible and acceptable. The educational report for trainers was found to be useful (4.4 out of 5). Conclusions: An assessment tool that evaluates training quality was developed and shown to be reliable, acceptable and of educational value. It has been successfully implemented into the English National Training Programme for laparoscopic colorectal surgery
