    Role of myocardial properties and pacing lead location on ECG in personalized paced heart models

    Personalised cardiac models were built from the computed tomography imaging data for two patients with implanted cardiac resynchronisation therapy devices. The cardiac models comprised a biventricular model of myocardial electrophysiology coupled with a model of the torso to simulate the body surface potential map. The models were verified against electrocardiogams (ECG) recorded in the patients from 240 leads on the body surface under left ventricular pacing. The simulated ECG demonstrated a significant sensitivity to the myocardial anisotropy and location of the pacing electrode tip in the models. An apicobasal cellular heterogeneity was shown to be less significant for the ECG pattern at the paced-ventricle activation than that showed earlier by Keller and co-authors (2012) for the normal activation sequence. © 2017 IEEE Computer Society.     Cognitive modeling of informatization influence on socio-economic indicators of the region

    The article discusses topical issues of the influence of informatization on the development of the country’s regions in the conditions of the modern unstable world. The nature of the development of a region can be reflected and understood on the basis of qualitative and quantitative information about its socio-economic indicators, about their relationship and trends in their changes under the influence of internal and external factors. At the same time, information can most often be incomplete, difficult to access, untimely, contradictory, etc. Therefore, in this paper, it is proposed to use a cognitive approach and cognitive modeling of complex systems to overcome the problems of information deficiency by imitating cognitive modeling of the structure and behavior of a complex regional system. The simulation was carried out using the author’s CMCS (Cognitive Modeling Complex System) software system. The results of multi-stage cognitive modeling, consisting in the development of cognitive maps “Influence of ICT on the state of the region” and “Digitalization of the republic” (according to the socio-economic state of the Republic of Dagestan), analysis of structural properties and modeling scenarios for the development of situations on the model are presented. Scenarios make it possible to foresee the ways of possible development of the system under the influence of various factors, including the factor of informatization

Dysferlinopathies are a form of limb-girdle muscle dystrophy type 2B with an incidence ranging from 1:1,300 to 1:200,000 in different populations. In 1996, Dr. S. N. Illarioshkin described a family from the Botlikhsky district of Dagestan, where limb-girdle muscle dystrophy type 2B and Miyoshi myopathy were diagnosed in 12 members from three generations of a large Avar family. In 2000, a previously undescribed mutation in the DYSF gene (c.TG573/574AT; p. Val67Asp) was detected in the affected members of this family. Twenty years later, in this work, we re-examine five known and seven newly affected family members previously diagnosed with dysferlinopathy. We observed disease progression in family members who were previously diagnosed and noted obvious clinical polymorphism of the disease. A typical clinical case is provided

    Venous thrombosis and thromboembolic complications are an important public health problem. According to current recommendations for the diagnosis, treatment and prevention of venous thromboembolic complications, patients are stratified by the risk of pulmonary embolism, by the risk of death in pulmonary embolism, respectively, diagnostic and therapeutic tactics are recommended. Adherence to the recommendations allows to reduce mortality from pulmonary embolism, in the opposite case, mortality from pulmonary embolism remains high due to the problems of rapid diagnosis and risk-adjusted therapy. If pulmonary embolism is a frequent complication of venous thrombosis, then venous gangrene is a rather rare complication, which is also characterized by high mortality. A rare case of a combination of two serious complications of deep vein thrombosis is described: thromboembolism of the pulmonary artery and venous gangrene of both lower extremities. Late diagnosis and late treatment led to amputation of both lower extremities, performed in the presence of a floating thrombus in the inferior vena cava and right heart. The causes of late diagnosis and the complexity of the treatment of simultaneously developed two complications are analyzed.Венозные тромбозы и тромбоэмболические осложнения являются актуальной проблемой здравоохранения. Согласно современным рекомендациям по диагностике, лечению и профилактике венозных тромбоэмболических осложнений, пациенты стратифицированы по риску развития тромбоэмболии легочной артерии (ТЭЛА), по риску смерти при ТЭЛА, соответственно рекомендована диагностическая и лечебная тактика. Приверженность рекомендациям позволяет уменьшить летальность от ТЭЛА, в противном случае летальность от ТЭЛА остается высокой из-за проблем быстрой диагностики и адаптированной к риску терапии. Если ТЭЛА – это частое осложнение тромбоза глубоких вен (ТГВ), то венозная гангрена – довольно редкое, однако также ассоциированное с высокой летальностью. В статье приведено редкое клиническое наблюдение сочетания сразу двух тяжелых осложнений ТГВ: ТЭЛА и венозной гангрены обеих нижних конечностей. Несвоевременная диагностика и запоздалое лечение привели к ампутации обеих нижних конечностей, выполненной при наличии флотирующего тромба в нижней полой вене и правых отделах сердца. Анализируются причины несвоевременной диагностики и сложность разработки индивидуальной стратегии лечения одновременно развившихся осложнений

    The article presents a clinical case of the successful complex treatment of a patient L., 58 years old, with diabetic neuroosteoarthropathy in the middle part of the foot (Charcot's foot), complicated by a deep plantar phlegmon. The strategy of complex treatment, perioperative management and rehabilitation is described in detail.     The constant increase in the number of patients with diabetes mellitus, among whom the incidence of long-term non-healing wounds is many times higher than in the general population, requires constant analysis of treatment protocols and their correction, if necessary. The peculiarities of the wound healing process in the presence of diabetes mellitus include phase perversion and reduced reparative potential. Another problem is the infectious process taking place against the background of diabetes mellitus. Its features include a tendency towards chronicity, frequent persistence of resistant and multiresistive forms of bacteria, and the formation of biofilms. All these factors are pushing to search for new approaches to treatment, and one of the dynamically developing areas is additional treatment of wounds with various types of physical energies. On the one hand, the use of negative pressure, ultrasound, plasma flows, a pulsating jet of liquid, etc. of the wound healing process has been studied for several decades. At the same time, there are still many unanswered problems. One of them is the effectiveness of ultrasonic cavitation in the treatment of purulent-necrotic complications of the neuro-ischemic diabetic foot in the presence of biofilm forms of bacteria in the wound.Постоянный рост количества пациентов с сахарным диабетом (СД), среди которых частота появления длительно незаживающих ран многократно выше, чем в популяции в целом, требует постоянного анализа протоколов лечения и их коррекции при необходимости. К особенностям течения раневого процесса при наличии СД следует отнести извращенность фаз и сниженный репаративный потенциал. Еще одна проблема – инфекционный процесс, протекающий на фоне СД. К его особенностям относят тенденцию к хронизации, частое персистирование резистентных и полирезистентных форм бактерий, образование биопленок. Все эти факторы подталкивают к поиску новых подходов к лечению, и одним из динамически развивающихся направлений является дополнительная обработка ран различными видами физических энергий. С одной стороны, использование влияния отрицательного давления, ультразвука, плазменных потоков, пульсирующей струи жидкости и проч. на течение раневого процесса исследуется уже несколько десятилетий. Вместе с тем есть еще много нерешенных проблем. Одна из них – эффективность ультразвуковой кавитации при лечении гнойно-некротических осложнений нейроишемической формы синдрома диабетической стопы (СДС) при наличии в ране биопленочных форм бактерий
