    Clinical audit in veterinary practice: theory v reality

    CLINICAL AUDIT is an effective tool for assessing and improving the clinical care provided to patients. Good guidance has previously been provided in the veterinary literature as to how to conduct clinical audit in veterinary practice (Mosedale 1998, Viner 2009, 2010, 2012, Dunn 2012, RCVS Knowledge 2015). These resources go into depth about how to conduct audit and the types of topics to choose. However, the combination of the limited veterinary evidence-base and the reality of practice makes traditional clinical audit as per the framework derived from the medical field challenging to implement in the veterinary setting. Despite this, it can still be a rewarding and valuable tool to use in practice

    A simplified (modified) Duke Activity Status Index (M-DASI) to characterise functional capacity: A secondary analysis of the Measurement of Exercise Tolerance before Surgery (METS) study

    Background Accurate assessment of functional capacity, a predictor of postoperative morbidity and mortality, is essential to improving surgical planning and outcomes. We assessed if all 12 items of the Duke Activity Status Index (DASI) were equally important in reflecting exercise capacity. Methods In this secondary cross-sectional analysis of the international, multicentre Measurement of Exercise Tolerance before Surgery (METS) study, we assessed cardiopulmonary exercise testing and DASI data from 1455 participants. Multivariable regression analyses were used to revise the DASI model in predicting an anaerobic threshold (AT) >11 ml kg −1 min −1 and peak oxygen consumption (VO 2 peak) >16 ml kg −1 min −1, cut-points that represent a reduced risk of postoperative complications. Results Five questions were identified to have dominance in predicting AT>11 ml kg −1 min −1 and VO 2 peak>16 ml.kg −1min −1. These items were included in the M-DASI-5Q and retained utility in predicting AT>11 ml.kg −1.min −1 (area under the receiver-operating-characteristic [AUROC]-AT: M-DASI-5Q=0.67 vs original 12-question DASI=0.66) and VO 2 peak (AUROC-VO2 peak: M-DASI-5Q 0.73 vs original 12-question DASI 0.71). Conversely, in a sensitivity analysis we removed one potentially sensitive question related to the ability to have sexual relations, and the ability of the remaining four questions (M-DASI-4Q) to predict an adequate functional threshold remained no worse than the original 12-question DASI model. Adding a dynamic component to the M-DASI-4Q by assessing the chronotropic response to exercise improved its ability to discriminate between those with VO 2 peak>16 ml.kg −1.min −1 and VO 2 peak<16 ml.kg −1.min −1. Conclusions The M-DASI provides a simple screening tool for further preoperative evaluation, including with cardiopulmonary exercise testing, to guide perioperative management

    Integration of the Duke Activity Status Index into preoperative risk evaluation: a multicentre prospective cohort study.

    BACKGROUND: The Duke Activity Status Index (DASI) questionnaire might help incorporate self-reported functional capacity into preoperative risk assessment. Nonetheless, prognostically important thresholds in DASI scores remain unclear. We conducted a nested cohort analysis of the Measurement of Exercise Tolerance before Surgery (METS) study to characterise the association of preoperative DASI scores with postoperative death or complications. METHODS: The analysis included 1546 participants (≥40 yr of age) at an elevated cardiac risk who had inpatient noncardiac surgery. The primary outcome was 30-day death or myocardial injury. The secondary outcomes were 30-day death or myocardial infarction, in-hospital moderate-to-severe complications, and 1 yr death or new disability. Multivariable logistic regression modelling was used to characterise the adjusted association of preoperative DASI scores with outcomes. RESULTS: The DASI score had non-linear associations with outcomes. Self-reported functional capacity better than a DASI score of 34 was associated with reduced odds of 30-day death or myocardial injury (odds ratio: 0.97 per 1 point increase above 34; 95% confidence interval [CI]: 0.96-0.99) and 1 yr death or new disability (odds ratio: 0.96 per 1 point increase above 34; 95% CI: 0.92-0.99). Self-reported functional capacity worse than a DASI score of 34 was associated with increased odds of 30-day death or myocardial infarction (odds ratio: 1.05 per 1 point decrease below 34; 95% CI: 1.00-1.09), and moderate-to-severe complications (odds ratio: 1.03 per 1 point decrease below 34; 95% CI: 1.01-1.05). CONCLUSIONS: A DASI score of 34 represents a threshold for identifying patients at risk for myocardial injury, myocardial infarction, moderate-to-severe complications, and new disability

    Positive effects of an invasive shrub on aggregation and abundance of a native small rodent

    Invasive plants can have dramatic effects on natural ecosystems. It is unclear, though, whether these will have a positive or negative effect on animal species' behavior and population parameters within ecosystems where invasive plants occur. Here, we use a 2-year time series of mouse trapping data to test the effects of an evergreen invasive shrub, Rhododendron ponticum, on population distribution and abundance in a population of wood mice (Apodemus sylvaticus) in southern England. Given the importance of aerial predators on rodent survival and the shield that the thick cover of Rhododendron branches and leaves provides, we predicted that Rhododendron would have a positive effect on mouse aggregation and abundance. The results confirmed both predictions: proximity to Rhododendron positively influenced mouse abundance, whereas a significant interaction between protective microhabitat features (logs) and Rhododendron suggest that reductions in predation risk drive the proximity results. In addition, as mouse densities increased, competition increased. During spring, when mouse territoriality was greatest, we found primarily large adults in the Rhododendron habitat, with subadult and juvenile mice more likely to be found away from Rhododendron patches. The effects of Rhododendron-driven increases in mouse density on lower (seed predation and dispersal) and upper trophic level (weasel populations) are also discussed. Questing tick's density and invertebrate biomass were also lower under Rhododendron. Our research shows that an invasive plant species can increase the abundance of a native mammal and that this could potentially lead to increases/decreases in other species populations within the community. © 2012 The Author

    Molecular epidemiology and phylogeny reveal complex spatial dynamics in areas where canine parvovirus is endemic

    Canine parvovirus type 2 (CPV-2) is a severe enteric pathogen of dogs, causing high mortality in unvaccinated dogs. After emerging, CPV-2 spread rapidly worldwide. However, there is now some evidence to suggest that international transmission appears to be more restricted. In order to investigate the transmission and evolution of CPV-2 both nationally and in relation to the global situation, we have used a long-range PCR to amplify and sequence the full VP2 gene of 150 canine parvoviruses obtained from a large cross-sectional sample of dogs presenting with severe diarrhea to veterinarians in the United Kingdom, over a 2-year period. Among these 150 strains, 50 different DNA sequence types (S) were identified, and apart from one case, all appeared unique to the United Kingdom. Phylogenetic analysis provided clear evidence for spatial clustering at the international level and for the first time also at the national level, with the geographical range of some sequence types appearing to be highly restricted within the United Kingdom. Evolution of the VP2 gene in this data set was associated with a lack of positive selection. In addition, the majority of predicted amino acid sequences were identical to those found elsewhere in the world, suggesting that CPV VP2 has evolved a highly fit conformation. Based on typing systems using key amino acid mutations, 43 of viruses were CPV-2a, and 57 CPV-2b, with no type 2 or 2c found. However, phylogenetic analysis suggested complex antigenic evolution of this virus, with both type 2a and 2b viruses appearing polyphyletic. As such, typing based on specific amino acid mutations may not reflect the true epidemiology of this virus. The geographical restriction that we observed both within the United Kingdom and between the United Kingdom and other countries, together with the lack of CPV-2c in this population, strongly suggests the spread of CPV within its population may be heterogeneously subject to limiting factors. This cross-sectional study of national and global CPV phylogeographic segregation reveals a substantially more complex epidemic structure than previously described. © 2011, American Society for Microbiology
