    The Prevalence of Skin Diseases and Its Association with Hygiene Behavior and Level of Education in a Pesantren, Jakarta Selatan 2013

    Skin diseases are very common in places where the society lives in a crowded area. Pesantren is anIslamic school with a dormitory for its students, thus making the spread of skin infection easier to occur. Theobjective of this research was to identify the association between the prevalence of skin diseases with the hygienebehavior and level of education of santris (students of pesantren). This cross-sectional study was conducted in apesantren in South Jakarta. Data collection was carried out through a questionnaire consisting of ten questionsabout hygienic behaviors and history of previous dermatological examinations from July until September 2013.Results showed that out of 98 santris, 88 of them had skin diseases (89.7%). The most frequent skin infectionwas scabies with 67 cases (49.3%). Furthermore, 78 (88.6%) out of all santris who had skin diseases, werecategorized to have poor hygienic behaviors. There were only 10 santris that did not have any skin disease, 3of them had good hygienic behaviors. There was no significant difference between hygienic behaviors of santrisand the prevalence of skin disease (p=0.350). Associated with the level of education, ibtidaiyah had the highestnumber of santris (51.2%) affected by skin diseases. There was a significant difference between the level ofeducation and the prevalence of skin diseases (p<0.001). In coclusion, the prevalence of skin diseases in thepesantren was 89.7%; there was no association between skin diseases and hygienic behaviors. However, therewas an association between skin diseases and level of education

    Preliminary lessons from COVID-19 disruptions of small-scale fishery supply chains

    The ongoing COVID-19 pandemic and associated mitigation measures have disrupted global systems that support the health, food and nutrition security, and livelihoods of billions of people. These disruptions have likewise affected the small-scale fishery (SSF) sector, disrupting SSF supply chains and exposing weaknesses in the global seafood distribution system. To inform future development of adaptive capacity and resilience in the sector, it is important to understand how supply chain actors are responding in the face of a macroeconomic shock. Comparing across seven SSF case studies in four countries, we explore how actors are responding to COVID-19 disruptions, identify constraints to adaptive responses, and describe patterns of disruption and response across cases. In all cases examined, actors shifted focus to local and regional distribution channels and particularly drew on flexibility, organization, and learning to re-purpose pre-existing networks and use technology to their advantage. Key constraints to reaching domestic consumers included domestic restrictions on movement and labor, reduced spending power amongst domestic consumers, and lack of existing distribution channels. In addition, the lack of recognition of SSFs as essential food-producers and inequities in access to technology hampered efforts to continue local seafood supply. We suggest that the initial impacts from COVID-19 highlight the risks in of over-reliance on global trade networks. The SSFs that were able to change strategies most successfully had local organizations and connections in place that they leveraged in innovative ways. As such, supporting local and domestic networks and flexible organizations within the supply chain may help build resilience in the face of future macroeconomic shocks. Importantly, bolstering financial wellbeing and security within the domestic market both before and during such large-scale disruptions is crucial for supporting ongoing supply chain operations and continued food provision during macroeconomic crises

    A Real-Coded Hybrid Genetic Algorithm to Determine Optimal Resin Injection Locations in the Resin Transfer Molding Process

    Real number-coded hybrid genetic algorithms for optimal design of resin injection locations for the resin transfer molding process are evaluated in this paper. Resin transfer molding (RTM) is widely used to manufacture composite parts with material and geometric complexities, especially in automotive and aerospace sectors. The sub-optimal location of the resin injection locations (gates) can leads to the formation of resin starved regions and require long mold fill times, thus affecting the part quality and increasing manufacturing costs. There is a need for automated design algorithms and software that can determine the best gate and vent locations for a composite part by using the current simulation capabilities. In the work presented here, the gates are encoded into real number strings for the GA. A sensitivity gradient-based fill time optimization algorithm was developed using the process physics, which can be used as a local optimization algorithm. The global search capabilities of the GA and the local search capabilities of the sensitivity gradient-based fill time optimization algorithm were combined in two separate hybrid optimization algorithms: a serial hybrid optimization algorithm and an interactive optimization algorithm. In addition, the sensitivity gradient-based algorithm involves the computation of the gradient of the fill time with respect to the gate location coordinates. This gradient information was included in the criteria for optimization to increase the capabilities of the hybrid GA. Several RTM molds with geometric and material complexities were selected and discretized. A number of studies were performed using the pure genetic algorithm, the gradient-based optimization algorithm and the two hybrid optimization algorithms using the mold fill time and it's gradient with respect to gate location coordinates as the cost criteria. These studies were performed for the cases of a single gate and two gates. The results were benchmarked against known best solutions in terms of quality of final solutions and the computational effort required

    Efficacy and safety of subcutaneous trastuzumab and intravenous trastuzumab as part of adjuvant therapy for HER2-positive early breast cancer: final analysis of the randomised, two-cohort PrefHer study

    Aim To assess efficacy (event-free survival, EFS) and safety in patients followed up for 3 years in the PrefHer study (NCT01401166). Patients and methods Post surgery and post chemotherapy in the (neo)adjuvant setting, patients with human epidermal growth factor receptor 2 (HER2)-positive early breast cancer were randomised to receive four cycles of the subcutaneous form of trastuzumab (Herceptin ® SC [H SC] via single-use injection device [Cohort 1] or delivery via a hand-held syringe from an SC Vial [Cohort 2]; 600 mg fixed dose) followed by four of the intravenous form of trastuzumab (Herceptin ® [H IV]; 8 mg/kg loading, 6 mg/kg maintenance doses) in the adjuvant setting or vice versa every 3 weeks. Patients could have received H before randomisation. H was then continued to complete a total of 18 cycles, including any cycles received before randomisation. Results A total of 488 patients were randomised across both cohorts. After median follow-up of 36.1 months, 3-year EFS across both groups in the evaluable intention-to-treat population (467 patients) was 90.6% overall, 89.9% in Cohort 1, and 91.1% in Cohort 2. No new safety signals were identified during long-term follow-up, with only one cardiac serious adverse event in the safety population (483 patients). Conclusions Three-year EFS data following H SC and H IV treatment are consistent with those reported by previous trials for H in the adjuvant setting. The overall safety profile during adjuvant treatment was as expected
