    Intervention Now to Eliminate Repeat Unintended Pregnancy in Teenagers (INTERUPT): a systematic review of intervention effectiveness and cost-effectiveness, and qualitative and realist synthesis of implementation factors and user engagement.

    BACKGROUND: Unintended repeat conceptions can result in emotional, psychological and educational harm to young women, often with enduring implications for their life chances. This study aimed to identify which young women are at the greatest risk of repeat unintended pregnancies; which interventions are effective and cost-effective; and what are the barriers to and facilitators for the uptake of these interventions. METHODS: We conducted a mixed-methods systematic review which included meta-analysis, framework synthesis and application of realist principles, with stakeholder input and service user feedback to address this. We searched 20 electronic databases, including MEDLINE, Excerpta Medica database, Applied Social Sciences Index and Abstracts and Research Papers in Economics, to cover a broad range of health, social science, health economics and grey literature sources. Searches were conducted between May 2013 and June 2014 and updated in August 2015. RESULTS: Twelve randomised controlled trials (RCTs), two quasi-RCTs, 10 qualitative studies and 53 other quantitative studies were identified. The RCTs evaluated psychosocial interventions and an emergency contraception programme. The primary outcome was repeat conception rate: the event rate was 132 of 308 (43%) in the intervention group versus 140 of 289 (48%) for the control group, with a non-significant risk ratio (RR) of 0.92 [95% confidence interval (CI) 0.78-1.08]. Four studies reported subsequent birth rates: 29 of 237 (12%) events for the intervention arm versus 46 out of 224 (21%) for the control arm, with an RR of 0.60 (95% CI 0.39-0.93). Many repeat conceptions occurred in the context of poverty, low expectations and aspirations and negligible opportunities. Qualitative and realist evidence highlighted the importance of context, motivation, future planning and giving young women a central and active role in the development of new interventions. CONCLUSIONS: Little or no evidence for the effectiveness or cost-effectiveness of any of the interventions to reduce repeat pregnancy in young women was found. Qualitative and realist evidence helped to explain gaps in intervention design that should be addressed. More theory-based, rigorously evaluated programmes need to be developed to reduce unintended repeat pregnancy in young women. TRIAL REGISTRATION: PROSPERO, CRD42012003168 .     A final course, or project is a standard requirement in Master of Public Administration (MPA) programs. This requirement is intended to permit the student to demonstrate advanced ability in the discipline. This article examines one such course at Florida State University. The course included readings, exercises, oral presentations, and a written report. This author assesses the merits and drawbacks of the course as well as notes the unintended consequences that resulted. In concluding the article, the author makes several recommendations for future courses based on the experiences gained in the Florida state model. Copyright 1989 by The Policy Studies Organization.

    Scale and scope in technology: large firms 1930/1990

    We examine historical empirical patterns of change in corporate. technological scale and scope. Much literature on scale and scope by business and economic historians has conflated product markets and technology together. However, given the technologically complex environment of the late twentieth century, the relationship between scale and scope in production and products is not simple, and conflated discussions may be naive. Consequently we have two aims. Firstly, we wish to see if technological scale and scope follow the historical pattern of broader notions of scale and scope. Secondly, given increasing technological complexity, we consider whether the nature of corporate technological scope has changed over time. Using data drawn from the University of Reading's patents database, we examine the technological activities in 32 of the world's historically largest patenting firms over the period 1930 to 1990. Shin-sham analyses based on panel-data regressions examine the different influences on technologicascope over time. A technological trend closely resembling that of broader notions of scale to scope is observable in most of the sectors. although it does not proceed uniformly. We also find, that whilst increases in corporate technological scope through diversification are not particularly linked to technological relatedness in recent times, they are periodically influenced by the rise of pervasive, fast-growing new technologies

    Technology diversification, coherence and performance of firms

    Technological diversification at the firm level (i.e., the expansion of a firm's technology base into a wide range of technology fields) is found to be a prevailing phenomenon in all three major industrialized regions,—the United States, Europe, and Japan—prompting the term multitechnology corporation. Whereas previous studies have provided insights into the composition of technology portfolios of multitechnology firms, little is known about the relationship between technological diversification and firms' technological performance. Against a backdrop of the technology and innovation management literature, the present article investigates the relationship between technological diversification and technological performance, taking into account the moderating role of technological coherence in firms' technology portfolios. Hereby, technological coherence is defined as the degree to which technologies in a technology portfolio are technologically related. To measure the technological coherence of portfolios, a measure of technological relatedness of technology fields is constructed based on patent citation patterns found in 450,000 European Patent Office (EPO) patent grants. Two hypotheses are presented here: (1) Technological diversification has an inverted U-shaped relationship with technological performance; and (2) technological coherence moderates the relationship between technological diversification and technological performance positively. These hypotheses are tested empirically using a panel data set (1995–2003) on patent portfolios pertaining to 184 U.S., European, and Japanese firms. The firms selected are the largest research and development (R&D) actors in five industries: pharmaceuticals and biotechnology; chemicals; engineering and general machinery; information technology (IT) hardware (i.e., computers and communication equipment); and electronics and electrical machinery. Empirical results, obtained by fixed-effects negative binomial regressions, support both hypotheses in the present article. Technological diversification has an inverted U-shaped relationship with technological performance. Technological diversification offers opportunities for cross-fertilization and technology fusion, but high levels of diversification may yield few marginal benefits as firms risk lacking sufficient levels of scale to benefit from wide-ranging technological diversification, and firms may encounter high levels of coordination and integration costs. Further, the results show that the net benefits of technological diversification are higher in technologically coherent technology portfolios. If firms build up a technologically coherent diversified portfolio, the presence of sufficient levels of scale is ensured and coordination costs are limited. At the same time, technologically coherent diversification puts firms in a better position to benefit form cross-fertilization between technologies.
