    Physical Activity during Full-Day and Half-Day Kindergarten

    The aim of this study was to compare the physical activity levels of children during fullday and half-day kindergarten. Of the 47 children who participated in this study, 22 (girls = 50%) attended full-day kindergarten and 25 (girls = 40%) attended half-days. Actigraph activity monitors were used to assess physical activity and sedentary behavior of the children. We found that children were more active during full-day kindergarten. The rates of light-intensity physical activity were significantly higher during full-day kindergarten. However, levels of moderate-to-vigorous physical activity (MVPA) were quite low, and efforts to promote MVPA would be beneficial

    Direct and indirect measurement of physical activity in older adults: a systematic review of the literature

    <p>Abstract</p> <p>Background</p> <p>Due to physiological and cognitive changes that occur with aging, accurate physical activity (PA) measurement in older adults represents a unique challenge. The primary purpose of this study was to systematically review measures of PA and their use and appropriateness with older adults. A secondary aim was to determine the level of agreement between PA measures in older adults.</p> <p>Methods</p> <p>Literature was identified through electronic databases. Studies were eligible if they examined the correlation and/or agreement between at least 2 measures, either indirect and/or direct, of PA in older adults (> 65 years of age).</p> <p>Results</p> <p>Thirty-six studies met eligibility criteria. The indirect and direct measures of PA across the studies differed widely in their ability to address the key dimensions (i.e., frequency, intensity, time, type) of PA in older adults. The average correlation between indirect and direct measures was moderate (r=0.38). The correlation between indirect and other indirect measures (r=0.29) was weak, while correlations between direct measures with other direct measures were high (real world: r= 0.84; controlled settings: r=0.92). Agreement was strongest between direct PA measures with other direct measures in both real world and laboratory settings. While a clear trend regarding the agreement for mean differences between other PA measures (i.e., direct with indirect, indirect with indirect) did not emerge, there were only a limited number of studies that reported comparable units.</p> <p>Conclusions</p> <p>Despite the lack of a clear trend regarding the agreement between PA measures in older adults, the findings underscore the importance of valid, accurate and reliable measurement. To advance this field, researchers will need to approach the assessment of PA in older adults in a more standardized way (i.e., consistent reporting of results, consensus over cut-points and epoch lengths, using appropriate validation tools). Until then researchers should be cautious when choosing measures for PA that are appropriate for their research questions and when comparing PA levels across various studies.</p

    Dystrophic calcification and heterotopic ossification in fibrocartilaginous tissues of the spine in diffuse idiopathic skeletal hyperostosis (DISH)

Diffuse idiopathic skeletal hyperostosis (DISH) is a prevalent noninflammatory spondyloarthropathy characterized by ectopic mineral formation along the anterolateral aspect of the vertebral column, yet little is known about its underlying pathogenesis. Our objective was to evaluate the histopathological features and composition of ectopic mineral within spinal tissues affected by DISH in humans. Thoracic spine segments from six embalmed cadaveric donors (one female and five males; median age 82 years) meeting the radiographic diagnostic criteria for DISH were evaluated using radiological, histological, and physical analyses. Overall, the histological features of ectopic mineralization at individual motion segments were heterogeneous, including regions of heterotopic ossification and dystrophic calcification. Heterotopic ossifications were characterized by woven and lamellar bone, multifocal areas of metaplastic cartilage, and bony bridges along the anterior aspect of the intervertebral disc space. Dystrophic calcifications were characterized by an amorphous appearance, a high content of calcium and phosphorus, an X-ray diffraction pattern matching that of hydroxyapatite, and radiodensities exceeding that of cortical bone. Dystrophic calcifications were found within the anterior longitudinal ligament and annulus fibrosus in motion segments both meeting and not meeting the radiographic criteria for DISH. In summary, our findings indicate that in DISH, ectopic mineral forms along the anterior aspect of the spine by both heterotopic ossification and dystrophic calcification of fibrocartilaginous tissues. Although both types of ectopic mineralization are captured by current radiographic criteria for DISH, dystrophic calcification may reflect a distinct disease process or an early stage in the pathogenesis of DISH

    Deep Learning Body Region Classification of MRI and CT examinations

    Standardized body region labelling of individual images provides data that can improve human and computer use of medical images. A CNN-based classifier was developed to identify body regions in CT and MRI. 17 CT (18 MRI) body regions covering the entire human body were defined for the classification task. Three retrospective databases were built for the AI model training, validation, and testing, with a balanced distribution of studies per body region. The test databases originated from a different healthcare network. Accuracy, recall and precision of the classifier was evaluated for patient age, patient gender, institution, scanner manufacturer, contrast, slice thickness, MRI sequence, and CT kernel. The data included a retrospective cohort of 2,934 anonymized CT cases (training: 1,804 studies, validation: 602 studies, test: 528 studies) and 3,185 anonymized MRI cases (training: 1,911 studies, validation: 636 studies, test: 638 studies). 27 institutions from primary care hospitals, community hospitals and imaging centers contributed to the test datasets. The data included cases of all genders in equal proportions and subjects aged from a few months old to +90 years old. An image-level prediction accuracy of 91.9% (90.2 - 92.1) for CT, and 94.2% (92.0 - 95.6) for MRI was achieved. The classification results were robust across all body regions and confounding factors. Due to limited data, performance results for subjects under 10 years-old could not be reliably evaluated. We show that deep learning models can classify CT and MRI images by body region including lower and upper extremities with high accuracy.Comment: 21 pages, 2 figures, 4 table

    Exploring the Relationship between Diet and TV, Computer and Video Game Use in a Group of Canadian Children

    Increased screen-time has been linked to unhealthy dietary practices but most studies have looked primarily at television viewing or an amalgam. Therefore the purpose of this study was to investigate the relationship between specific screen-time (TV, computer, video game) and a selection of healthy dietary intake measures (calories, carbohydrate, fat, sugar, fruit, vegetables, fibre and sugar-sweetened beverages (SSB)) in a group of Canadian children. We used single day sedentary and dietary recalls to assess sedentary behaviour and diet in 1423 children (9.90 (0.58) y; 737 girls, 686 boys) from the Action Schools! BC Dissemination study. Correlations and multiple regression analyses were used to explore sedentary behaviour-diet relationships. TV and video game use were correlated with higher calories, fat, sugar and SSB consumption (r = 0.07 to 0.09; p &lt;.01) and lower fibre intake (r = -0.05 to -0.06; p &lt;.05). TV use was also correlated with lower fruit and vegetable intake. Regression analyses showed that when controlling for other variables, only TV and video game use predicted sugar and SSB consumption (β =.06 to.08; p &lt;.05). Computer use was correlated with calories but did not significantly predict any of the measures of dietary intake. Although screen time was significantly associated with less healthy eating profiles, it did not account for much variance in dietary behaviour of these children

    Metabolic and Transcriptional Modules Independently Diversify Plasma Cell Lifespan and Function

    Plasma cell survival and the consequent duration of immunity vary widely with infection or vaccination. Using fluorescent glucose analog uptake, we defined multiple developmentally independent mouse plasma cell populations with varying life- spans. Long-lived plasma cells imported more fluo- rescent glucose analog, expressed higher surface levels of the amino acid transporter CD98, and had more autophagosome mass than did short-lived cells. Low amino acid concentrations triggered re- ductions in both antibody secretion and mitochon- drial respiration, especially by short-lived plasma cells. To explain these observations, we found that glutamine was used for both mitochondrial respira- tion and anaplerotic reactions, yielding glutamate and aspartate for antibody synthesis. Endoplasmic reticulum (ER) stress responses, which link meta- bolism to transcriptional outcomes, were similar between long- and short-lived subsets. Accordingly, population and single-cell transcriptional compari- sons across mouse and human plasma cell subsets revealed few consistent and conserved dif- ferences. Thus, plasma cell antibody secretion and lifespan are primarily defined by non-transcriptional metabolic traits

    Biological responses to change in Antarctic sea ice habitats

    Sea ice is a key habitat in the high latitude Southern Ocean and is predicted to change in its extent, thickness and duration in coming decades. The sea-ice cover is instrumental in mediating ocean–atmosphere exchanges and provides an important substrate for organisms from microbes and algae to predators. Antarctic krill, Euphausia superba, is reliant on sea ice during key phases of its life cycle, particularly during the larval stages, for food and refuge from their predators, while other small grazers, including copepods and amphipods, either live in the brine channel system or find food and shelter at the ice-water interface and in gaps between rafted ice blocks. Fish, such as the Antarctic silverfish Pleuragramma antarcticum, use platelet ice (loosely-formed frazil crystals) as an essential hatching and nursery ground. In this paper, we apply the framework of the Marine Ecosystem Assessment for the Southern Ocean (MEASO) to review current knowledge about relationships between sea ice and associated primary production and secondary consumers, their status and the drivers of sea-ice change in this ocean. We then use qualitative network modelling to explore possible responses of lower trophic level sea-ice biota to different perturbations, including warming air and ocean temperatures, increased storminess and reduced annual sea-ice duration. This modelling shows that pelagic algae, copepods, krill and fish are likely to decrease in response to warming temperatures and reduced sea-ice duration, while salp populations will likely increase under conditions of reduced sea-ice duration and increased number of days of &gt;0°C. Differences in responses to these pressures between the five MEASO sectors were also explored. Greater impacts of environmental pressures on ice-related biota occurring presently were found for the West and East Pacific sectors (notably the Ross Sea and western Antarctic Peninsula), with likely flow-on effects to the wider ecosystem. All sectors are expected to be impacted over coming decades. Finally, we highlight priorities for future sea ice biological research to address knowledge gaps in this field

    Unpacking vertical and horizontal integration: childhood overweight/obesity programs and planning, a Canadian perspective

    Abstract Background Increasingly, multiple intervention programming is being understood and implemented as a key approach to developing public health initiatives and strategies. Using socio-ecological and population health perspectives, multiple intervention programming approaches are aimed at providing coordinated and strategic comprehensive programs operating over system levels and across sectors, allowing practitioners and decision makers to take advantage of synergistic effects. These approaches also require vertical and horizontal (v/h) integration of policy and practice in order to be maximally effective. Discussion This paper examines v/h integration of interventions for childhood overweight/obesity prevention and reduction from a Canadian perspective. It describes the implications of v/h integration for childhood overweight and obesity prevention, with examples of interventions where v/h integration has been implemented. An application of a conceptual framework for structuring v/h integration of an overweight/obesity prevention initiative is presented. The paper concludes with a discussion of the implications of vertical/horizontal integration for policy, research, and practice related to childhood overweight and obesity prevention multiple intervention programs. Summary Both v/h integration across sectors and over system levels are needed to fully support multiple intervention programs of the complexity and scope required by obesity issues. V/h integration requires attention to system structures and processes. A conceptual framework is needed to support policy alignment, multi-level evaluation, and ongoing coordination of people at the front lines of practice. Using such tools to achieve integration may enhance sustainability, increase effectiveness of prevention and reduction efforts, decrease stigmatization, and lead to new ways to relate the environment to people and people to the environment for better health for children
