    Building public health preparedness and food and agriculture defense capabilities using whole community and one health concepts

    Emergency managers are embracing the Whole Community approach described in Federal Emergency Management Agency (FEMA) preparedness policy, doctrine, and guidance. The Whole Community approach entails broad collaboration and integration of effort among multiple disciplines and preparedness partners to coordinate solutions for all threats and hazards. Potential public health emergencies – to include foodborne and animal disease outbreaks – are issues on the emergency management agenda requiring such broad coordination. Scientists and public health practitioners across multiple disciplines describe a philosophy very similar to “Whole Community” known as “One Health,” linking efforts to address the shared threats to human, animal and environmental health. This paper recommends a coordinated strategy for FEMA to link “Whole Community” and “One Health” tenets as part of a national preparedness effort

    How does climate adaptation affect emergency management?

    Recent reports from credible research groups suggest that climate change is a reality. The steady rise in extreme weather events over the past decade represents the leading edge of climate change. Climate-induced interactions within and between the natural environment and our human and constructed systems will not only exacerbate existing vulnerabilities but will create new ones. Emergency managers at all levels need to plan and prepare for climate adaptation challenges, which should enable communities to successfully weather the storm. The ability to adapt to the coming changes is dependent upon the actions we take today to create resilience and sustainability. Leaving aside the issue of causation, this paper discusses the implications of climate change for emergency mangers by examining predicted impacts, exploring barriers to action, and concluding with strategies for moving forward

    Presynchronizing PGF2α and GnRH injections before timed artificial insemination CO-Synch + CIDR program

    Fixed-time artificial insemination is an effective management tool that reduces the labor associated with more conventional artificial insemination programs requiring detection of estrus. The 7-day CO-Synch + controlled internal drug release (CIDR) insert protocol has been shown to effectively initiate estrus and ovulation in cycling and non-cycling suckled beef cows, producing pregnancy rates at or greater than 50% in beef cows. The gonadotropin-releasing hormone (GnRH) injection that begins the CO-Synch + CIDR program initiates ovulation in a large proportion of cows, particularly anestrous cows. The CIDR, which releases progesterone intravaginally, prevents short estrous cycles that usually follow the first postpartum ovulation in beef cows. Our hypothesis was that inducing estrus with a prostaglandin injection followed 3 days later with a GnRH injection, 7 days before applying the 7-day CO-Synch + CIDR protocol, might increase the percentage of cycling cows that would exhibit synchronous follicular waves after the onset of the CO-Synch + CIDR protocol. We also hypothesized that the additional GnRH injection would increase the percentage of anestrous cows that would ovulate, thereby increasing pregnancy outcomes

    A Sea Turtle Population Assessment for Florida\u27s Big Bend, Northeastern Gulf of Mexico

    Coastal waters of Florida’s Big Bend, Gulf of Mexico (GOM) once supported one of the largest sea turtle fisheries in the United States. To fill an information gap in this region on abundance and distribution of sea turtles, we used vessel—based distance sampling and active capture methods to characterize current foraging aggregations near the St. Martins Marsh Aquatic Preserve. Over 10 sampling periods between 2012—2018, we completed 513 km of transects and recorded 819 turtles among 4 species—green turtle (Chelonia mydas, n = 624), Kemp’s ridley (Lepidochelys kempii, n = 147), loggerhead (Caretta caretta, n = 47), and a single hawksbill (Eretmochelys imbricata). Turtle densities in 4 study plots within the 200 km2 study site ranged from 57—221 immature green turtles/km2, 16—56 immature Kemp’s ridleys/km2, and 1—14 juvenile—to—adult loggerheads/km2. Of 200 green turtles captured, 67.5% showed skin tumors consistent with fibropapillomatosis, a frequency similar to that from urbanized estuaries of Florida’s east coast. The largest green turtles (\u3e 60 cm straight standard carapace length), abundant in the southern portion of our study area, are of note because this size class is uncommonly recorded within US territorial waters. Analyses of green turtle mtDNA haplotypes found contributions from rookeries in the western GOM, Mexican Caribbean, and Costa Rica. Although Big Bend protected areas were principally designed to conserve marine and coastal habitats, these regulatory zones have also effectively encompassed a hotspot for foraging sea turtles

    Potentiation of endogenous nitric oxide with superoxide dismutase inhibits platelet-mediated thrombosis in injured and stenotic arteries

    AbstractObjectives. We tested the hypothesis that dismutation of superoxide anion increases endogenous levels of nitric oxide, resulting in inhibition of cyclic variations in blood flow in arteries that are injured and stenotic.Background. Platelet adhesion and aggregation leading to cyclic flow variations might result, in part, from generation of superoxide anion that can deplete endogenously produced nitric oxide.Methods. Spontaneous cyclic flow variations, monitored with a proximal Doppler probe, were induced in the carotid artery of anesthetized rabbits by clamping the vessel with forceps and placing a high grade stenosis at the site of injury. Bovine copper/zinc superoxide dismutase (12 mg/kg body weight, n = 5), a synthetic low molecular weight mimetic (12 mg/kg, n = 8) or buffer vehicle (n = 8) was administered intravenously as divided boluses over 45 min, and the frequency of cyclic flow variations was monitored for 4 h.Results. Cyclic flow variations remained stable for 4 h in vehicle-treated animals (15 ± 1 [mean ± SEM]/30 min at baseline and 16 ± 1/30 min after 4 h, n = 8) but exhibited a marked and persistent reduction in animals given copper/zinc superoxide dismutase (from 14 ± 1/30 min at baseline to 4 ± 1/30 min after 4 h) or the mimetic (from 15 ± 1/30 min at baseline to 3 ± 1/30 min after 4 h, p < 0.005). They were restored in three of four mimetic-treated animals during infusion of NG-monomethyl-l-arginine (100 mg/kg), an inhibitor of nitric oxide production. In addition, levels of cyclic guanosine 5â€Č-monophosphate in platelets were elevated after administration of the mimetic (from 2.4 ± 0.5 fmol/106platelets at baseline to 4.9 ± 0.6 fmol/106platelets 45 min after the mimetic, p < 0.03, n = 6), whereas mean arterial blood pressure was decreased and flow velocity in the carotid artery was increased consistent with mediation of the effect on cyclic flow variations by increased endogenous nitric oxide.Conclusions. Dismutation of superoxide anion appears to attenuate platelet thrombus formation at a site of vessel injury by potentiation of endogenously produced nitric oxide. This approach may have utility to inhibit platelet-rich thrombosis in injured and stenotic arteries where production of superoxide anion is increased

    Association Between Empathy and Burnout Among Emergency Medicine Physicians

    Background: The association between physician self-reported empathy and burnout has been studied in the past with diverse findings. We aimed to determine the association between empathy and burnout among United States emergency medicine (EM) physicians using a novel combination of tools for validation. Methods: This was a prospective single-center observational study. Data were collected from EM physicians. From December 1, 2018 to January 31, 2019, we used the Jefferson scale of empathy (JSE) to assess physician empathy and the Copenhagen burnout inventory (CBI) to assess burnout. We divided EM physicians into different groups (residents in each year of training, junior/senior attendings). Empathy, burnout scores and their association were analyzed and compared among these groups. Results: A total of 33 attending physicians and 35 EM residents participated in this study. Median self-reported empathy scores were 113 (interquartile range (IQR): 105 - 117) in post-graduate year (PGY)-1, 112 (90 - 115) in PGY-2, 106 (93 - 118) in PGY-3 EM residents, 112 (105 - 116) in junior and 114 (101 - 125) in senior attending physicians. Overall burnout scores were 43 (33 - 50) in PGY-1, 51 (29 - 56) in PGY-2, 43 (42 - 53) in PGY-3 EM residents, 33 (24 - 47) in junior attending and 25 (22 - 53) in senior attending physicians separately. The Spearman correlation (ρ) was -0.11 and ÎČ-weight was -0.23 between empathy and patient-related burnout scores. Conclusion: Self-reported empathy declines over the course of EM residency training and improves after graduation. Overall high burnout occurs among EM residents and improves after graduation. Our analysis showed a weak negative correlation between self-reported empathy and patient-related burnout among EM physicians

    Effects of a supplement designed to increase ATP levels on muscle strength, power output, and endurance

    <p>Abstract</p> <p>Background</p> <p>The present study examined the acute effects of a nutritional supplement intended to improve adenosine triphosphate (ATP) concentrations on vertical jump height, isometric strength of the leg extensors, leg extension endurance, and forearm flexion endurance.</p> <p>Methods</p> <p>Twenty-four healthy men (mean age ± SD = 23 ± 4 yrs, stature = 181 ± 7 cm, and body mass = 82 ± 12 kg) volunteered to complete a familiarization trial plus 2 randomly-ordered experimental trials separated by a 7-day washout period. Participants received either 6 (body mass < 91 kg) or 8 (body mass ≄ 91 kg) tablets of the treatment (TR; 625 mg of adenylpyrophosphoric acid and calcium pyruvate, 350.8 mg of cordyceps sinensis extract and yohimbine hydrochloride) or placebo (PL; 980 mg of microcrystalline cellulose) 1 hour prior to the following tests: countermovement vertical jump (CVJ), forearm flexion repetitions to exhaustion, isometric maximal voluntary contractions (MVCs) of the leg extensors, and a 50-repetition maximal concentric isokinetic leg extension endurance test.</p> <p>Results</p> <p>There were no differences between the TR and PL trials for CVJ height (<it>P </it>> 0.05), isometric MVC peak torque (<it>P </it>> 0.05), maximal concentric isokinetic peak torque (<it>P </it>> 0.05), percent decline during the leg extension endurance tests (<it>P </it>> 0.05), or repetitions to exhaustion during the forearm flexion endurance tests (<it>P </it>> 0.05).</p> <p>Conclusion</p> <p>These findings indicated no improvements in the measured variables as a result of ingesting this nutritional supplement. Future studies should examine whether chronic supplementation or a loading period is necessary to observe any ergogenic effects of this supplement.</p
