    Cognitive performance along the migraine cycle: a negative exploratory study

    Migraine patients frequently report cognitive difficulties in the proximity and during migraine attacks. We performed an exploratory comparison of executive functioning across the four stages of the migraine cycle. Consecutive patients with episodic migraine undertook cognitive tests for attention, processing speed, set-shifting, and inhibitory control. Performance was compared between patients in different migraine stages, controlling for attack frequency and prophylactic medication. One hundred forty-three patients (142 women, average age 36.2 ± 9.9 years) were included, 28 preictal (≤48 h before the attack), 21 ictal (during the attack), 18 postictal (≤24 h after attack), and 76 interictal. Test performance (age and literacy adjusted z-scores) was not significantly different across migraine phases, despite a tendency for a decline before the attack. This negative study shows that cognitive performance fluctuates as patients approach the attack. To control for individual variability, this comparison needs to be better characterized longitudinally with a within-patient design.info:eu-repo/semantics/publishedVersio

    Structural anatomical investigation of long-term memory deficit in behavioural frontotemporal dementia

    Although a growing body of work has shown that behavioral variant frontotemporal dementia (bvFTD) could present with severe amnesia in approximately half of cases, memory assessment is currently the clinical standard to distinguish bvFTD from Alzheimer's disease (AD). Thus, the concept of "relatively preserved episodic memory" in bvFTD remains the basis of its clinical distinction from AD and a criterion for bvFTD's diagnosis. This view is supported by the idea that bvFTD is not characterized by genuine amnesia and hippocampal degeneration, by contrast to AD. In this multicenter study, we aimed to investigate the neural correlates of memory performance in bvFTD as assessed by the Free and Cued Selective Reminding Test (FCSRT). Imaging explorations followed a two-step procedure, first relying on a visual rating of atrophy of 35 bvFTD and 34ADpatients' MRI, contrasted with 29 controls; and then using voxel-based morphometry (VBM) in a subset of bvFTD patients. Results showed that 43% of bvFTD patients presented with a genuine amnesia. Data-driven analysis on visual rating data showed that, in bvFTD, memory recall & storage performances were significantly predicted by atrophy in rostral prefrontal and hippocampal/perihippocampal regions, similar to mild AD. VBM results in bvFTD (p(FWE)<0.05) showed similar prefrontal and hippocampal regions in addition to striatal and lateral temporal involvement.     Impact of susceptibility-induced distortion correction on perfusion imaging by pCASL with a segmented 3D GRASE readout

    Purpose: The consensus for the clinical implementation of arterial spin labeling (ASL) perfusion imaging recommends a segmented 3D Gradient and Spin-Echo (GRASE) readout for optimal signal-to-noise-ratio (SNR). The correction of the associated susceptibility-induced geometric distortions has been shown to improve diagnostic precision, but its impact on ASL data has not been systematically assessed and it is not consistently part of pre-processing pipelines. Here, we investigate the effects of susceptibility-induced distortion correction on perfusion imaging by pseudo-continuous ASL (pCASL) with a segmented 3D GRASE readout. Methods: Data acquired from 28 women using pCASL with 3D GRASE at 3T was analyzed using three pre-processing options: without distortion correction, with distortion correction, and with spatial smoothing (without distortion correction) matched to control for blurring effects induced by distortion correction. Maps of temporal SNR (tSNR) and relative perfusion were analyzed in eight regions-of-interest (ROIs) across the brain. Results: Distortion correction significantly affected tSNR and relative perfusion across the brain. Increases in tSNR were like those produced by matched spatial smoothing in most ROIs, indicating that they were likely due to blurring effects. However, that was not the case in the frontal and temporal lobes, where we also found increased relative perfusion with distortion correction even compared with matched spatial smoothing. These effects were found in both controls and patients, with no interactions with the participant group. Conclusion: Correction of susceptibility-induced distortions significantly impacts ASL perfusion imaging using a segmented 3D GRASE readout, and this step should therefore be considered in ASL pre-processing pipelines. This is of special importance in clinical studies, reporting perfusion across ROIs defined on relatively undistorted images and when conducting group analyses requiring the alignment of images across different subjects.info:eu-repo/semantics/publishedVersio

    Involvement of the cerebellum in structural connectivity enhancement in episodic migraine

    Background: The pathophysiology of migraine remains poorly understood, yet a growing number of studies have shown structural connectivity disruptions across large-scale brain networks. Although both structural and functional changes have been found in the cerebellum of migraine patients, the cerebellum has barely been assessed in previous structural connectivity studies of migraine. Our objective is to investigate the structural connectivity of the entire brain, including the cerebellum, in individuals diagnosed with episodic migraine without aura during the interictal phase, compared with healthy controls. Methods: To that end, 14 migraine patients and 15 healthy controls were recruited (all female), and diffusion-weighted and T1-weighted MRI data were acquired. The structural connectome was estimated for each participant based on two different whole-brain parcellations, including cortical and subcortical regions as well as the cerebellum. The structural connectivity patterns, as well as global and local graph theory metrics, were compared between patients and controls, for each of the two parcellations, using network-based statistics and a generalized linear model (GLM), respectively. We also compared the number of connectome streamlines within specific white matter tracts using a GLM. Results: We found increased structural connectivity in migraine patients relative to healthy controls with a distinct involvement of cerebellar regions, using both parcellations. Specifically, the node degree of the posterior lobe of the cerebellum was greater in patients than in controls and patients presented a higher number of streamlines within the anterior limb of the internal capsule. Moreover, the connectomes of patients exhibited greater global efficiency and shorter characteristic path length, which correlated with the age onset of migraine. Conclusions: A distinctive pattern of heightened structural connectivity and enhanced global efficiency in migraine patients compared to controls was identified, which distinctively involves the cerebellum.     Impact of truncating diffusion MRI scans on diffusional kurtosis imaging

    Objective Difusional kurtosis imaging (DKI) extends difusion tensor imaging (DTI), characterizing non-Gaussian difusion efects but requires longer acquisition times. To ensure the robustness of DKI parameters, data acquisition ordering should be optimized allowing for scan interruptions or shortening. Three methodologies were used to examine how reduced difusion MRI scans impact DKI histogram-metrics: 1) the electrostatic repulsion model (OptEEM); 2) spherical codes (OptSC); 3) random (RandomTRUNC). Materials and methods Pre-acquired difusion multi-shell data from 14 female healthy volunteers (29±5 years) were used to generate reordered data. For each strategy, subsets containing diferent amounts of the full dataset were generated. The subsampling efects were assessed on histogram-based DKI metrics from tract-based spatial statistics (TBSS) skeletonized maps. To evaluate each subsampling method on simulated data at diferent SNRs and the infuence of subsampling on in vivo data, we used a 3-way and 2-way repeated measures ANOVA, respectively. Results Simulations showed that subsampling had diferent efects depending on DKI parameter, with fractional anisotropy the most stable (up to 5% error) and radial kurtosis the least stable (up to 26% error). RandomTRUNC performed the worst while the others showed comparable results. Furthermore, the impact of subsampling varied across distinct histogram characteristics, the peak value the least afected (OptEEM: up to 5% error; OptSC: up to 7% error) and peak height (OptEEM: up to 8% error; OptSC: up to 11% error) the most afected. Conclusion The impact of truncation depends on specifc histogram-based DKI metrics. The use of a strategy for optimizing the acquisition order is advisable to improve DKI robustness to exam interruptions.info:eu-repo/semantics/publishedVersio

    Biomarcadores acessíveis de demência frontotemporal: um estudo de classificação baseado na conectividade da rede cerebral

    El desarrollo de biomarcadores de demencias efectivos y asequibles resulta esencial dada la dificultad de realizar un diagnóstico temprano de estas patologías. En este sentido, los métodos de electroencefalografía (EEG) brindan alternativas promisorias por su bajo costo, portabilidad y creciente robustez. En este trabajo, nos basamos en señales de EEG y en un novedoso método de intercambio de información para estudiar la conectividad en estado de reposo en pacientes afectados por la variante conductual de la demencia frontotemporal (vcDFT) y en un grupo control. Para evaluar la especificidad de los resultados obtenidos, también examinamos pacientes que padecen la enfermedad de Alzheimer (EA). El poder de clasificación de los patrones de conectividad resultantes se evaluó mediante un algoritmo de clasificación supervisada (máquina de soporte vectorial). Además, comparamos el poder de clasificación obtenido mediante (i) la conectividad funcional, (ii) una batería de pruebas neuropsicológicas pertinentes y (iii) la combinación de ambos. Los pacientes con vcDFT mostraron un patrón de hipoconectividad específico en conexiones frontotemporales de proximidad media, que no mostraron alteraciones en los pacientes con EA. Estas alteraciones en la conectividad funcional en la vcDFT se replicaron mediante EEG de baja densidad (20 electrodos). Por otra parte, mientras que las pruebas neuropsicológicas llevaron a una discriminación aceptable entre los pacientes con vcDFT y los del grupo control, la adición de los resultados de conectividad aumentó el poder de clasificación. Finalmente, la clasificación entre los pacientes con vcDFT y los con EA fue mejor cuando se usó como base la conectividad que cuando se usaron las pruebas neuropsicológicas. Neste trabalho, nos baseamos em sinais de EEG e um novo método de troca de informações para estudar a conectividade em estado de repouso em pacientes afetados pela variante comportamental da demência frontotemporal (vcDFT) e em um grupo controle. Para avaliar a especificidade dos resultados obtidos, também examinamos pacientes que sofrem da Sindrome de Alzheimer (SA). O poder de classificação dos padrões de conectividade resultantes foi avaliado por um algoritmo de classificação supervisionado (máquina de suporte de vetores). Além disso, comparamos o poder de classificação obtido através de (i) conectividade funcional, (ii) uma bateria de testes neuropsicológicos relevantes e (iii) a combinação de ambos. Pacientes com vcDFT mostraram um padrão de hipoconnectividade específica nas conexões frontotemporais de proximidade média, que não mostraram alterações nos pacientes com SA. Essas alterações na conectividade funcional na vcDFT foram replicadas por EEG de baixa densidade (20 eletrodos). Por outro lado, enquanto as provas neuropsicológicas levaram a uma discriminação aceitável entre pacientes com vcDFT e o grupo controle, a adição de resultados de conectividade aumentou o poder de classificação. Finalmente, a classificação entre os pacientes com vcDFT e aqueles com SA foi melhor quando foi usada como base a conectividade que quando se utilizou os testes neuropsicológicos. Attention Deficit/Hyperactivity Disorder (ADHD) is a clinical syndrome characterized by an onset in early life. More than 65% of patients persist with manifestations of ADHD in adulthood.
