    Freqüência de Candida sp. em biópsias de lesões da mucosa bucal

    Candidosis is the most common fungal infection in the oral cavity, and is usually associated with local and systemic predisposing factors. The ocurrence and relevance of Candidal infection in oral lesions such as liquen planus, leukoplakias and carcinomas are still to be understood. The aim of the present study was to define the frequency of infection by Candida sp. on biopsies of oral mucosal lesions and associate its presence with malignant and dysplastic lesions. Histopathology reports issued between 1990 and 2001 inclusive were reviewed. Three sections of each mucosal biopsy were stained using the periodic acid-Schiff (PAS) technique. From the 832 biopsies 27.2% were PAS positive, of which 83.25% were obtained from male patients. There was positive association between fungic infection and mild, moderate and severe epithelial dysplasia, squamous cell carcinoma and hiperqueratosis (p < 0.05). There was no association between fungic infection and inflammatory fibrous hyperplasia, hyperkeratosis, lichen planus and pyogenic granuloma (p < 0.05). The frequency of infection in the tongue was significantly higher (p < 0.05) than in the other sites. Our results do not show a causal relation between Candida sp. and dysplastic lesions and carcinomas, but do confirm the higher presence of that microrganism in those lesions.O objetivo desse trabalho foi determinar a freqüência da infecção por Candida sp. em biópsias de lesões da mucosa bucal, assim como associar a presença de Candida sp. com lesões malignas e lesões com vários graus de displasia. Foram utilizadas 832 biópsias da mucosa bucal, previamente incluídas em parafinas, cujos blocos foram obtidos dos arquivos da Disciplina de Patologia da Faculdade de Odontologia de Araraquara da UNESP, no período entre 1990-2001. Três cortes seqüenciais foram corados pelo ácido periódico de Schiff (PAS). Do total de biópsias 27,2% foram PAS positivas, dessas 83,25% eram provenientes de pacientes do sexo masculino. Houve associação positiva entre infecção com displasia epitelial leve, moderada, severa, carcinoma espinocelular e hiperqueratose (p < 0,05). Não houve associação entre hiperplasia fibrosa inflamatória, líquen plano, granuloma piogênico (p < 0,05) com infecções fúngicas. A língua foi o sítio mais acometido por infecções em relação a outros sítios (p < 0,05). A partir dos dados quantitativos, concluiu-se que houve correlação positiva de infecção por fungos, lesões displásicas e carcinoma, sendo mais freqüente no sexo masculino. Estes dados não permitem inferir se o fungo causa displasia epitelial e carcinoma, mas confirmam a maior presença de Candida nessas lesões

    Salivary IgA antibody responses to Streptococcus mitis and Streptococcus mutans in preterm and fullterm newborn children

    Objectives: The intensities and specificities of salivary IgA antibody responses to antigens of Streptococcus mutans, the main pathogen of dental caries, may influence colonization by these organisms during the first 1.5 year of life. Thus, the ontogeny of salivary IgA responses to oral colonizers continues to warrant investigation, especially with regard to the influence of birth conditions, e.g. prematurity, on the ability of children to efficiently respond to oral microorganisms. In this study, we characterised the salivary antibody responses to two bacterial species which are prototypes of pioneer and pathogenic microorganisms of the oral cavity (Streptococcus mitis and Streptococcus mutans, respectively) in fullterm (FT) and preterm (PT) newborn children. Methods: Salivas from 123 infants (70 FT and 53 PT) were collected during the first 10 h after birth and levels of IgA and IgM antibodies and the presence of S. mutans and S. mitis were analysed respectively by ELISA and by chequerboard DNA-DNA hybridization. Two subgroups of 24 FT and 24 PT children were compared with respect to patterns of antibody specificities against S. mutans and S. mitis antigens, using Western blot assays. Cross-adsorption of 10 infant's saliva was tested to S. mitis, S. mutans and Enterococcus faecalis antigens. Results: Salivary levels of IgA at birth were 2.5-fold higher in FT than in PT children (Mann-Whitney; P &lt; 0.05). Salivary IgA antibodies reactive with several antigens of S. mitis and S. mutans were detected at birth in children with undetectable levels of those bacteria. Adsorption of infant saliva with cells of S. mutans produced a reduction of antibodies recognizing S. mitis antigens in half of the neonates. The diversity and intensity of IgA responses were lower in PT compared to FT children, although those differences were not significant. Conclusion: These data provide evidence that children have salivary IgA antibodies shortly after birth, which might influence the establishment of the oral microbiota, and that the levels of salivary antibody might be related to prematurity.     Investigation of hypovitaminosis D in patients infected with SARS-CoV-2 and its relationship with clinical worsening: a cross-sectional observational retrospective clinical study

    Numerous studies have demonstrated the profile of infection and symptoms related to COVID-19 that occur with pulmonary manifestations (such as Acute Respiratory Syndrome), digestive symptoms, anosmia, and ageusia. Many comorbidities have been associated with deaths and severe cases of the disease, such as diabetes, hypertension, obesity, and heart disease. However, many questions remain unanswered, especially the association of disease severity with hypovitaminosis D. Vitamin D deficiency is widely found in patients in Intensive Care Units, and recent studies have shown that it has been suggested that patients with severe manifestations of COVID-19 have hypovitaminosis D. Therefore, the present study aims to associate the presence of hypovitaminosis D in patients with the acute respiratory syndrome, positive for SARS-COV-2, admitted to the José Alencar Regional Hospital in Uberaba/Brazil and the evolution of the disease (days of hospitalization, hospitalization in the ICU, discharge and death) through the analysis of vitamin D (25(OH)D) hospitalized patients clinical records. The incidence of hypovitaminosis D among the patients was also assessed. The results may contribute to the understanding of the disease, as well as the need for vitamin supplementation

Neste estudo exploramos a influência do sistema imune de mucosa na infecção inicial pelo SM em crianças altamente expostas a este microrganismo. Cento e dezenove crianças, com idade inicial entre 5 a 13 meses de idade foram analisadas no início do estudo (T0) e após 6 (T6), 12 (T12) e 18 (T18) meses, amostras bucais foram coletadas para determinação dos níveis de infecção por SM, através do cultivo em mitis salivarius ágar com bacitracina. Os níveis de IgA, IgA1 e IgM foram também determinados em amostras de saliva, através de ensaios de ELISA. Um subgrupo de 21 crianças infectadas por SM (entre T0 e T6) foram pareadas a outras 21 crianças da amostra sem níveis detectáveis de SM. As reações entre anticorpos IgA da saliva com antígenos (Ags) de SM foram comparadas entre estes subgrupos, através de ensaios de western blot. A intensidade das bandas reativas foi determinada através da densitometria e expressas como unidades arbitrárias (ua). A reatividade de IgA salivar com peptídeos derivados da seqüência da GbpB, preditos como regiões ligantes à MHC de classe II, foram também avaliados através de multiplex com o Luminex. Os níveis salivares de IgA aumentaram com a idade (de 82,4 a 823,4µg/ml), enquanto que os níveis de IgM mantiveram-se baixos durante todo o estudo (média:4,2-2,8 µg/ml). Os níveis de infecção por SM foram maiores em T6 (média:45 ufc/área) e uma forte resposta de IgA aos Ags de SM pôde ser detectada logo aos 6 meses de idade. A resposta de IgA anti-GbpB foi observada em 38% das 21 crianças infectadas por SM, enquanto que 73% dos pares não infectados apresentavam estes anticorpos (Qui-quadrado, p<0.03). Um pico de resposta de IgA a GbpB ocorreu durante a fase de maior desafio de infecção (T6). Não houve diferenças significativas nos padrões de reatividade de IgA aos diversos peptídeos de GbpB testados, entre os grupos de crianças infectadas e não infectadas por SM. Os resultados indicam que uma resposta complexa de IgA a SM pode ocorrer a partir dos 6 meses e que a especificidade de resposta a Ags envolvidos na virulência podem influenciar na susceptibilidade à infecção por SM. Nenhum dos peptídeos da GbpB correspondeu às intensidades de resposta à GbpB nativa observadas entre os pares de crianças estudadosAbstract: Mutans streptococci (MS) are the main pathogens of dental caries. In this study we explored the influence of the muccosal immune system in the initial colonization by MS in children highly exposed to this microorganism. A total of 119 children, who were of 5 to 13 months of age, were enrolled in this prospective study. MS levels of infection were determined in cultures on mitis salivarius agar with bacitracin at baseline (T0), 6 (T6), 12 (T12) and 18 months (T18)-of follow-up. Saliva samples were also collected at all phases of the study for determination of the levels of antibodies IgA, IgA1 and IgM in ELISA assays. A subset of 21 MS-infected early between T0 and T6 were matched to other 21children, but who were not MS-infected. Patterns of IgA antibody reactivity to MS Ags were compared between these 21 pairs through assays of western blot. Intensities of IgAreactive bands were determined densitometrically and were expressed as arbitrary unit (au). Reactivities of salivary IgA antibodies to peptides derived from the GbpB sequence, which were predicted as having affinity to MHC of class II molecules were evaluated in multiplex with the help of the Luminex technology. Levels of antibody IgA in saliva increased with age (82.4-823.35 µg/ml), while levels of IgM remained at low levels (mean:4.2-2.78 µg/ml). The highest levels of MS infection in the infected group were observed at T6 (mean:45 cfu/plate). Robust responses to SM antigens were detected in children as early as 6 months of age. Only 38% of early-infected children carried IgA-reative GbpB while this antigen was recognized by the majority (73%) of children that were not infected by these microorganisms (chi-square, p<0.03). A peak of IgA response to GbpB occurred during the phase of highest SM infectious challenge (by T6). All the GbpB-derived peptides have shown reactivity with salivary IgA, independently of the status of MS infection. The results indicated that complex salivary IgA responses to MS Ags can occur by 6 months of age and that the patterns of salivary IgA specificities might influence in the susceptibility of initial infections to MS.     Analysis of colostrum IgA against bacteria involved in neonatal infections

    <div><p>ABSTRACT Objective To describe e compare the specificity of IgA antibodies against bacteria extract of Klebsiella pneumoniae , Staphylococcus aureus , Escherichia coli , and Salmonella enteritidis . Methods Colostrum samples were aseptically collected in the first 12 hours after C-section delivery. The specificity of IgA against bacteria extracts was analyzed by the Western blot. Results The findings showed proteins of high molecular weight frequently detectable in the samples. S. aureus was the most frequently found bacterium in the samples (p<0.05). Approximately 93.8, 56.3, 62.5 and 60.4% of samples presented IgA reactive to S. aureus , K. pneumoniae , S. enteritidis, and E. coli, respectively. Roughly 40% of samples showed no IgA reactive to K. pneumoniae, S. enteritidis and E. coli . Conclusion Clinical evidence of the importance of breastfeeding for the immune protection of neonates was consistent with the observed immunological findings, since most samples showed IgA reactive against the species tested. The application and development of immunotherapies during pregnancy, focused on frequently detected antigens, could be an important tool to enhance the presence of IgA in colostrum.</p></div

    Factors affecting the placement or replacement of direct restorations in a dental school

    Context: The knowledge of the reasons for the placement of direct restorations makes possible to trace an epidemiological profile of a specific population and to direct the teaching of dentistry to techniques that are commonly used today and will be continued performed in the future. Purpose: The aim of this study was to verify the reasons for placement and replacement of direct restorations in patients treated in the Dental Clinic of the Uberaba University - Brazil. Materials and Methods: This study evaluated 306 restorative procedures carried out on 60 patients. During the treatment planning, a form that contained information about the patient′s gender, tooth number, the classification of restorations, the reasons for placement and replacement of amalgam and tooth-colored restorations, the material that had to be removed and the new material used to fill the cavities was filled for each patient. Statistical analysis was carried out using Chi-square test (α = 0.05). Results: The data showed that most of the patients were female (66.7%). Of all the restorations placed, 60.45% were 1 st -time placements, while 39.55% were replacements. For 1 st -time restorations, the main reason for placement was primary caries (76.76%), followed by non-carious cervical lesions (15.14%). The amalgam restorations were replaced more frequently (67.77%). The primary reason for replacements was the presence of secondary caries (for both previous amalgam (42.68%) and composite (66.67%) restorations (P < 0.05). The resin composite was the most indicated material for the new restorations (98.04%) (P < 0.05). Conclusions: The main reason for placement of direct restorations was primary caries, while secondary caries was the main reason for replacements. In almost all cases, the material used to fill the cavities was the resin composite
