    OAGB Bowel Function in Patients With up to 5 Years Follow-Up: Updated Outcomes

    Objective: One-anastomosis gastric bypass (OAGB) is considered an effective technique in weight reduction and remission of comorbidities. However, in common with many bariatric and metabolic/bariatric procedures, gastrointestinal side effects are frequently reported, but clinical experience varies. The objective of this study was to analyze the bowel function of patients who undergo OAGB looking at 5-year postoperative outcomes. Method: This study is cross-sectional, descriptive and analytical, developed with individuals undergoing OAGB (n = 208) in yhe period between 2015 and 2020. The time periods evaluated were 1 to 6 months (T1), 6 to 12 months (T2), and 1 to 5 years (T3). Data analysis was performed using SPSS v.28.0, considering a significance level p ≤ 0.05. Results: 114 participants (54.8%), 79.8% women, mean age 47.0 ± 12.6 years, and BMI 40.1 ± 5.6 kg/m2, 51.9% dyslipidemia, 43.6% arterial hypertension, and 19.1% diabetes mellitus. The T1 group had more severe symptoms/nausea than the T2 group. The T2 group had a significantly lower defecation frequency than the T1 and T3 groups. As for the occurrence of diarrhea, associations were not found in the considered groups. The T3 group had a greater severity of constipation associated with greater difficulty in consuming red meat, white meat, rice, vegetables, and salads. Conclusions: Gastrointestinal symptoms are prevalent in the first postoperative months. However, diarrhea was not common. The patient selection policy and surgical technique were decisive in this result. Constipation was prevalent in patients between 1 and 5 postoperative years. It was also prevalent in those who had food intolerance, which from a nutritional point of view is an adverse factor for optimal bowel function.info:eu-repo/semantics/publishedVersio

    Genetic analysis of rennet coagulation time, curd-firming rate, and curd firmness assessed over an extended testing period using mechanical and near-infrared instruments

    Abstract The aims of this study were (1) to analyze rennet coagulation time (RCT), curd-firming rate, and curd firmness obtained by extending the standard 30-min testing time to 45min; (2) to estimate heritabilities of the aforementioned traits determined by mechanical (Formagraph; Foss Electric, Hillerod, Denmark) and near-infrared optical (Optigraph; Ysebaert, Frepillon, France) instruments, and to assess the statistical relevance of their genetic background by using the Bayes factor procedure, the deviance information criterion, and the mean squared error; (3) to estimate phenotypic and genetic relationships between instruments within trait and between traits within instrument; and (4) to obtain correlations for sire rankings based on the used instruments. Individual milk samples were collected from 913 Brown Swiss cows reared in 63 herds located in Trento Province (Italy). Milk coagulation properties (MCP) were measured using 2 different instruments: Formagraph and Optigraph. Both instruments were housed in the same laboratory and operated by the same technician. Each sample was analyzed simultaneously on each instrument. All experimental conditions (milk temperature and the concentration and type of rennet) were identical. For the analysis, univariate and bivariate animal models were implemented using Bayesian methods. Univariate analyses were conducted to test the hypothesis that the traits showed additive genetic determination. Deviance information criterion, Bayes factor, and mean squared error were used as model choice criteria. The main results were that (1) RCT could be measured on all samples by extending the observation time to 45min, and its genetic parameters ( h 2 =0.23) and breeding values could be estimated while avoiding the bias of noncoagulating samples; (2) curd-firming rate could be measured on almost all milk samples, and its genetic parameters could be estimated for the first time on a field data set ( h 2 =0.21); (3) for the first time, genetic parameters of curd firmness 45min after rennet addition ( h 2 =0.12) were estimated, and they were compared with curd firmness 30min after rennet addition ( h 2 =0.17); and (4) MCP estimated using the Optigraph appeared to be genetically different from those determined by Formagraph, with the partial exception of RCT (genetic correlation=0.97). Breeding strategies for the improvement of MCP must be planned with caution. Currently, the high throughput, ease of use, and reduced costs of analysis make predictions obtained from mid-infrared spectroscopy (MIRS) on untreated milk samples a promising alternative to produce relevant data at the population level. The use of mechanical lactodynamographs to establish reference data for MIRS calibrations have been already studied, whereas the use of near-infrared optical lactodynamographs as a reference method for MIRS calibrations needs to be investigated

    Genetic parameters for casein and urea content in the Italian Brown Swiss dairy cattle

    A total of 137,753 test day records of 20,745 Italian Brown Swiss dairy cows from 26 provinces of Italy were used to estimate heritability for casein and urea content in milk and their genetic correlations with other production traits and milk somatic cell score. Milk component values were obtained by Fourier Transformed Infrared (IR) Spectroscopy from milk samples collected during national routine recording and were analysed using test day repeatability animal models. Fixed effects included 1,001 levels of herd-test date, 15 classes of days in milk, and 13 classes of age at calving within parity. The variation among cows was large for most of the traits. The heritability value for casein content was 0.31, as for protein content, and genetic and phenotypic correlations between these two traits were large (0.99 and 0.97 respectively). Milk urea content had a heritability of 0.17 and a positive genetic relationship with fat (0.12), null with protein (0.03) and casein (0.002) content and a negative genetic correlation with milk yield (-0.17) suggesting that the genetic improvement for milk urea content would be possible, but genetic gain would be affected by other traits included as selection criteria in the economic index and by their relative economic emphasis

    Enhanced Recovery After Surgery (ERAS) protocol in bariatric and metabolic surgery (BMS)-analysis of practices in nutritional aspects from five continents

    This study aims to understand the prevalent practices on the nutritional aspects of the enhanced recovery after surgery (ERAS) protocol based on the knowledge and practice of surgeons, nutritionists, and anesthesiologists who work in the bariatric and metabolic surgery (BMS) units worldwide. This cross-sectional study enrolled BMS unit professionals from five continents-Africa, America, Asia, Europe, and Oceania. An electronic questionnaire developed by the researchers was provided to evaluate practices about the three nutritional aspects of ERAS protocol in BMS (Thorel et al. 2016): preoperative fasting, carbohydrate loading, and early postoperative nutrition. Only surgeons, nutritionists, and anesthesiologists were invited to participate. One hundred twenty-five professionals answered the questionnaires: 50.4% from America and 39.2% from Europe. The profile of participating professionals was bariatric surgeons 70.2%, nutritionists 26.4%, and anesthesiologists 3.3%. Approximately 47.9% of professionals work in private services, for about 11 to 20 years (48.7%). In all continents, a large majority were aware of the protocol. Professionals from the African continent reported having implemented the ERAS bariatric protocol 4.0 ± 0 years ago. It is worth mentioning that professionals from the five continents implemented the ERAS protocol based on the published literature (p = 0.012). About preoperative fasting abbreviation protocol, a significant difference was found between continents and consequently between services (p = 0.000). There is no uniformity in the conduct of shortening of fasting in the preoperative period and the immediate postoperative period. Early postoperative (PO) period protein supplementation is not performed in a standard fashion in all units globally. ERAS principles and practices are partial and insufficiently implemented on the five continents despite the prevalent knowledge of professionals based on evidence.     Short communication: Characterization of a monoclonal antibody for κ-casein B of cow's milk1

    A monoclonal antibody (antik-B) against an oligopeptide of 23 AA corresponding to the region 131-153 of bovine kappa-casein (kappa-CN) B was generated using the Human Combinatorial Antibody Library (HuCAL) technology. Both AA substitutions distinguishing kappa-CN A and B are located in that region (positions 136 and 148). In this study, the reactivity of antik-B to milk samples collected from cows previously genotyped as CSN3*AA, CSN3*AB, and CSN3*BB was tested. According to Western blot results, antik-B recognized kappa-CN B and it showed no cross-reactivity toward kappa-CN A and other milk proteins. Furthermore, a modified Western blot method, urea-PAGE Western blot, was set up to assess the reactivity of antik-B toward all isoforms of kappa-CN B. In conclusion, antik-B was specific to kappa-CN B in milk and it seemed to be reactive toward all its isoforms

    O Brasil está entre os maiores produtores e exportadores de grãos do mundo. Entretanto, apesar de seu sucesso, o setor agropecuário está passando por mudanças em sua estrutura e modelos alternativos de organização da estrutura produtiva agropecuária baseados no pilar: aumento da produção/produtividade e preservação ambiental passaram a ter grande destaque. É nesse contexto que se encontram os sistemas produtivos baseados no modelo de sistemas integrados de produção, como a integração Lavoura-Pecuária-Floresta (iLPF), sistemas que têm como princípios básicos a produção sustentável por meio da interação de atividades agrícolas, pecuárias e florestais, realizados em uma mesma área. A literatura fornece poucos exemplos de métodos e ferramentas desenvolvidas para a avaliação destes sistemas. Este trabalho traz contribuições importantes para compreender uma das partes mais críticas destes sistemas, que é o custo de produção. Apresentamos uma nova proposta de visualização dos custos de produção para o setor agropecuário que se baseia no sistema de custeio por atividades (Activity Based Costing) e apresentamos os benefícios que esta metodologia trouxe ao processo de gestão da propriedade e à tomada de decisão do produtor rural

    Posttranslational mechanisms associated with reduced NHE3 activity in adult vs. young prehypertensive SHR

First published July 14, 2010; doi:10.1152/ajprenal.00654.2009.-Abnormalities in renal proximal tubular (PT) sodium transport play an important role in the pathophysiology of essential hypertension. the Na(+)/H(+) exchanger isoform 3 (NHE3) represents the major route for sodium entry across the apical membrane of renal PT cells. We therefore aimed to assess in vivo NHE3 transport activity and to define the molecular mechanisms underlying NHE3 regulation before and after development of hypertension in the spontaneously hypertensive rat (SHR). NHE3 function was measured as the rate of bicarbonate reabsorption by means of in vivo stationary microperfusion in PT from young prehypertensive SHR (Y-SHR; 5-wk-old), adult SHR (A-SHR; 14-wk-old), and age-matched Wistar Kyoto (WKY) rats. We found that NHE3-mediated PT bicarbonate reabsorption was reduced with age in the SHR (1.08 +/- 0.10 vs. 0.41 +/- 0.04 nmol/cm(2)xs), while it was increased in the transition from youth to adulthood in the WKY rat (0.59 +/- 0.05 vs. 1.26 +/- 0.11 nmol/cm(2)xs). Higher NHE3 activity in the Y-SHR compared with A-SHR was associated with a predominant microvilli confinement and a lower ratio of phosphorylated NHE3 at serine-552 to total NHE3 (P-NHE3/total). After development of hypertension, P-NHE3/total increased and NHE3 was retracted out of the microvillar microdomain along with the regulator dipeptidyl peptidase IV (DPPIV).     Endothelial Dysfunction In Cardiovascular And Endocrine-metabolic Diseases: An Update.

    The endothelium plays a vital role in maintaining circulatory homeostasis by the release of relaxing and contracting factors. Any change in this balance may result in a process known as endothelial dysfunction that leads to impaired control of vascular tone and contributes to the pathogenesis of some cardiovascular and endocrine/metabolic diseases. Reduced endothelium-derived nitric oxide (NO) bioavailability and increased production of thromboxane A2, prostaglandin H2 and superoxide anion in conductance and resistance arteries are commonly associated with endothelial dysfunction in hypertensive, diabetic and obese animals, resulting in reduced endothelium-dependent vasodilatation and in increased vasoconstrictor responses. In addition, recent studies have demonstrated the role of enhanced overactivation of β-adrenergic receptors inducing vascular cytokine production and endothelial NO synthase (eNOS) uncoupling that seem to be the mechanisms underlying endothelial dysfunction in hypertension, heart failure and in endocrine-metabolic disorders. However, some adaptive mechanisms can occur in the initial stages of hypertension, such as increased NO production by eNOS. The present review focuses on the role of NO bioavailability, eNOS uncoupling, cyclooxygenase-derived products and pro-inflammatory factors on the endothelial dysfunction that occurs in hypertension, sympathetic hyperactivity, diabetes mellitus, and obesity. These are cardiovascular and endocrine-metabolic diseases of high incidence and mortality around the world, especially in developing countries and endothelial dysfunction contributes to triggering, maintenance and worsening of these pathological situations.44920-3
