Incidence and evaluation of incidental abnormal bone marrow signal on magnetic resonance imaging.
PurposeThe increased use of magnetic resonance imaging (MRI) has resulted in reports of incidental abnormal bone marrow (BM) signal. Our goal was to determine the evaluation of an incidental abnormal BM signal on MRI and the prevalence of a subsequent oncologic diagnosis.MethodsWe conducted a retrospective cohort study of patients over age 18 undergoing MRI between May 2005 and October 2010 at Tufts Medical Center (TMC) with follow-up through November 2013. The electronic medical record was queried to determine imaging site, reason for scan, evaluation following radiology report, and final diagnosis.Results49,678 MRIs were done with 110 patients meeting inclusion criteria. Twenty two percent underwent some evaluation, most commonly a complete blood count, serum protein electrophoresis, or bone scan. With median follow-up of 41 months, 6% of patients were diagnosed with malignancies including multiple myeloma, non-Hodgkins lymphoma, metastatic non-small cell lung cancer, and metastatic adenocarcinoma. One patient who had not undergone evaluation developed breast cancer 24 months after the MRI.ConclusionsIncidentally noted abnormal or heterogeneous bone marrow signal on MRI was not inconsequential and should prompt further evaluation
The ACS Survey of Galactic Globular Clusters: M54 and Young Populations in the Sagittarius Dwarf Spheroidal Galaxy
We present new Hubble Space Telescope photometry of the massive globular
cluster M54 (NGC 6715) and the superposed core of the tidally disrupted
Sagittarius (Sgr) dSph galaxy as part of the ACS Survey of Galactic Globular
Clusters. Our deep (F606W~26.5), high-precision photometry yields an
unprecedentedly detailed color-magnitude diagram showing the extended blue
horizontal branch and multiple main sequences of the M54+Sgr system. The
distance and reddening to M54 are revised usingboth isochrone and main-sequence
fitting to (m-M)_0=17.27 and E(B-V)=0.15. Preliminary assessment finds the
M54+Sgr field to be dominated by the old metal-poor populations of Sgr and the
globular cluster. Multiple turnoffs indicate the presence of at least two
intermediate-aged star formation epochs with 4 and 6 Gyr ages and [Fe/H]=-0.4
to -0.6. We also clearly show, for the first time, a prominent, 2.3 Gyr old Sgr
population of near-solar abundance. A trace population of even younger (0.1-0.8
Gyr old), more metal-rich ([Fe/H]\sim0.6) stars is also indicated. The Sgr
age-metallicity relation is consistent with a closed-box model and multiple
(4-5) star formation bursts over the entire life of the satellite, including

Antibacterial Activity of Marine and Black Band Disease Cyanobacteria against Coral-Associated Bacteria
Black band disease (BBD) of corals is a cyanobacteria-dominated polymicrobial disease that contains diverse populations of heterotrophic bacteria. It is one of the most destructive of coral diseases and is found globally on tropical and sub-tropical reefs. We assessed ten strains of BBD cyanobacteria, and ten strains of cyanobacteria isolated from other marine sources, for their antibacterial effect on growth of heterotrophic bacteria isolated from BBD, from the surface mucopolysaccharide layer (SML) of healthy corals, and three known bacterial coral pathogens. Assays were conducted using two methods: co-cultivation of cyanobacterial and bacterial isolates, and exposure of test bacteria to (hydrophilic and lipophilic) cyanobacterial cell extracts. During co-cultivation, 15 of the 20 cyanobacterial strains tested had antibacterial activity against at least one of the test bacterial strains. Inhibition was significantly higher for BBD cyanobacteria when compared to other marine cyanobacteria. Lipophilic extracts were more active than co-cultivation (extracts of 18 of the 20 strains were active) while hydrophilic extracts had very limited activity. In some cases co-cultivation resulted in stimulation of BBD and SML bacterial growth. Our results suggest that BBD cyanobacteria are involved in structuring the complex polymicrobial BBD microbial community by production of antimicrobial compounds

Whole-exome sequencing and clinical interpretation of FFPE tumor samples to guide precision cancer medicine
Translating whole exome sequencing (WES) for prospective clinical use may impact the care of cancer patients; however, multiple innovations are necessary for clinical implementation. These include: (1) rapid and robust WES from formalin-fixed paraffin embedded (FFPE) tumor tissue, (2) analytical output similar to data from frozen samples, and (3) clinical interpretation of WES data for prospective use. Here, we describe a prospective clinical WES platform for archival FFPE tumor samples. The platform employs computational methods for effective clinical analysis and interpretation of WES data. When applied retrospectively to 511 exomes, the interpretative framework revealed a “long tail” of somatic alterations in clinically important genes. Prospective application of this approach identified clinically relevant alterations in 15/16 patients. In one patient, previously undetected findings guided clinical trial enrollment leading to an objective clinical response. Overall, this methodology may inform the widespread implementation of precision cancer medicine
