La relazione che organizza il contesto sanitario: domanda dellâutenza e risposta dei servizi sanitari, nel territorio e nellâospedale - The relationship which organizes the healthcare context: usersâ demand and response of healthcare services, in the territory and the hospital
Our intent is to present citizensâ demand of health care services in Italy through a research structured in three studies. We used the Emotional Text Analysis (ETA) to lead the three s
tudies: the first study regarded the citizensâexpectations of the healthcare services, the second study regarded the point of view of the hospital personnel (medical doctors and nurses), the third regarded the point of view of the General Practitioners onthe health care services. These three studies are briefly presented. Data and outcome of an Assumed Similarity Test applied to all experimental subjects of the three studies in order to make a comparison among them, are also presented. The findings of the Emotional Text Analysis show that citizens, potential users of health services, center their requests on their individual subjectivity, and seek answers both to their suffering and to the feeling of alienation that characterizes the being sick feeling that makes them go to a medical doctor. For their part, general practitioners and hospital doctors immediately transform this subjective feeling in an objective medical diagnosis. Here a first gap emerges between patientâs demand and medical response, leavingthe possibility of an alliance on the diagnosis in order to jointly fight the disease. It has long been in place though a change that has expelled the patient also from sharing the diagnostic process, increasingly taken from self-centered dynamics within the healthcare system. This widens the gap between the request of the citizens and the response of the health services, that contributes to that conflictual growth to which defensive medicine gives a dysfunctional answer. The Assumed Similarity Test contributes to the interpretation of the health care dynamic identified in the Emotional Text Analysis; particularly in regard to the unfolding conflict, given by the hypothesis emerging from the data analysis, that a feeling of closeness for the hospital personnel goes along with an emotional stance of overpowering the othe

The NASA Spitzer Space Telescope
The National Aeronautics and Space Administration's Spitzer Space Telescope (formerly the Space Infrared Telescope Facility) is the fourth and final facility in the Great Observatories Program, joining Hubble Space Telescope (1990), the Compton Gamma-Ray Observatory (1991â2000), and the Chandra X-Ray Observatory (1999). Spitzer, with a sensitivity that is almost three orders of magnitude greater than that of any previous ground-based and space-based infrared observatory, is expected to revolutionize our understanding of the creation of the universe, the formation and evolution of primitive galaxies, the origin of stars and planets, and the chemical evolution of the universe. This review presents a brief overview of the scientific objectives and history of infrared astronomy. We discuss Spitzer's expected role in infrared astronomy for the new millennium. We describe pertinent details of the design, construction, launch, in-orbit checkout, and operations of the observatory and summarize some science highlights from the first two and a half years of Spitzer operations. Mammary implants: laboratory simulation of recreational diving conditions
To ascertain whether mammary implants are prone to changes in conformation or structure if they are submitted to recreational dives, eight mammary implants were submitted to 40 simulated dives to imitate an average recreational diving schedule. Matching implants were used as a control group. Photographs were taken before and after completion of the protocol. All implants were observed for changes in volume and checked for integrity. Variations in density were evaluated using a Tc scan. No changes in volume occurred after each dive. None of the implants showed ruptures, and Tc scanning failed to reveal any differences in density between tested and control implants. Cohesive-gel implants submitted to the simulated dives showed some morphological alterations. This study indicates that the mammary implants tested could be implanted in a sports diver, but raises concern about whether the increased exposure to stress could negatively affect their durability. Strange Messages: Chemical and Thermal Freeze-out in Nuclear Collisions
Thermal models are commonly used to interpret heavy-ion data on particle
yields and spectra and to extract the conditions of chemical and thermal
freeze-out in heavy-ion collisions. I discuss the usefulness and limitations of
such thermal model analyses and review the experimental and theoretical
evidence for thermalization in nuclear collisions. The crucial role of
correlating strangeness production data with single particle spectra and
two-particle correlation measurements is pointed out. A consistent dynamical
picture for the heavy-ion data from the CERN SPS involves an initial
prehadronic stage with deconfined color and with an appreciable isotropic

A Liposome-Micelle-Hybrid (LMH) Oral Delivery System for Poorly Water-Soluble Drugs: Enhancing Solubilisation and Intestinal Transport
A novel liposome-micelle-hybrid (LMH) carrier system was developed as a superior oral drug delivery platform compared to conventional liposome or micelle formulations. The optimal LMH system was engineered by encapsulating TPGS micelles in the aqueous core of liposomes and its efficacy for oral delivery was demonstrated using lovastatin (LOV) as a model poorly soluble drug with P-gp (permeability glycoprotein) limited intestinal absorption. LOV-LMH was characterised as unilamellar, spherical vesicles encapsulating micellar structures within the interior aqueous core and showing an average diameter below 200 nm. LMH demonstrated enhanced drug loading, water apparent solubility and extended/controlled release of LOV compared to conventional liposomes and micelles. LMH exhibited enhanced LOV absorption and transportation in a Caco-2 cell monolayer model of the intestine by inhibiting the P-gp transporter system compared to free LOV. The LMH system is a promising novel oral delivery approach for enhancing bioavailability of poorly water-soluble drugs, especially those presenting P-gp effluxes limited absorption
Family Planning and Child Survival Programs as Assessed In 1991
This volume continues a tradition initiated at the Population Council nearly 25 years ago when the first edition of âPopulation and Family Planning Programsâ was issued. That factbook began a series of 12 editions produced through 1993. The data in that series were consolidated and updated in another publication, and the present volume is now issued, based partly upon a 1989 questionnaire inquiry to over 100 developing countries. A range of maternal and child survival data were added to the family planning information. The relationships among fertility behavior, infant and child mortality, and maternal health are closely interwoven; moreover, many programs address all three issues rather than any one alone. The current volume provides the principal statistical indicators that are available for most developing countries. Some data are drawn from the questionnaire, whereas other data are taken from agencies that specialize in particular topics. We have limited this edition to the period since the mid-1980s. New data are included for large-scale family planning programs, together with closely related indicators on child survival and reproductive health
A new measurement of J/psi suppression in Pb-Pb collisions at 158 GeV per nucleon
We present a new measurement of J/psi production in Pb-Pb collisions at 158
GeV/nucleon, from the data sample collected in year 2000 by the NA50
Collaboration, under improved experimental conditions with respect to previous
years. With the target system placed in vacuum, the setup was better adapted to
study, in particular, the most peripheral nuclear collisions with unprecedented
accuracy. The analysis of this data sample shows that the (J/psi)/Drell-Yan
cross-sections ratio measured in the most peripheral Pb-Pb interactions is in
good agreement with the nuclear absorption pattern extrapolated from the
studies of proton-nucleus collisions. Furthermore, this new measurement
confirms our previous observation that the (J/psi)/Drell-Yan cross-sections
ratio departs from the normal nuclear absorption pattern for semi-central Pb-Pb
collisions and that this ratio persistently decreases up to the most central

The dependence of the anomalous J/psi suppression on the number of participant nucleons
The observation of an anomalous J/psi suppression in Pb-Pb collisions by the NA50 Collaboration can be considered as the most striking indication for the deconfinement of quarks and gluons at SPS energies. In this Letter, we determine the J/psi suppression pattern as a function of the forward hadronic energy E-ZDC measured in a Zero Degree Calorimeter (ZDC). The direct connection between EZDC and the geometry of the collision allows us to calculate, within a Glauber approach, the precise relation between the number of participant nucleons N-part and E-ZDC. Then, we check if the experimental data can be better explained by a sudden or a smooth onset of the anomalous J/psi suppression as a function of the number of participants.
