Clinical Cases in Obstetrics, Gynaecology and Women's Health

An updated edition for our popular Clinical Cases collection, this book is aimed medical students following the core curriculum in Australia and New Zealand; and the United Kingdom, junior doctors at resident or house officer level, and general practitioners. This handy, lab coat pocket-sized book broadly covers the whole syllabus for the medical student, and the whole range of obstetric and gynaecological problems likely to come into the path of the busy junior hospital doctor or general practitioner, but it is not a textbook providing simply a list of differential diagnoses and treatments. The approach has been developed from the experienced authors teaching of problem-based learning scenarios (PBLs) to clinical students. PBLs introduce students to typical clinical situations and then encourage those students to think about how they would take appropriate histories, examine, investigate and finally treat their patients. In this book, by using case histories that are more developed and complex, we aim to show readers in greater detail how women may present with particular conditions, and demonstrate what should take place in the way of consultation, investigations and treatment. However, by questioning our readers as we go along we also hope to encourage them to think about why they would choose a certain clinical course of action and to base their decisions firmly on current scientific evidence. In addition to the purely clinical aspects of the cases, the emotional, social and psychological aspects of the care of each woman is described. Each of the 50 case histories commences with a straightforward description following a woman through the clinical presentation of a particular condition. Along the way, important points in clinical examination and diagnosis, complications, investigations and management are incorporated into the text as a conversation with the reader, and essential points are highlighted in boxes. Being an Australian author team, tips are included for those junior doctors faced with obstetric or gynaecological problems in a smaller metropolitan or rural setting - facilities for care and the advice of senior practitioners may be more limited in such surroundings and different care paths may be more appropriate, especially in remote parts of Australia

RPM: um protocolo para comunicação anônima em redes par a par (P2P)

Programa de Pós-graduação em Engenharia de Automação e SistemasO anonimato é um preocupação crescente nos atuais sistemas baseados na Internet. As redes de anonimato tradicionais, baseadas em misturadores ou multicast, possuem limitações de confiabilidade, confidencialidade e desempenho. A ampla escala de redes P2P pode ser usada para minimizar tais limitações, mas essas redes têm de lidar com o fenômeno do churn (entrada e saída de nós na rede) e a menor confiabilidade dos nós individuais (devido ao roteamento na camada de aplicação). Esta dissertação apresenta o RPM (Random Path + Multicast), um protocolo para comunicação anônima em sistemas P2P. Além do anonimato, o RPM tem por objetivo a resistência ao churn e a redução do custo computacional normalmente associado a sistemas de anonimato. Para o seu desenvolvimento, primeiramente foi realizada uma revisão bibliográfica de grande parte da literatura referente ao assunto abordado. Posteriormente foram definidos os objetivos gerais e específicos do projeto visando definir assim sua estrutura funcional. Ao final, foram realisados diversos testes através de simulações, os quais demonstraram que o RPM atinge eficazmente seus objetivos, especialmente com respeito à resistência ao churn

The senior manager as a strategy manager in change scenarios: practice in the brazilian public sector

How does the senior manager of a public organization practice the strategy and mobilize his team to enable continuously, in the organization where it is located: meeting its mission, stakeholder needs and the dynamics of the internal and external environment? This question guides the research of this article which also presents the structuring of a framework that brings together four academic approaches - Carlos Matus\u27 Situational Strategic Planning (1991; 1997; 2005), as a central element; Whittington\u27s Strategy as a Social Practice (1997; 2002; 2004; 2006); Freeman\u27s Stakeholder Theory (2010); and Burns\u27 Transformational Leadership (1978; 2003). Data were collected in semi-structured interviews and treated qualitatively by Thomas\u27s inductive analysis (2006), the Gibbs coding and categorization method (2009) and the matching presented by Trochim (1989). The framework should serve as a reference for the action of the manager regarding the mentioned issue

Análise de desempenho de fundos comportamentais

[77]-81Este trabalho analisou o desempenho de 31 fundos mútuos comportamentais atuantes nos EUA, Europa e Japão descritos em Santoni e Kelshiker (2010). Foram observados os desempenhos dos fundos e seus respectivos benchmarks em quatro indicadores: Índice de Sharpe, Índice de Sortino, Medida Ômega e Medida de Desempenho Comportamental. O horizonte da análise foi de 10 anos (jan/04 a dez/14) dividido em intervalos de 6, 12, 36, 60 e 120 meses. A partir da consolidação dos indicadores os fundos foram ranqueados e classificados em três faixas de desempenho: superior, intermediário e inferior. No intervalo de 120 meses não houve, na média geral, diferença de desempenho significativa (a 5%) entre os fundos e os Benchmarks. A análise por intervalos indicou que o desempenho dos fundos em relação aos Benchmarks piora conforme aumenta o prazo de aplicação. Nos intervalos mais curtos (6 e 12 meses) não houve, na média, diferença de desempenho significativa enquanto nos prazos mais longos (36 e 60 meses) o desempenho médio dos fundos foi significativamente inferior aos Benchmarks. Na média de todos os intervalos o desempenho médio dos fundos foi significativamente inferior aos Benchmarks. Dentre os indicadores utilizados, o índice de Sortino foi o que apresentou maior correlação com o desempenho geral dos fundos.This work has analyzed the performance of 31 behavioral mutual funds operating in USA, Europe and Japan, as described in Santoni and Kelshiker (2010). It has been observed the performance of the funds and their respective Benchmarks according to four measures: Sharpe Index, Sortino Index, Omega Measure and Behavioral Perfomance Measure. The analysis covered a 10-year period (jan-04 to dec-14) slipt into intervals of 6, 12, 36, 60 and 120 months. Based on the consolidation of the performance measures, the funds have been ranked and classified into three performance categories: upper, intermediate and lower. In the 120-month interval there has not been, on average, a significant difference (at 5%) in performance between funds and Benchmarks. The analysis by intervals showed that the funds’ performance worsens in relation to the Benchmarks as the investment period increases. In shorter intervals (6 and 12 months) there has not been, on average, a significant difference in performance while in the longer intervals (36 and 60 months) the funds average performance was significantly lower than the Benchmarks. Computing the mean of all intervals, the funds average performance was significantly lower than the Benchmarks. Among the performance measures used, the Sortino Index presented the highest correlation with the general performance of the funds

Cuidando da grávida adolescente - experiência na ESF Alecrin, Japeri, Rio de Janeiro

A gravidez na adolescência é um problema muito comum e significativo. Em países subdesenvolvidos, como no Brasil, cresce o número de adolescentes com idade igual ou inferior a 18 anos e o presente estudo demonstrou que na comunidade do Alecrim, localidade de Engenheiro Pedreira, município de Japeri, rio de Janeiro, a incidência alcançou 59%. A partir do diagnóstico feito pela equipe de saúde, um plano de intervenção foi elaborado de forma a qualificar o cuidado à adolescente grávida com ações de assistência, promoção e prevenção. Evidenciou-se melhora do vínculo gestante-equipe, acompanhamento de todas as gestantes (19 adolescentes) com sete consultas ou mais, encaminhamento ao pré-natal de alto risco e desenvolvimento de grupo para discutir questões vivenciadas pelas participantes. Contudo, o trabalho suscitou na equipe a necessidade de qualificar a atenção a saúde do adolescente antes que a gestação ocorra
