    Occupational chronic obstructive pulmonary disease

    Occupational chronic obstructive pulmonary disease, despite having been widely discussed for nearly half a century, is still rarely addressed in Brazil. Various studies, especially those that were population-based, have revealed the relationship between occupational exposure to aerosols and impairment of the airways. This chapter aims to remind physicians of the diagnosis of occupational chronic obstructive pulmonary disease by presenting a succinct review of the literature on the theme, which should be incorporated into the Global Initiative for Chronic Obstructive Lung Disease, in terms of the scope of the diagnostic basis as well as in terms of the questionnaire specific for the disease. Collecting detailed work histories and characterizing exposure to inhaled agents known to have deleterious effects on the respiratory system will surely result in improved approaches to making diagnoses and prognoses of this disease, as well as contributing to its greater control.A doença pulmonar obstrutiva crônica ocupacional, apesar de ampla discussão há quase meio século, ainda é muito pouco abordada em nosso meio. Diversos estudos, especialmente os de base populacional, revelaram a associação entre as exposições ocupacionais aos aerodispersóides e o comprometimento das vias aéreas. Este capítulo objetiva alertar para o diagnóstico da doença pulmonar obstrutiva crônica de origem ocupacional apresentando uma revisão suscinta sobre o tema que deverá ser incorporado ao projeto Global Initiative for Chronic Obstructive Lung Disease, tanto no seu escopo de fundamentação diagnóstica quanto em seu questionário específico.     Tuberculose pulmonar: relação entre baciloscopia do escarro e lesões radiológicas

    OBJECTIVES: To determine the frequency of radiological manifestations of chest tuberculosis among the tuberculosis outpatients at the Santa Casa de Misericórdia de São Paulo Hospital, and to correlate these radiological findings with the sputum bacilloscopy. SAMPLE AND METHODS: A review was made of the medical record cards and chest X-rays of all patients attended between January 1996 and December 1998. Patients with a diagnosis of tuberculosis who presented intrathoracic manifestations of the disease and negative anti-HIV serology were selected. RESULTS: The selection included 153 patients, with an average age of 37.5 years, who were predominantly male (60.8%) and white (56.9%). Pulmonary lesions were present in 121 (79.9%) and extrapulmonary lesions in 32 (20.1%). Parenchymal-infiltrate lesions appeared in 56 patients (36.6%), cavity lesions in 55 (36.0%), pleural effusion in 28 (18.3%), isolated nodules in 6 (3.9%), mediastinal enlargement in 4 (2.6%) and miliary pattern in 4 (2.6%). Cavities were present in 45.5% of the patients with pulmonary lesions, generally in association with the parenchymal-infiltrate lesions. Parenchymal infiltrate was present in 86.8% of the patients with pulmonary lesions. There was significant presence of alcohol-acid resistant bacillus in the sputum of patients with cavities (76.4%), in comparison with those without cavities (50%) (p = 0.003). CONCLUSIONS: Parenchymal-infiltrate lesions are the most frequent radiological manifestation of pulmonary tuberculosis, and they are generally associated with cavities. There is a relationship between the presence of acid fast bacilli in sputum and pulmonary cavity lesions.OBJETIVOS: Determinar a freqüência das manifestações radiológicas da tuberculose nos pacientes do Ambulatório de Tuberculose do Departamento de Medicina da Santa Casa de Misericórdia de São Paulo, e correlacionar a manifestação radiológica e a baciloscopia direta do escarro. CASUÍSTICA E MÉTODOS: Foram revisados os prontuários e as radiografias de tórax de todos os doentes atendidos entre janeiro/1996 e dezembro/1998. Foram selecionados os doentes com diagnóstico de tuberculose e que apresentavam manifestação intratorácica da doença e sorologia anti-HIV negativa. RESULTADOS: Foram incluídos 153 doentes, média de 37,5 anos, predominância do sexo masculino (60,8%) e da raça branca (56,9%). Lesões pulmonares estavam presentes em 121 (79,1%); no restante ocorreu envolvimento torácico sem comprometimento pulmonar. Alterações do tipo infiltrado pulmonar ocorreram em 56 (36,6%), cavidade em 55 (36,0%), derrame pleural em 28 (18,3%), nódulo isolado em 6 (3,9%), linfonodomegalia mediastinal em 4 (2,6%) e padrão miliar em 4 (2,6%). Excetuando-se as lesões extrapulmonares, cavidades estiveram presentes em 45,5% dos doentes com lesões pulmonares. Considerando o infiltrado pulmonar que acompanhou a lesão cavitária, em 105 (68,6%) o infiltrado esteve presente. Excluindo-se aqueles com lesões extrapulmonares, o infiltrado apareceu em 86,8% dos doentes com lesões pulmonares. Baciloscopia foi positiva em 76,4% dos doentes com cavidade e em 50,0% dos sem cavidade (p = 0,003). CONCLUSÕES: O infiltrado pulmonar constitui-se na manifestação radiográfica mais freqüente da tuberculose pulmonar, estando a cavidade geralmente associada às lesões do tipo infiltrado pulmonar.     ASTHMA: THE PERCEPTION OF IMPAIRMENTS, DISABILITIES AND HANDICAPS OF CHILDREN AND ADOLESCENTS

    O presente trabalho pautou-se na Classificação Internacional das Deficiências, Incapacidades e Desvantagens e teve por objetivo conhecer a percepção de crianças e adolescentes asmáticos ante a deficiência, a incapacidade e a desvantagem. Doze crianças e adolescentes, com diagnóstico médico de asma persistente moderada foram entrevistadas e realizaram dois desenhos: um desenho livre e outro sobre asma. Utilizou-se a abordagem qualitativa de pesquisa. Foram levantadas questões concernentes ao conhecimento da doença e ao seu impacto na vida diária dessas crianças, às estratégias que podem operar-se na casa, na escola e nas relações interpessoais. Concluiu-se que essas questões, quando conhecidas e compreendidas, trazemalertas às equipes de saúde no sentido de conscientizá-las quanto à estreita relação existente entre crianças e adolescentes asmáticos e a maneira como esses sentem e agem perante a doença.This article was based on the International Classification of Impairments, Disabilities and Handicaps and its aim was to identify the perception of asthmatic children and adolescentsas to their impairments, handicaps and disabilities. Twelve children and adolescents  ali with a medical diagnosis of persistent moderated asthma, were interviewed and then instructed to produced two drawings: one concerning free-will and the another about asthma. A qualitative research approach was,used. Aspects related to their knowledge of the disease, its impact on daily life, and the strategies they could utilize in order to better operate at home, in school and their personal relations, was proposed. We concluded that these issues, when acknowledged and understood, can assist the health team to alert them and increase their awareness as to the closerelationship between asthmatic children and adolescents as well as the way they feel and act related to this disease

    Avaliação da composição corporal, capacidade funcional e função pulmonar em pacientes com Doença Pulmonar Obstrutiva Crônica

    La enfermedad pulmonar obstructiva crónica (EPOC) está relacionada con un bajo índice de masa corporal (IMC), reducción de la capacidad funcional y reducción de la densidad ósea, justificando así la importancia de evaluar todos estos parámetros en pacientes con la enfermedad. Este es un estudio transversal, con una muestra de 20 pacientes, que se sometieron a la medición de la composición corporal por bioimpedancia, la evaluación de la capacidad funcional por la distancia recorrida en el Incremental Shuttle Walk Test (ISWT) y la evaluación de la gravedad de la obstrucción por la espirometría. Se encontraron diferencias significativas entre la distancia recorrida y prevista del ISWT (pA doença pulmonar obstrutiva crônica (DPOC) está relacionada a um baixo índice de massa corporal (IMC), à redução da capacidade funcional e à redução da densidade óssea, justificando assim a importância de se avaliar todos esses parâmetros nos pacientes portadores da doença. Trata-se de um estudo transversal, com amostra composta por 20 pacientes, que realizaram medida de composição corporal por bioimpedância, avaliação da capacidade funcional pela distância percorrida no Incremental Shuttle Walk Test (ISWT) e avaliação da gravidade da obstrução pela espirometria.     Evaluation of obstructive sleep apnea in non-cystic fibrosis bronchiectasis: A crosssectional study

    The relationship between sleep disorders and bronchiectasis has not been well described. We hypothesize that, due to the irreversible dilatation of the bronchi, the presence of secretions, and airflow obstruction, patients with non-cystic fibrosis bronchiectasis may be predisposed to hypoxemia during sleep, or to symptoms that may lead to arousal. A cross-sectional observational study was performed involving 49 patients with a clinical diagnosis of non-cystic fibrosis bronchiectasis (NCFB). All patients underwent clinical evaluation, spirometry, and polysomnography, and were evaluated for the presence of excessive daytime sleepiness (EDS) and risk of obstructive sleep apnea (OSA). The mean age of the participants was 50.3 +/- 13.6 years51.1% of patients were male and had a mean body mass index of 23.8 +/- 3.4 kg/m(2). The mean total sleep time (TST) was 325.15 +/- 64.22 min with a slight reduction in sleep efficiency (84.01 +/- 29.2%). Regarding sleep stages, stage 1 sleep and REM sleep were abnormal. OSA was present in 40.82% of the patients. The mean arousal index was 5.6 +/- 2.9/h and snoring was observed in 71.43% of the patients. The oxygen desaturation index (ODI) was 14.35 +/- 15.36/h, mean minimum oxygen saturation (SpO(2) nadir) was 83.29 +/- 7.99%, and mean TST with an SpO(2) less than 90% was 30.21 +/- 60.48 min. EDS was exhibited by 53.06% of the patients and 55.1% were at high risk of developing OSA. The patients infected by Pseudomonas aeruginosa had higher apnea-hypopnea indices, ODI, and TST with SpO(2) < 90%, and lower values of SpO(2) nadir.     Estimulação diafragmática elétrica transcutânea (EDET) para fortalecimento muscular respiratório: estudo clínico controlado e randomizado

    O objetivo do estudo foi demonstrar o efeito de dois protocolos da estimulação diafragmática elétrica transcutânea (EDET) sobre a força muscular respiratória de mulheres saudáveis, sendo um protocolo segundo Geddes et al. (1988) e outro padronizado pelo equipamento Phrenics. Mulheres saudáveis foram divididas em 3 grupos: Controle (n=7); EDET com Phrenics (n=7) e EDET com Dualpex (n=7), sendo o tratamento realizado 2 vezes por semana, durante 6 semanas (12 sessões). Foram avaliadas a pressão inspiratória máxima (PImáx) e pressão expiratória máxima (PEmáx), antes e após o tratamento. A análise estatística foi realizada pelo teste Shapiro-Wilk e Kruskal Wallis com pós-hoc de Dunn (pThe objective of this study was to demonstrate the effect of two protocols of transcutaneous electrical diaphragmatic stimulation (TEDS) on the respiratory muscle strength of healthy women, with one protocol according to Geddes et al.
