    Patient-reported outcomes following flexible sigmoidoscopy screening for colorectal cancer in a demonstration screening programme in the UK

    <p>OBJECTIVES: Flexible sigmoidoscopy (FS) screening for colorectal cancer will be introduced into the National Cancer Screening Programmes in England in 2013. Patient-reported outcome measures (PROMs) from trial participants indicate high acceptability and no adverse physical or psychological consequences, but this may not generalize to routine screening in the community. This study examined PROMs in a community-based FS screening programme.</p> <p>METHODS: Eligible adults aged 58-59 (n = 2016) registered at 34 London general practices were mailed a National Health Service-endorsed invitation to attend FS screening. Pain and side-effects were assessed in a 'morning-after' questionnaire, and satisfaction was assessed in a three-month follow-up questionnaire. Anxiety, self-rated health and colorectal symptoms were assessed at prescreening and follow-up.</p> <p>RESULTS: In total, 1020 people attended screening and were included in the current analyses, of whom 913 (90%) returned the morning-after questionnaire, and 674 (66%) the follow-up questionnaire. The prescreening questionnaire had been completed by 751 (74%) of those who attended. The majority (87%) of respondents reported no pain or mild pain, and the most frequent side-effect (wind) was only experienced more than mildly by 16%. Satisfaction was extremely high, with 98% glad they had the test; 97% would encourage a friend to have it. From prescreening to follow-up there were no changes in anxiety or self-rated health, and the number of colorectal symptoms declined. Satisfaction and changes in wellbeing were not moderated by gender, deprivation, ethnicity or screening outcome.</p> <p>CONCLUSIONS: PROMs indicate high acceptability of FS screening in 58-59 year olds, with no adverse effects on colorectal symptoms, health status or psychological wellbeing.</p&gt

    Serum Vitamin D Levels and Polycystic Ovary syndrome: A Systematic Review and Meta-Analysis

Nutrients, 7(6), 4555-4577. doi:10.3390/nu7064555Vitamin D deficiency (VDD) is common in women with and without polycystic ovary syndrome (PCOS) and may be associated with metabolic and endocrine disorders in PCOS. The aim of this meta-analysis is to assess the associations of serum vitamin D levels with metabolic and endocrine dysregulations in women with PCOS, and to determine effects of vitamin D supplementation on metabolic and hormonal functions in PCOS patients. The literature search was undertaken through five databases until 16 January 2015 for both observational and experimental studies concerning relationships between vitamin D and PCOS. A total of 366 citations were identified, of which 30 were selected (n = 3182). We found that lower serum vitamin D levels were related to metabolic and hormonal disorders in women with PCOS. Specifically, PCOS patients with VDD were more likely to have dysglycemia (e.g., increased levels of fasting glucose and homeostatic model assessment-insulin resistance index (HOMA-IR)) compared to those without VDD. This meta-analysis found no evidence that vitamin D supplementation reduced or mitigated metabolic and hormonal dysregulations in PCOS. VDD may be a comorbid manifestation of PCOS or a minor pathway in PCOS associated metabolic and hormonal dysregulation. Future prospective observational studies and randomized controlled trials with repeated VDD assessment and better characterization of PCOS disease severity at enrollment are needed to clarify whether VDD is a co-determinant of hormonal and metabolic dysregulations in PCOS, represents a consequence of hormonal and metabolic dysregulations in PCOS or both

    Mechanochemical Regulation of a Photochemical Reaction

    We introduce the concept of mechanochemically gated photoswitching. Mechanical regulation of a photochemical reaction is exemplified using a newly designed mechanophore based on a cyclopentadiene–maleimide Diels–Alder adduct. Ultrasound-induced mechanical activation of the photochemically inert mechanophore in polymers generates a diarylethene photoswitch via a retro-[4 + 2] cycloaddition reaction that photoisomerizes between colorless and colored states upon exposure to UV and visible light. Control experiments demonstrate the thermal stability of the cyclopentadiene–maleimide adduct and confirm the mechanical origin of the “unlocked” photochromic reactivity. This technology holds promise for applications such as lithography and stress-sensing, enabling the mechanical history of polymeric materials to be recorded and read on-demand

    Racial and Socioeconomic Disparities in Melanoma Incidence Rates in Georgia: 2000-2011

    Background: The objective of this research was to investigate melanoma incidence rates and health outcomes in Georgia over time and by race, socio-economic status (SES), and gender. Methods: Age-adjusted melanoma incidence rates were obtained from the Georgia Comprehensive Cancer Registry SEER*Stat Database (2000-2011). To compare incidence rates across counties, and public health districts and by race, SES and gender, maps were generated using Geographic Information Systems (GIS). A cluster analysis was performed by use of SaTScan, and maps were created to visualize clusters of melanoma cases. Results: In Georgia, from 2000-2011, age-adjusted incidence rates for melanoma were higher among Whites than Blacks (28.0 vs. 1.1 per 100,000 population). For both races, high rates were found to be associated with high SES. For Whites, high rates were concentrated in urban areas relative to Blacks in rural areas. Clusters of melanoma incident cases were found mainly in the north central region of Georgia. Conclusions: For Georgia, results for map comparisons are consistent with previous research findings that higher melanoma incidence rates are associated with high SES for Whites and, to a lesser extent, for Blacks. Melanoma interventions in Georgia should focus on urban White and rural Black at-risk populations, especially those with high SES

No primeiro experimento, foi usada uma raçao-base de 51% restolho do milho, 28% torta de algodao, 20% milho e 1% sal, na quaI a torta de algodao foi substituîda por 5%, 10%, e 15% de feno de mata-pasto (Cassia sericea), enquanto um Iote-testemunha era mantido em pastoreio. Os animais confinados ganharam 95, 106, 103 e 93 gramas/cabeça/dia (P > 0,05), para as rações com 0%, 5%, 10% e 15% de mata-pasto, respectivamente. Os animais em pastoreio perderam 11 gramas/cabeça/dia (P 0.05) for tho rations with 0%, 5%, 10% and 15% of "mata-pasto", respective-ly.     Quantum threshold for optomechanical self-structuring in a Bose-Einstein condensate

    Theoretical analysis of the optomechanics of degenerate bosonic atoms with a single feedback mirror shows that self-structuring occurs only above an input threshold that is quantum mechanical in origin. This threshold also implies a lower limit to the size (period) of patterns that can be produced in a condensate for a given pump intensity. These thresholds are interpreted as due to the quantum rigidity of Bose-Einstein condensates, which has no classical counterpart. Above the threshold, the condensate self-organizes into an ordered supersolid state with a spatial period self-selected by optical diffraction

    Doing gender locally: The importance of ‘place’ in understanding marginalised masculinities and young men’s transitions to ‘safe’ and successful futures

    Observable anxieties have been developing about the position of boys and young men in contemporary society in recent years. This is expressed as a crisis of masculinity, in which place is often implicitly implicated, but is rarely considered for its role in the shaping of young men’s practices, trajectories and aspirations. Drawing on research conducted with young people who accessed a range of social care support services, this article argues that transition means different things for young men in different locales and that local definitions of masculinity are required to better understand young men’s lives and the opportunities available to them. The authors argue that home life, street life, individual neighbourhoods, regions and nations all shaped the young men’s identities and the practices they (and the staff working with them) drew on in order to create successful futures and ‘safe’ forms of masculinity. It is suggested that this place-based approach has the potential to re-shape the ‘crisis’ discourse surrounding masculinity and the anxieties associated with young men

    Laser ablation loading of a radiofrequency ion trap

    The production of ions via laser ablation for the loading of radiofrequency (RF) ion traps is investigated using a nitrogen laser with a maximum pulse energy of 0.17 mJ and a peak intensity of about 250 MW/cm^2. A time-of-flight mass spectrometer is used to measure the ion yield and the distribution of the charge states. Singly charged ions of elements that are presently considered for the use in optical clocks or quantum logic applications could be produced from metallic samples at a rate of the order of magnitude 10^5 ions per pulse. A linear Paul trap was loaded with Th+ ions produced by laser ablation. An overall ion production and trapping efficiency of 10^-7 to 10^-6 was attained. For ions injected individually, a dependence of the capture probability on the phase of the RF field has been predicted.
