    Comparison of the Nutritional Values of Toddler Milks Available in Italy

    If breast milk is not available infant formula ensures a balanced intake of nutrients and is undoubtedly more suitable for infants than cows’ milk. In particular, it should point out the absolute necessity to postpone to the end of the first year of life, or even after the 2 nd year, the use of cow's milk for the extreme imbalance of nutrients that lead to highprotein diets and low levels of polyunsaturated fats, iron and zinc. As a consequence, in the absence of breast milk, the use of an appropriately adapted formula in the first year of life and the use of “toddler milk” from 12 to 36 months may represent adequate nutritional alternatives, especially when compare to the use of cow milk, and in particular may  appear to play a fundamental role in the prevention of iron deficiency anemia. Different varieties of toddler milk are currently available in Italy.     Nutritional value of a rice-hydrolysate formula in infants with cows' milk protein allergy: a randomized pilot study.

    This study was designed to assess whether a rice-hydrolysate formula allows normal growth and adequate metabolic balance in infants with cows' milk protein allergy. Infants (seven females, nine males; aged 6–14 months) were randomly assigned to receive a rice-hydrolysate formula ( n = 8) or a soy formula (control group, n = 8). Standardized growth indices ( Z scores) and biochemical parameters were evaluated during a 6-month treatment period. Infants in both groups showed normal growth patterns during the study, and no adverse reactions were seen. Mean plasma biochemical parameters were within the normal ranges, and did not differ between groups. In conclusion, rice-hydrolysate formula may be a nutritionally suitable alternative for infants with cows' milk protein allergy. Larger studies, with satisfactory power, should be undertaken to confirm these findings

    This illustrated book consists of seven chapters. The first 3, ranging from proto-history to the present day, address most of historical moments of the evolution of knowledge and teaching in Human Anatomy and related Sciences produced in South Sardinia, the City of Cagliari, its University and Anatomical Institutions. Chapter 4 is entirely devoted to the city of Cagliari Anatomical Theatres and Anatomical Rooms, places previously little known or totally unknown: an itinerary from Palazzo Belgrano seat of the Rectorate, to the Anatomical Institute of via Porcell. The latter, opened in 1922-23 has been Cagliari most important Anatomical Institution until 1998. Chapter 5 outlines the historical evolution of methods used for anatomical studies, from dissection to microscopy, tracing the links of Anatomy with various philosophical and scientific disciplines as well. Chapter 6 summarizes the general chronology and offers a prosopographical insight into the life, thought, and historical-social context of the Persons who have alternated in Anatomy teaching. Besides Professors, Sectors and Assistants, several of whom renowned protagonists of the international scientific world, are carefully considered. Chapter 7. Finally, we dedicate an ample space to the most recent changes that highlight the evolution of the investigation methods applied by optical microscopy and transmission and scanning electron microscopy; they encompass immunochemistry, morphometry, cell cultures, in a constantly transforming academic context. Developments in the last 55 years allowed us to write pages of history closer to our times and rich in memories of places, facts and people. They concern the Institute which later became a Department, the PhD program, the museum spaces in which excels the Anatomical Wax Museum, for its worldwide relevance and undisputed beauty of the works. Last, but not least, a space is dedicated to the Research Laboratories of the present-day Cytomorphology Section of the Department of Biomedical Sciences, and their contribution to the progress of scientific research in the morphological and functional field

    Bendamustine plus rituximab is an effective first-line treatment in hairy cell leukemia variant: A report of three cases

    Hairy cell leukemia variant (HCLv) is a chronic lymphoproliferative disorder classified as a provisional entity in the 2016 WHO Classification of Lymphoid Tumors. HCLv is characterized by unfavorable prognosis, low complete remission rates and limited disease control following classical hairy cell leukemia-based regimens. In this study, we report 3 cases of elderly patients with treatment-naive, TP53 un-mutated HCLv, who were effectively treated with four cycles of bendamustine plus rituximab. The regimen was completed in all the patients with acceptable toxicity. All patients achieved a complete clinical response with no evidence of residual disease at bone marrow biopsy and flow-cytometry examination. After a median follow-up of 19 months, the 3 subjects are still in complete remission. In this work, bendamustine plus rituximab proved to be an effective and feasible first-line treatment strategy for elderly patients with TP53 un-mutated HCLv

    A Specific Prebiotic Mixture Added to Starting Infant Formula Has Long-Lasting Bifidogenic Effects123

    There is some evidence that early colonization of the intestine affects the composition of the intestinal microbiota after weaning. In the present study, the effect of prebiotics administered from the first day of life on fecal counts of bifidobacteria and lactobacilli were studied during and after the administration of the prebiotics. In this double-blind, randomized, placebo-controlled, explorative study, 20 newborns of hepatitis C virus-infected mothers who decided not to breast feed due to their concerns regarding their plasma viral load were randomly assigned to either a formula with 8 g/L of a specific prebiotic mixture (short-chain galacto-oligosaccharides and long-chain fructo-oligosaccharides, ratio 9:1) or a formula containing the same amount of maltodextrin (placebo). Clinical examination including anthropometric measurements, microbiological analysis of fecal samples, and blood leukocyte population analysis were performed at birth and 3, 6, and 12 mo age. At the age of 12 mo, hepatitis B vaccine-specific IgG serum titers (Hepatitis B virus surface antibodies) were also measured. Prebiotic supplementation resulted in more fecal bifidobacteria (P < 0.0001) and lactobacilli (P = 0.0044) compared with the placebo group. These differences between the groups were maintained during the second half of the first year without any prebiotic supplementation. There was no influence of the different diets on anthropometric data or the measured immunological variables. The data from this small explorative study indicate that early colonization of the intestine might have long-lasting effects on the composition of the intestinal microbiota

    Splanchnic vein thrombosis : current perspectives

    Splanchnic vein thrombosis (SVT) including portal, mesenteric, splenic vein thrombosis and the Budd-Chiari syndrome, is a manifestation of unusual site venous thromboembolism. SVT presents with a lower incidence than deep vein thrombosis of the lower limbs and pulmonary embolism, with portal vein thrombosis and Budd-Chiari syndrome being respectively the most and the least common presentations of SVT. SVT is classified as provoked if secondary to a local or systemic risk factor, or unprovoked if the causative trigger cannot be identified. Diagnostic evaluation is often affected by the lack of specificity of clinical manifestations: the presence of one or more risk factors in a patient with a high clinical suspicion may suggest the execution of diagnostic tests. Doppler ultrasonography represents the first line diagnostic tool because of its accuracy and wide availability. Further investigations, such as computed tomography and magnetic resonance angiography, should be executed in case of suspected thrombosis of the mesenteric veins, suspicion of SVT-related complications, or to complete information after Doppler ultrasonography. Once SVT diagnosis is established, a careful patient evaluation should be performed in order to assess the risks and benefits of the anticoagulant therapy and to drive the optimal treatment intensity. Due to the low quality and large heterogeneity of published data, guidance documents and expert opinion could direct therapeutic decision, suggesting which patients to treat, which anticoagulant to use and the duration of treatment.peer-reviewe

    Novel genetic association of TNF-α-238 and PDCD1-7209 polymorphisms with long-term non-progressive HIV-1 infection.

    About 2-5% of HIV-1-infected subjects, defined as long-term non-progressors (LTNPs), remain immunologically stable for a long time without treatment. The factors governing this condition are known only in part, and include genetic factors. Thus, we studied 20 polymorphisms of 15 genes encoding proinflammatory and immunoregulatory cytokines, chemokines and their receptors, genes involved in apoptosis, and the gene HCP5. METHODS: We analyzed 47 Caucasian LTNPs infected for &gt;9 years, compared with 131 HIV-1-infected Caucasian patients defined as 'usual progressors'. The genotypes were determined by methods based upon PCR, and the statistical analysis was performed by univariate logistic regression. RESULTS: The well-known CCR5Δ32 del32 allele, the cell death-related TNF-α-238 A and PDCD1-7209 T alleles, and HCP5 rs2395029 G, a non-coding protein associated with the HLA-B*5701, were found positively associated with the LTNP condition. No association was observed for other single nucleotide polymorphisms (SDF-1-801, IL-10-592, MCP-1-2518, CX3CR1 V249I, CCR2V64I, RANTES-403, IL-2-330, IL-1β-511, IL-4-590, FASL IVS3nt-169, FAS-670, FAS-1377, FASL IVS2nt-124, PDCD1-7146, MMP-7-181, and MMP7-153). CONCLUSIONS: The novel genetic associations between allelic variants of genes TNF-α-238 and PDCD1-7209 with the LTNP condition underline the importance of host genetic factors in the progression of HIV-1 infection and in immunological preservation

    Astro MBSE: overview on requirement management approaches for astronomical instrumentation

    Systems Engineering requires the involvement of different engineering disciplines: Software, Electronics, Mechanics (often nowadays together as Mechatronics), Optics etc. Astronomical Instrumentation is no exception to this. A critical point is the handling of the requirements, their tracing, flow down and the interaction with stakeholders (flow up) and subsystems (flow down) in order to have traceable and methodical evolution and management. In the Italian Astronomical Community, we are developing methodologies and tools to share the expertise in this field among the different projects. In this paper we will focus on the requirement management approach among different projects (ground and space based, …). The target and synthesis of tis work will be a support framework for the Requirement management of the Italian Astronomical Community (INAF) projects

    Astro MBSE: model based system engineering synthesized for the Italian astronomical community

    Systems Engineering requires the involvement of different engineering disciplines: Software, Electronics, Mechanics (often nowadays together as Mechatronics), Optics etc. Systems Engineering of Astronomical Instrumentation is no exception to this. A critical point is the handling of the different point of view introduced by these disciplines often related to different tools and cultures. Model Based Systems Engineering (MBSE) approach can help the Systems Engineer to always have a complete view of the full system. Moreover, in an ideal situation, all of the information resides in the model thus allowing different views of the System without having to resort to different sources of information, often outdated. In the real world, however, this does not happen because the different actors (Optical Designers, Mechanical Engineers, Astronomers etc.) should adopt the same language and this is clearly, at least nowadays and for the immediate future, close to impossible. In the Italian Astronomical Community, we are developing methodologies and tools to share the expertise in this field among the different projects. In this paper we present the status of this activity that aims to deliver to the community proper tools and template to enable a uniformed use of MBSE (friendly name Astro MBSE) among different projects (ground and space based, …). We will analyze here different software and different approaches. The target and synthesis of this work will be a support framework for the MBSE based system Engineering activity to the Italian Astronomical Community (INAF)

    Definition of the Immune Parameters Related to COVID-19 Severity

    A relevant portion of patients with disease caused by the severe acute respiratory syndrome coronavirus 2 (COVID-19) experience negative outcome, and several laboratory tests have been proposed to predict disease severity. Among others, dramatic changes in peripheral blood cells have been described. We developed and validated a laboratory score solely based on blood cell parameters to predict survival in hospitalized COVID-19 patients. We retrospectively analyzed 1,619 blood cell count from 226 consecutively hospitalized COVID-19 patients to select parameters for inclusion in a laboratory score predicting severity of disease and survival. The score was derived from lymphocyte- and granulocyte-associated parameters and validated on a separate cohort of 140 consecutive COVID-19 patients. Using ROC curve analysis, a best cutoff for score of 30.6 was derived, which was associated to an overall 82.0% sensitivity (95% CI: 78–84) and 82.5% specificity (95% CI: 80–84) for detecting outcome. The scoring trend effectively separated survivor and non-survivor groups, starting 2 weeks before the end of the hospitalization period. Patients’ score time points were also classified into mild, moderate, severe, and critical according to the symptomatic oxygen therapy administered. Fluctuations of the score should be recorded to highlight a favorable or unfortunate trend of the disease. The predictive score was found to reflect and anticipate the disease gravity, defined by the type of the oxygen support used, giving a proof of its clinical relevance. It offers a fast and reliable tool for supporting clinical decisions and, most important, triage in terms of not only prioritization but also allocation of limited medical resources, especially in the period when therapies are still symptomatic and many are under development. In fact, a prolonged and progressive increase of the score can suggest impaired chances of survival and/or an urgent need for intensive care unit admission
