    Breastfeeding Friendly Airport to Support Healthy Tourism

    Motivation for breastfeeding and social support is very important for the success of exclusive breastfeeding. Breastfeeding support and motivation is obtained from family, leaders and the government. The implication of the government support given is the existence of regulations for organizer of public facilities that stating that the owner of public facilities must support the exclusive breastfeeding program at the airport. The airport is an important means for the mobility of both domestic and foreign tourist. The role of airport is very important to realizing healthy tourism. This study used a case study design. The subjects of this study were the leaders, staff and visitors of Adisutjipto International Airport. The number of informants is five people. The research instruments were interview guides and observation forms. Knowledge, attitudes and informant support for the exclusive breastfeeding program are good. There needs to be a stronger promotion effort for all levels of society so that exclusive breastfeeding programs can be achieved. Provision of nursery rooms at Adisutjipto International Airport is in accordance with government standards with facilities such as tables, chairs, baby boxes, washing stand, dispensers and trash can.     Perbedaan Efektivitas Chair-Based Exercise dan Senam Lansia dalam Meningkatkan Fleksibilitas Lumbal Lanjut Usia

    Latar belakang: Fleksibilitas adalah kemampuan sendi bergerak melalui lingkup gerak tanpa nyeri. Senam Lansia (SL) merupakan salah satu usaha untuk mempertahankan dan meningkatkan fleksibilitas lumbal lanjut usia, yang dilakukan dalam posisi berdiri dan memberikan hasil yang baik terhadap fleksibilitas. Beberapa masalah terjadi pada lanjut usia seperti arthritis (khususnya pada sendi lutut) dan penurunan kekuatan otot yang menyebabkan mereka kesulitan untuk beraktivitas dalam posisi berdiri. Chair-Based Exercise (CBE) yang dilakukan dalam posisi duduk, merupakan alternatif latihan yang dapat digunakan untuk meningkatkan fleksibilitas lumbal lanjut usia. Tujuan: Membuktikan tidak ada perbedaan efektivitas CBE dan SL dalam meningkatkan fleksibilitas lumbal. Metode: Penelitian ini merupakan penelitian Quasi experimental design. Dua puluh dua lansia dari 3 panti werdha yang berusia 60-74 tahun dibagi menjadi 2 kelompok. Kelompok CBE (n=12) mendapatkan Chair-Based Exercise dan kelompok SL (n=10) mendapatkan senam lansia. CBE merupakan latihan aerobik yang dilakukan dalam posisi duduk, terdiri dari 3 tahapan: pemanasan, inti dan pendinginan. Sedangkan Senam lansia adalah latihan aerobik yang dilakukan dalam posisi berdiri, terdiri dari 3 tahapan: pemanasan, inti, pendinginan. Kedua latihan berdurasi 30 menit untuk 1 sesi latihan, dilakukan sebanyak 5 kali seminggu selama 6 minggu. Fleksibilitas lumbal dinilai sebelum dan setelah perlakuan dengan menggunakan Modified Back Saver Sit and Reach Test (MBSSRT). Hasil: Tidak terdapat perbedaan yang bermakna delta nilai MBSSRT tungkai kanan dan kiri antara kelompok CBE dan SL setelah diberikan latihan. Kesimpulan: Tidak terdapat perbedaan antara CBE dan SL dalam meningkatkan fleksibilitas lumbal lanjut usia. Kata kunci: Chair-Based Exercise, Senam Lansia, fleksibilitas lumbal, lanjut usia

    The surgical safety checklist and patient outcomes after surgery: a prospective observational cohort study, systematic review and meta-analysis

    © 2017 British Journal of Anaesthesia Background: The surgical safety checklist is widely used to improve the quality of perioperative care. However, clinicians continue to debate the clinical effectiveness of this tool. Methods: Prospective analysis of data from the International Surgical Outcomes Study (ISOS), an international observational study of elective in-patient surgery, accompanied by a systematic review and meta-analysis of published literature. The exposure was surgical safety checklist use. The primary outcome was in-hospital mortality and the secondary outcome was postoperative complications. In the ISOS cohort, a multivariable multi-level generalized linear model was used to test associations. To further contextualise these findings, we included the results from the ISOS cohort in a meta-analysis. Results are reported as odds ratios (OR) with 95% confidence intervals. Results: We included 44 814 patients from 497 hospitals in 27 countries in the ISOS analysis. There were 40 245 (89.8%) patients exposed to the checklist, whilst 7508 (16.8%) sustained ≥1 postoperative complications and 207 (0.5%) died before hospital discharge. Checklist exposure was associated with reduced mortality [odds ratio (OR) 0.49 (0.32–0.77); P\u3c0.01], but no difference in complication rates [OR 1.02 (0.88–1.19); P=0.75]. In a systematic review, we screened 3732 records and identified 11 eligible studies of 453 292 patients including the ISOS cohort. Checklist exposure was associated with both reduced postoperative mortality [OR 0.75 (0.62–0.92); P\u3c0.01; I2=87%] and reduced complication rates [OR 0.73 (0.61–0.88); P\u3c0.01; I2=89%). Conclusions: Patients exposed to a surgical safety checklist experience better postoperative outcomes, but this could simply reflect wider quality of care in hospitals where checklist use is routine
