    Maternal depressive symptoms during and after pregnancy and child developmental milestones

    Abstract Background: Maternal depressive symptoms during and after pregnancy predict poorer child neurodevelopment. The effects of timing, symptom severity, and additive influences remain unclear. Methods: A total of 2,231 mothers of the Prediction and Prevention of Pre‐eclampsia and Intrauterine Growth Restriction (PREDO) study completed the Center for Epidemiological Studies Depression Scale biweekly up to 14 times during pregnancy and twice up to 12 months after pregnancy. At child’s age 1.9–5.7 years, the mothers completed the Beck Depression Inventory‐II on their concurrent depressive symptoms and Ages and Stages Questionnaire on child developmental milestones. Results: Higher mean maternal depressive symptoms, each biweekly score, and consistently clinically relevant symptomatology during pregnancy predicted lower total developmental milestones, fine and gross motor, communication, problem solving, and personal/social skills scores in children. Although maternal depressive symptoms up to 12 months after pregnancy and in early childhood also predicted lower developmental milestones scores, developmental milestones scores were the lowest in children whose mothers’ depressive symptoms were above the clinical cutoff either only during pregnancy, both during and up to 12 months after pregnancy, or at each three time‐points. Conclusion: Maternal depressive symptoms during pregnancy, in the first year postpartum and in early childhood are associated with poorer child neurodevelopment. Our findings further suggest that antenatal and postpregnancy depression have additive effects on neurodevelopment. Children of mothers with the most chronic and severe depressive symptoms during pregnancy had the most neurodevelopmental disadvantages. Our findings emphasize the adverse effects of maternal depression during and after pregnancy and in early childhood on child neurodevelopment

    Disorganized Attachment in Infancy: A Review of the Phenomenon and Its Implications for Clinicians and Policy-Makers

    Disorganized/Disoriented (D) attachment has seen widespread interest from policy makers, practitioners, and clinicians in recent years. However, some of this interest seems to have been based on some false assumptions that (1) attachment measures can be used as definitive assessments of the individual in forensic/child protection settings and that disorganized attachment (2) reliably indicates child maltreatment, (3) is a strong predictor of pathology, and (4) represents a fixed or static trait of the child, impervious to development or help. This paper summarizes the evidence showing that these four assumptions are false and misleading. The paper reviews what is known about disorganized infant attachment and clarifies the implications of the classification for clinical and welfare practice with children. In particular, the difference between disorganized attachment and attachment disorder is examined, and a strong case is made for the value of attachment theory for supportive work with families and for the development and evaluation of evidence-based caregiving interventions

    Executive Functions of Six-Year-Old Boys with Normal Birth Weight and Gestational Age

    Impaired fetal development, reflected by low birth weight or prematurity, predicts an increased risk for psychopathology, especially attention deficit hyperactivity disorder (ADHD). Such effects cut across the normal range of birth weight and gestation. Despite the strength of existing epidemiological data, cognitive pathways that link fetal development to mental health are largely unknown. In this study we examined the relation of birth weight (>2500 g) and gestational age (37–41 weeks) within the normal range with specific executive functions in 195 Singaporean six-year-old boys of Chinese ethnicity. Birth weight adjusted for gestational age was used as indicator of fetal growth while gestational age was indicative of fetal maturity. Linear regression revealed that increased fetal growth within the normal range is associated with an improved ability to learn rules during the intra/extra-dimensional shift task and to retain visual information for short period of time during the delayed matching to sample task. Moreover, faster and consistent reaction times during the stop-signal task were observed among boys born at term, but with higher gestational age. Hence, even among boys born at term with normal birth weight, variations in fetal growth and maturity showed distinct effects on specific executive functions

    L'attaccamento va in tribunale: protezione e affidamento dei minori

    In molti contesti professionali, compreso quello del Tribunale per i minorenni, si fa riferimento alla teoria dell’attaccamento e alla relativa ricerca, con fraintendimenti ampiamente diffusi che spesso si traducono in applicazioni scorrette. La finalità di questa dichiarazione di consenso è, pertanto, quella di migliorarne la comprensione, contrastare la disinformazione a riguardo e guidarne l’uso nel contesto del tribunale per i minorenni secondo una modalità basata sulle evidenze, considerando in particolare i processi decisionali circa la protezione e l’affidamento dei minori. L’articolo è diviso in due parti. Nella prima ci occupiamo dei problemi relativi all’utilizzo di teoria e ricerca sull’attaccamento nel contesto del Tribunale per i minorenni e ne discutiamo le ragioni. A questo proposito, esaminiamo le applicazioni della teoria che si ispirano al principio elettivo del superiore interesse del minore, discutiamo i fraintendimenti a riguardo e identifichiamo i fattori che ne hanno ostacolato un’accurata implementazione. Nella seconda parte, forniamo indicazioni per una sua adeguata e corretta applicazione. A tal fine, siamo partiti da tre principi di riferimento: il bisogno del bambino di caregiver familiari e non abusanti, il valore della continuità di cure sufficientemente buone e i benefici delle reti di relazioni di attaccamento. Discutiamo, inoltre, di quanto le valutazioni sulla qualità dell’attaccamento e sul comportamento di cura siano adeguate a ispirare i processi decisionali forensi rivolti ai minori. Concludiamo che la valutazione dei comportamenti di cura dovrebbe ricoprire un ruolo centrale. Nonostante non ci sia fra noi completo consenso riguardo all’utilizzo delle valutazioni sulla qualità dell’attaccamento nelle decisioni attinenti all’affidamento e alla protezione del minore, tali valutazioni si rivelano, al momento, le più adatte a individuare obiettivi e modalità degli interventi di sostegno. Infine, offriamo indicazioni per organizzare le future collaborazioni di ricerca interdisciplinare

    Associations between DSM-IV diagnosis, psychiatric symptoms and morning cortisol levels in a community sample of adolescents

    Purpose. Dysfunction of the hypothalamic-pituitary-adrenocortical axis (HPA-axis) is implicated in a variety of psychiatric and emotional disorders. In this study, we explore the association between HPA-axis functioning, as measured by morning cortisol, and common psychiatric disorders and symptoms among a community sample of adolescents. Method. Data from a cross-sectional school-based survey of 501 school pupils, aged 15, were used to establish the strength of association between salivary morning cortisol and both diagnosis of psychiatric disorders and a number of psychiatric symptoms, as measured via a computerised psychiatric interview. Analysis, conducted separately by gender, used multiple regressions, adjusting for relevant confounders. Results-á-áWith one exception (a positive association between conduct disorder symptoms and cortisol among females) there was no association between morning cortisol and psychiatric diagnosis or symptoms. However, there was a significant two-way interaction between gender and conduct symptoms, with females showing a positive and males a negative association between cortisol and conduct symptoms. A further three-way interaction showed that while the association between cortisol and conduct symptoms was negative among males with a few mood disorder symptoms, among females with many mood symptoms it was positive. Conclusions. Except in relation to conduct symptoms, dysregulation of morning cortisol levels seems unrelated to any psychiatric disorder or symptoms. However, the relationship between cortisol and conduct symptoms is moderated by both gender and mood symptoms. Findings are compatible with the recent work suggesting research should concentrate on the moderated associations between gender, internalising and externalising symptoms and cortisol, rather than any simple relationship

    Association between perinatal methylation of the neuronal differentiation regulator HES1 and later childhood neurocognitive function and behaviour

    Background: Early life environments induce long-term changes in neurocognitive development and behaviour. In animal models, early environmental cues affect neuropsychological phenotypes via epigenetic processes but as yet there is little direct evidence for such mechanisms in humans. Method: We examined the relation between DNA methylation at birth and child neuropsychological outcomes in two culturally diverse populations using a genome-wide methylation analysis and validation by pyrosequencing. Results: Within the UK Southampton Women’s Survey (SWS) we first which identified 41 differentially methylated regions of interest (DMROI) at birth associated with child’s full-scale IQ at age 4-years. Associations between HES1 DMROI methylation and later cognitive function were confirmed by pyrosequencing in 175 SWS children. Consistent with these findings, higher HES1 methylation was associated with higher executive memory function in a second independent group of 200 SWS seven-year olds. Finally, we examined a pathway for this relationship within a Singaporean cohort (n=108). Here, HES1 DMROI methylation predicted differences in early infant behavior, known to be associated with academic success. In vitro, methylation of HES1 inhibited ETS transcription factor binding, suggesting a functional role of this site. Conclusions: Thus, our findings suggest that perinatal epigenetic processes mark later neuro-cognitive function and behavior, providing support for a role of epigenetic processes in mediating the long-term consequences of early life environment on cognitive development. <br/
