Learning to rank from medical imaging data
Medical images can be used to predict a clinical score coding for the
severity of a disease, a pain level or the complexity of a cognitive task. In
all these cases, the predicted variable has a natural order. While a standard
classifier discards this information, we would like to take it into account in
order to improve prediction performance. A standard linear regression does
model such information, however the linearity assumption is likely not be
satisfied when predicting from pixel intensities in an image. In this paper we
address these modeling challenges with a supervised learning procedure where
the model aims to order or rank images. We use a linear model for its
robustness in high dimension and its possible interpretation. We show on
simulations and two fMRI datasets that this approach is able to predict the
correct ordering on pairs of images, yielding higher prediction accuracy than
standard regression and multiclass classification techniques
"A whole way of life": ontology of culture from Raymond Williams's perspective
An overall understanding of culture, both the culture of community one lives in and the culture of communities one communicates with, seems to be important for people to live their lives under the shelter of peace. This study hands over and foregrounds what people should notice when they face with their own and other nation’s culture in order to understand it better and prevent probable problems. Knowing about the essence of one's own culture, the person can protect it while it is being attacked by other cultures. It is predicted that by being aware of all the criteria just mentioned, people can both protect their own genuine culture and communicate with other communities, with different cultures, without facing with or creating crucial problems; as a result, they can live peacefully and help the matter of globalization. The main goal of this study is to present ontology of culture through which people would be able to get how to know their own and other's cultures. This knowledge helps them to communicate properly by knowing about what aspects of culture they should focus on when facing other cultures in order not to create any crucial problem

Debris collection from implosion of microballoons
Recovery of krypton from implosion of glass microballoons has been studied in the development of a radiochemical diagnostic for determination of /sub fuel/. Collection onto metal surfaces following implosions performed on the OMEGA laser with 1-3 TW (1-2 kJ) of 0.35 ..mu..m light is consistent with an ion implantation mechanism. The dependence of the intrinsic collection efficiency on the energy fluence to the collector surface and its variation in implosions carried out under the same nominal conditions indicate ion energies extending to at least 0.1 MeV and energy distribution functions that are sensitive to the details of the implosion dynamics. Intrinsic sticking efficiencies approaching 0.5 can be obtained in the limit of low total energy fluence to the collector surface (less than or equal to 0.1 J cm/sup -2/)
The orbit structure of Dynkin curves
Let G be a simple algebraic group over an algebraically closed field k;
assume that Char k is zero or good for G. Let \cB be the variety of Borel
subgroups of G and let e in Lie G be nilpotent. There is a natural action of
the centralizer C_G(e) of e in G on the Springer fibre \cB_e = {B' in \cB | e
in Lie B'} associated to e. In this paper we consider the case, where e lies in
the subregular nilpotent orbit; in this case \cB_e is a Dynkin curve. We give a
complete description of the C_G(e)-orbits in \cB_e. In particular, we classify
the irreducible components of \cB_e on which C_G(e) acts with finitely many
orbits. In an application we obtain a classification of all subregular orbital
varieties admitting a finite number of B-orbits for B a fixed Borel subgroup of

Association of HLA types A1-B8-DR3 and B27 with rapid and slow progression of HIV disease
We examined how HLA types A1-B8-DR3 and B27 were related to progression of clinical disease and rate of loss of CD4 lymphocytes in the Edinburgh City Hospital cohort of HIV-positive patients, mainly injection drug users. Patients (n = 692) were prospectively followed from 1985 through March 1994. Accurately estimated seroconversion times were determined retrospectively for a subgroup of 313 (45%). Of 262 patients (39%) who were fully or partially HLA typed, 155 (50%) had known seroconversions. Of 34 patients typed positive for A1-B8-DR3, 29 progressed to CDC stage IV, 22 to AIDS and 20 died. Twelve patients were typed positive for B27; six of these progressed to CDC stage IV, one to AIDS and none died. In a proportional hazards analysis of the 313 patients with known seroconversions, A1-B8-DR3 was significantly associated with covariate-adjusted relative risks of 3.7 (95% CI 1.9-7.2), 3.1 (1.6-6.0) and 1.9 (1.1-3.2) for progression from seroconversion to death, AIDS and CDC stage IV, respectively. Events for B27 were too rare to include B27 in analyses to death and AIDS, but B27 was significantly associated with slower progression to CDC stage IV (0.3, CI 0.1-0.9). Random effects growth curve models were used to estimate individual rates of loss of square root CD4 count and loss of CD4 percentage, for 603 and 617 patients, respectively. A1-B8-DR3 was associated with rapid loss of both markers (p=0.02 and p = 0.01, respectively); B27 was associated with slow loss of both markers (p=0.04 and p<0.005
Hurricane Charley Exposure and Hazard of Preterm Delivery, Florida 2004
Objective Hurricanes are powerful tropical storm systems with high winds which influence many health effects. Few studies have examined whether hurricane exposure is associated with preterm delivery. We aimed to estimate associations between maternal hurricane exposure and hazard of preterm delivery. Methods We used data on 342,942 singleton births from Florida Vital Statistics Records 2004–2005 to capture pregnancies at risk of delivery during the 2004 hurricane season. Maternal exposure to Hurricane Charley was assigned based on maximum wind speed in maternal county of residence. We estimated hazards of overall preterm delivery (<37 gestational weeks) and extremely preterm delivery (<32 gestational weeks) in Cox regression models, adjusting for maternal/pregnancy characteristics. To evaluate heterogeneity among racial/ethnic subgroups, we performed analyses stratified by race/ethnicity. Additional models investigated whether exposure to multiples hurricanes increased hazard relative to exposure to one hurricane. Results Exposure to wind speeds ≥39 mph from Hurricane Charley was associated with a 9 % (95 % CI 3, 16 %) increase in hazard of extremely preterm delivery, while exposure to wind speed ≥74 mph was associated with a 21 % (95 % CI 6, 38 %) increase. Associations appeared greater for Hispanic mothers compared to non-Hispanic white mothers. Hurricane exposure did not appear to be associated with hazard of overall preterm delivery. Exposure to multiple hurricanes did not appear more harmful than exposure to a single hurricane. Conclusions Hurricane exposure may increase hazard of extremely preterm delivery. As US coastal populations and hurricane severity increase, the associations between hurricane and preterm delivery should be further studied
Interpersonal interactions for haptic guidance during maximum forward reaching
Caregiver-patient interactions rely on interpersonal coordination (IPC) involving the haptic and visual modalities. We investigated in healthy individuals spontaneous IPC during joint maximum forward reaching. A 'contact-provider' (CP; n=2) kept light interpersonal touch (IPT) laterally with the wrist of the extended arm of a forward reaching, blind-folded 'contact-receiver' (CR; n=22). Due to the stance configuration, CP was intrinsically more stable. CR received haptic feedback during forward reaching in two ways: (1) presence of a light object (OBT) at the fingertips, (2) provision of IPT. CP delivered IPT with or without vision or tracked manually with vision but without IPT. CR's variabilities of Centre-of-Pressure velocity (CoP) and wrist velocity, interpersonal cross-correlations and time lags served as outcome variables. OBT presence increased CR's reaching amplitude and reduced postural variability in the reach end-state. CR's variability was lowest when CP applied IPT without vision. OBT decreased the strength of IPC. Correlation time lags indicated that CP retained a predominantly reactive mode with CR taking the lead. When CP had no vision, presumably preventing an effect of visual dominance, OBT presence made a qualitative difference: with OBT absent, CP was leading CR. This observation might indicate a switch in CR's coordinative strategy by attending mainly to CP's haptic 'anchor'. Our paradigm implies that in clinical settings the sensorimotor states of both interacting partners need to be considered. We speculate that haptic guidance by a caregiver is more effective when IPT resembles the only link between both partners
The Structure of a Rigorously Conserved RNA Element within the SARS Virus Genome
We have solved the three-dimensional crystal structure of the stem-loop II motif (s2m) RNA element of the SARS virus genome to 2.7-Å resolution. SARS and related coronaviruses and astroviruses all possess a motif at the 3′ end of their RNA genomes, called the s2m, whose pathogenic importance is inferred from its rigorous sequence conservation in an otherwise rapidly mutable RNA genome. We find that this extreme conservation is clearly explained by the requirement to form a highly structured RNA whose unique tertiary structure includes a sharp 90° kink of the helix axis and several novel longer-range tertiary interactions. The tertiary base interactions create a tunnel that runs perpendicular to the main helical axis whose interior is negatively charged and binds two magnesium ions. These unusual features likely form interaction surfaces with conserved host cell components or other reactive sites required for virus function. Based on its conservation in viral pathogen genomes and its absence in the human genome, we suggest that these unusual structural features in the s2m RNA element are attractive targets for the design of anti-viral therapeutic agents. Structural genomics has sought to deduce protein function based on three-dimensional homology. Here we have extended this approach to RNA by proposing potential functions for a rigorously conserved set of RNA tertiary structural interactions that occur within the SARS RNA genome itself. Based on tertiary structural comparisons, we propose the s2m RNA binds one or more proteins possessing an oligomer-binding-like fold, and we suggest a possible mechanism for SARS viral RNA hijacking of host protein synthesis, both based upon observed s2m RNA macromolecular mimicry of a relevant ribosomal RNA fold
Establishing key criteria to define and compare models of specialist palliative care : a mixed-methods study using qualitative interviews and Delphi survey
Background:
Specialist palliative care services have various configurations of staff, processes and interventions, which determine how care is delivered. Currently, there is no consistent way to define and distinguish these different models of care.
Aim:
To identify the core components that characterise and differentiate existing models of specialist palliative care in the United Kingdom.
Design:
Mixed-methods study: (1) semi-structured interviews to identify criteria, (2) two-round Delphi study to rank/refine criteria, and (3) structured interviews to test/refine criteria.
Setting/participants:
Specialist palliative care stakeholders from hospice inpatient, hospital advisory, and community settings.
Results:
(1) Semi-structured interviews with 14 clinical leads, from eight UK organisations (five hospice inpatient units, two hospital advisory teams, five community teams), provided 34 preliminary criteria.
(2) Delphi study: Round 1 (54 participants): thirty-four criteria presented, seven removed and seven added. Round 2 (30 participants): these 34 criteria were ranked with the 15 highest ranked criteria, including setting, type of care, size of service, diagnoses, disciplines, mode of care, types of interventions, ‘out-of-hours’ components (referrals, times, disciplines, mode of care, type of care), external education, use of measures, bereavement follow-up and complex grief provision.
(3) Structured interviews with 21 UK service leads (six hospice inpatients, four hospital advisory and nine community teams) refined the criteria from (1) and (2), and provided four further contextual criteria (team purpose, funding, self-referral acceptance and discharge).
Conclusion:
In this innovative study, we derive 20 criteria to characterise and differentiate models of specialist palliative care – a major paradigm shift to enable accurate reporting and comparison in practice and research
