    The effects of dietary counseling on children with food allergy: a prospective, multicenter intervention study.

    Although dietary counseling is generally recommended in children with food allergy (FA), its effect on the nutritional status of these patients has not yet been evaluated. Our nonrandomized multicenter prospective intervention study was undertaken to investigate the effects of dietary counseling on children with FA. Anthropometric data, dietary intakes, and laboratory biomarkers of nutritional status were evaluated in children with FA (aged 6 to 36 months) before and after dietary counseling, by multidisciplinary teams composed of pediatricians, dietitians, and nurses. Ninety-one children with FA (49 boys and 42 girls; mean age 18.9 months, 95% CI 16.5 to 21.3) were evaluated; 66 children without FA (41 boys and 25 girls; mean age 20.3 months, 95% CI 17.7 to 22.8) served as controls providing baseline values only. At enrollment, energy and protein intakes were lower in children with FA (91 kcal/kg/day, interquartile range [IQR]=15.1, minimum=55.2, maximum=130.6; and 2.2 g/kg/day, IQR=0.5, minimum=1.5, maximum=2.7, respectively) than in children without FA (96 kcal/kg/day, IQR=6.1, minimum=83.6, maximum=118.0; and 4.6 g/kg/day, IQR=1.2, minimum=2.0, maximum=6.1, respectively; P<0.001). A weight to length ratio <2 standard deviations was more frequent in children with FA than in children without FA (21% vs 3%; P<0.001). At 6 months following dietary counseling, the total energy intake of children with FA was similar to the baseline values of control children. Dietary counseling also resulted in a significant improvement of their anthropometric and laboratory biomarkers of nutritional status.     Ocean model resolution dependence of Caribbean sea-level projections

    Abstract Sea-level rise poses severe threats to coastal and low-lying regions around the world, by exacerbating coastal erosion and flooding. Adequate sea-level projections over the next decades are important for both decision making and for the development of successful adaptation strategies in these coastal and low-lying regions to climate change. Ocean components of climate models used in the most recent sea-level projections do not explicitly resolve ocean mesoscale processes. Only a few effects of these mesoscale processes are represented in these models, which leads to errors in the simulated properties of the ocean circulation that affect sea-level projections. Using the Caribbean Sea as an example region, we demonstrate a strong dependence of future sea-level change on ocean model resolution in simulations with a global climate model. The results indicate that, at least for the Caribbean Sea, adequate regional projections of sea-level change can only be obtained with ocean models which capture mesoscale processes.info:eu-repo/semantics/publishe

    Sustainability of grape-ethanol energy chain

    The aim of this work is to evaluate the sustainability, in terms of greenhouse gases emission saving, of a new potential bio-ethanol production chain in comparison with the most common ones. The innovation consists of producing bio-ethanol from different types of no-food grapes, while usually bio-ethanol is obtained from matrices taken away from crop for food destination: sugar cane, corn, wheat, sugar beet. In the past, breeding programs were conducted with the aim of improving grapevine characteristics, a large number of hybrid vine varieties were produced and are nowadays present in the Viticulture Research Centre (CRA-VIT) Germplasm Collection. Some of them are potentially interesting for bio-energy production because of their high production of sugar, good resistance to diseases, and ability to grow in marginal lands. Life cycle assessment (LCA) of grape ethanol energy chain was performed following two different methods: i) using the spreadsheet BioGrace, developed within the Intelligent Energy Europe program to support and to ease the Renewable Energy Directive 2009/28/EC implementation; ii) using a dedicated LCA software. Emissions were expressed in CO2 equivalent (CO2eq). These two tools gave very similar results. The overall emissions impact of ethanol production from grapes on average is about 33 g CO2eq MJ–1 of ethanol if prunings are used for steam production and 53 g CO2eq MJ–1 of ethanol if methane is used. The comparison with other bio-energy chains points out that the production of ethanol using grapes represents an intermediate situation in terms of general emissions among the different production chains. The results showed that the sustainability limits provided by the normative are respected to this day. On the contrary, from 2017 this production will be sustainable only if the transformation processes will be performed using renewable sources of energy

    Influence of LV Neutral Grounding on Global Earthing Systems

    International Standards define a Global Earthing System as an earthing net created interconnecting local Earthing Systems (generally through the shield of MV cables and/or bare buried conductors). In Italy the Regulatory Authority for Electricity and Gas requires Distributors to guarantee the electrical continuity of LV neutral conductor. This requirement has led to the standard practice of realizing “reinforcement groundings” along the LV neutral conductor path and at users’ delivery cabinet. Moreover, in urban high load scenarios (prime candidates to be part of a Global Earthing System), it is common that LV distribution scheme creates, trough neutral conductors, an effective connection between grounding systems of MV/LV substations, modifying Global Earthing System consistency. Aim of this paper is to evaluate the effect, in terms of electrical safety, of the above mentioned LV neutral distribution scheme when an MV-side fault to ground occurs. At this purpose simulations are carried out on a realistic urban test case and suitable evaluation indexes are proposed

    A Practical Method to Test the Safety of HV/MV Substation Grounding System

    The adequacy of a Grounding System (GS) to the safety conditions has to be periodically tested by measurements. The test methods and techniques used to verify the electrical characteristics of the GS include the measurements of step and touch voltages. The goal of the test is to verify that touch voltage and step voltage remain below a safe value in all the zones of the installation. The measurements can present some operational difficulties. The purpose of this paper is to present the procedure, step-by-step, of a practical method of measuring touch/step voltages in grounding systems located in urban or industrial areas with reduced accessibility. The suggested method uses auxiliary current electrodes located at short distances. This paper demonstrates by test measurements done in a real case that the method provides conservative results

    Testing the cognitive-behavioural maintenance models across DSM-5 bulimic-type eating disorder diagnostic groups: A multi-centre study

    The original cognitive-behavioural (CB) model of bulimia nervosa, which provided the basis for the widely used CB therapy, proposed that specific dysfunctional cognitions and behaviours maintain the disorder. However, amongst treatment completers, only 40–50 % have a full and lasting response. The enhanced CB model (CB-E), upon which the enhanced version of the CB treatment was based, extended the original approach by including four additional maintenance factors. This study evaluated and compared both CB models in a large clinical treatment seeking sample (N = 679), applying both DSM-IV and DSM-5 criteria for bulimic-type eating disorders. Application of the DSM-5 criteria reduced the number of cases of DSM-IV bulimic-type eating disorders not otherwise specified to 29.6 %. Structural equation modelling analysis indicated that (a) although both models provided a good fit to the data, the CB-E model accounted for a greater proportion of variance in eating-disordered behaviours than the original one, (b) interpersonal problems, clinical perfectionism and low self-esteem were indirectly associated with dietary restraint through over-evaluation of shape and weight, (c) interpersonal problems and mood intolerance were directly linked to binge eating, whereas restraint only indirectly affected binge eating through mood intolerance, suggesting that factors other than restraint may play a more critical role in the maintenance of binge eating. In terms of strength of the associations, differences across DSM-5 bulimic-type eating disorder diagnostic groups were not observed. The results are discussed with reference to theory and research, including neurobiological findings and recent hypotheses

    Psychological treatments and psychotherapies in the neurorehabilitation of pain. Evidences and recommendations from the italian consensus conference on pain in neurorehabilitation

    BACKGROUND: It is increasingly recognized that treating pain is crucial for effective care within neurological rehabilitation in the setting of the neurological rehabilitation. The Italian Consensus Conference on Pain in Neurorehabilitation was constituted with the purpose identifying best practices for us in this context. Along with drug therapies and physical interventions, psychological treatments have been proven to be some of the most valuable tools that can be used within a multidisciplinary approach for fostering a reduction in pain intensity. However, there is a need to elucidate what forms of psychotherapy could be effectively matched with the specific pathologies that are typically addressed by neurorehabilitation teams. OBJECTIVES: To extensively assess the available evidence which supports the use of psychological therapies for pain reduction in neurological diseases. METHODS: A systematic review of the studies evaluating the effect of psychotherapies on pain intensity in neurological disorders was performed through an electronic search using PUBMED, EMBASE, and the Cochrane Database of Systematic Reviews. Based on the level of evidence of the included studies, recommendations were outlined separately for the different conditions. RESULTS: The literature search yielded 2352 results and the final database included 400 articles. The overall strength of the recommendations was medium/low. The different forms of psychological interventions, including Cognitive-Behavioral Therapy, cognitive or behavioral techniques, Mindfulness, hypnosis, Acceptance and Commitment Therapy (ACT), Brief Interpersonal Therapy, virtual reality interventions, various forms of biofeedback and mirror therapy were found to be effective for pain reduction in pathologies such as musculoskeletal pain, fibromyalgia, Complex Regional Pain Syndrome, Central Post-Stroke pain, Phantom Limb Pain, pain secondary to Spinal Cord Injury, multiple sclerosis and other debilitating syndromes, diabetic neuropathy, Medically Unexplained Symptoms, migraine and headache. CONCLUSIONS: Psychological interventions and psychotherapies are safe and effective treatments that can be used within an integrated approach for patients undergoing neurological rehabilitation for pain. The different interventions can be specifically selected depending on the disease being treated.
