    Abstract Background: Self-care is a nursing sensitive outcome. In Portugal there is a lack of validated scales to assess self-care. Scales have their own validation methods to be used in cultural contexts which are different from their original contexts. Aim: To translate, adapt, validate, and assess the psychometric properties of the Therapeutic Self-Care Scale (Doran & Sidani, 2005) for the Portuguese population. Methodology: A cross-sectional study was carried out on a sample of 990 inpatients in surgical and medical wards in 4 hospitals of the central region of Portugal. The Therapeutic Self-Care Scale was administered by nurses in interview format between March and July 2012. This scale measures the patient's ability to perform 4 different categories of self-care activities. Results: n=990; mean age=63.57 years, and SD=17.14. Keiser-Meyer-Olkin (KMO) =0.948, Bartlett's Test of Sphericity: p <0.001. The principal component analysis concluded on a one-dimensional structure, in which a single factor explained 81.318 of the total variance. Internal validity: Cronbach' α=0.979. Conclusions: The Therapeutic Self-care Scale showed valid psychometric properties making it suitable for the Portuguese population

    Objetiva-se com o presente artigo a análise dos reflexos provocados pela sentença trabalhista perante o órgão responsável pela previdência social, denominado de INSS. Isto porque, a eficácia de tal sentença tem gerado polêmicas na jurisprudência trabalhista brasileira, pois há uma resistência por parte dos órgãos administrativos previdenciários, no sentido de não reconhecer a sentença e o acordo judicial trabalhista como prova única do tempo de serviço para fins de concessão de benefí­cios. Todavia, após a EC nº 20 de 1998, houve uma ampliação da competência da Justiça do Trabalho para executar as contribuições previdenciárias decorrentes de suas decisões, tornando a partir daí­ um grande avanço de inegável valia e qualificação desta Justiça Especializada, pois além de resguardar os direitos dos trabalhadores, também serviu de meio fiscalizatório, e tão importante quanto, foram os montantes que se passou a arrecadar para os cofres da Autarquia Previdenciária vertidos da execução perante a Justiça do Trabalho, o que sem dúvidas auxilia sobremaneira o sustento da Seguridade Social.     He-Ne Laser Effects on Blood Microcirculation During Wound Healing: A Method of In Vivo Study Through Laser Doppler Flowmetry

    Background and Objectives: Low-intensity laser therapy (LILT) is widely used for wound healing promotion and its mechanism of action may be due to an enhancement of blood supply. The aim of this study was to evaluate blood flow alterations in a wound healing model, using laser Doppler flowmetry (LDF) associated with a normalized perfusion parameter. Study Design/Materials and Methods: An injury was provoked in 15 rats and blood flow was measured periodically over a period of 21 days. Control groups were established to evaluate LDF and He-Ne laser effects on microcirculation. A 1 J/cm 2 dose was utilized, with 6 mW/ cm 2 irradiance. Results: The results demonstrated flow alterations provoked by lesion, and inflammatory response (P < 0.05). There were no statistical differences between groups. Conclusions: The results did not show a significant sustained effect on microcirculation with this He-Ne dose

    Global patient outcomes after elective surgery: prospective cohort study in 27 low-, middle- and high-income countries.

    BACKGROUND: As global initiatives increase patient access to surgical treatments, there remains a need to understand the adverse effects of surgery and define appropriate levels of perioperative care. METHODS: We designed a prospective international 7-day cohort study of outcomes following elective adult inpatient surgery in 27 countries. The primary outcome was in-hospital complications. Secondary outcomes were death following a complication (failure to rescue) and death in hospital. Process measures were admission to critical care immediately after surgery or to treat a complication and duration of hospital stay. A single definition of critical care was used for all countries. RESULTS: A total of 474 hospitals in 19 high-, 7 middle- and 1 low-income country were included in the primary analysis. Data included 44 814 patients with a median hospital stay of 4 (range 2-7) days. A total of 7508 patients (16.8%) developed one or more postoperative complication and 207 died (0.5%). The overall mortality among patients who developed complications was 2.8%. Mortality following complications ranged from 2.4% for pulmonary embolism to 43.9% for cardiac arrest. A total of 4360 (9.7%) patients were admitted to a critical care unit as routine immediately after surgery, of whom 2198 (50.4%) developed a complication, with 105 (2.4%) deaths. A total of 1233 patients (16.4%) were admitted to a critical care unit to treat complications, with 119 (9.7%) deaths. Despite lower baseline risk, outcomes were similar in low- and middle-income compared with high-income countries. CONCLUSIONS: Poor patient outcomes are common after inpatient surgery. Global initiatives to increase access to surgical treatments should also address the need for safe perioperative care. STUDY REGISTRATION: ISRCTN5181700
